Comparing Allergist and Cardiologist Considerations for the Optimal Management of Thienopyridines Hypersensitivity.

BACKGROUND AND OBJECTIVE: The thienopyridine family includes ticlopidine, clopidogrel and prasugrel which are antiplatelet drugs largely used, mainly associated to aspirin, for treatment of acute coronary syndromes and after percutaneous coronary interventions, to avoid thrombosis. In some patients, thienpyridines may cause hypersensitivity reactions which jeopardize the optimal therapeutic and preventive approach to vascular diseases. The management of thienopyridine hypersensitivity seems to be best done as an interdisciplinary collaboration between the allergist and cardiologist. METHOD: T

Robotically assisted percutaneous coronary intervention: benefits to the patient and the cardiologist.

Robotic percutaneous interventional systems represent the future of interventional cardiology. Robotic technology significantly reduces operator exposure to ionizing radiation, and improves ergonomics in the cardiac catheterization laboratory, thereby preventing orthopedic injuries. It may enable more accurate stent implantation and reduce the incidence of contrast-induced nephropathy. Robotics has the potential of converting the current 'high-risk' catheterization laboratory into a safe, physician- and patient-friendly environment.

Renal angioplasty for atherosclerotic renal artery stenosis: Cardiologist's perspective.

Atherosclerotic renal artery stenosis (ARAS) is frequently associated with concomitant coronary and peripheral arterial disease with a significant impact on cardiovascular morbidity and mortality. Renal angioplasty of ARAS is more challenging because of increased incidence of technical failures, complications, and restenosis; while there is barely perceptible control of hypertension and only marginal improvement in renal function. This is because most of the patient population in recent randomized trials had unmanifested or clinically silent renovascular disease. Manifestations of RAS should b

[Primary cardiac amyloidosis -- condition which can be diagnosed by a cardiologist].

UNLABELLED: Vitiligo is a rare cutaneous adverse event associated with cyclin-dependent kinase 4/6 (CDK4/6) inhibitors. We report a 56-year-old woman with de novo metastatic HR+/HER2- breast cancer who developed vitiligo several years into long-term ribociclib therapy. She was initially treated with palbociclib plus letrozole and later transitioned to a reduced dose of ribociclib due to cytopenias and coverage limitations. After 3 years on ribociclib, she developed asymptomatic hypopigmented patches on her hands and face. Dermatologic evaluation confirmed vitiligo. Treatment was conservative w

Congenital heart block in neonatal lupus: the pediatric cardiologist's perspective.

CLINICAL PRESENTATION: Congenital heart block (CHB) in the absence of major structural abnormalities is associated with maternal antibodies to Ro (SS-A) and La (SS-B). CHB is most commonly diagnosed between 18 and 24 wk of gestation, and may be first, second or third degree (complete). Mortality approaches approximately 20%, and most surviving children require pacemakers. Affected infants may develop cardiomyopathy. Abnormalities in the skin, liver and blood of neonates are also associated with anti-Ro/La antibodies, and are usually self-limiting; these manifestations and CHB are collectively

Cardiac amyloidosis: what every interventional cardiologist should know.

Cardiac amyloidosis (CA) is increasingly encountered by interventional cardiologists during the evaluation of chest pain, myocardial injury, coronary artery disease, atrial arrhythmias, and aortic stenosis. Its recognition can be challenging because amyloid cardiomyopathy often coexists with common coronary, valvular, and arrhythmic conditions. A key diagnostic clue is discordance: the apparent coronary, valvular, or hemodynamic abnormality does not fully explain the patient's symptoms, myocardial phenotype, low-flow physiology, or persistent high-sensitivity cardiac troponin elevation. This r

Patient and Cardiologist Perspectives on the Treatment of Patients With Concomitant ASCVD and Obesity.

Atherosclerotic cardiovascular disease (ASCVD) and obesity pose challenges to patients, healthcare professionals (HCPs), and health systems. The aim of this study was to understand the journey of patients with ASCVD and obesity, their caregivers, and managing HCPs to optimize treatment outcomes. Patients with ASCVD and body mass index ≥ 27 kg/m2 (N = 61), their caregivers (N = 12), and HCPs (N = 24) were given an online survey and participated in 1:1 interviews. Cardiology HCPs (N = 120) were given a quantitative online survey. Despite the presence of risk factors, patients described little co

Long-acting nifedipine in the management of essential hypertension: a review for cardiologists.

Calcium channel antagonists, specifically long-acting nifedipine formulations, play a crucial role in treating hypertension and angina. Originally used for angina, nifedipine has been widely employed as an antihypertensive medication for over 40 years. It offers rapid action and oral bioavailability with minimal maternal or fetal side effects, making it suitable for treating hypertensive crises during pregnancy. However, it can cause a sudden drop in blood pressure and tachycardia. Long-acting formulations, such as gastrointestinal therapeutic systems, gradually release nifedipine over 24 hour

Donation after circulatory death: A transplant cardiologist's take on neuroprognostication.

Donation after circulatory death (DCD) is becoming increasingly utilized in heart transplantation and has the potential to further expand the donor pool. As transplant cardiologists gain more familiarity with DCD donor selection, there are many issues that lack consensus including how we incorporate the neurologic examination, how we measure functional warm ischemic time (fWIT), and what fWIT thresholds are acceptable. DCD donor selection calls for prognostication tools to help determine how quickly a donor may expire, and in current practice there is no standardization in how we make these pr

An assessment of radiation use, knowledge and concerns in trainee cardiologists in Ireland.

OBJECTIVE: To evaluate knowledge, routine use and concerns of trainee cardiologists in the Republic of Ireland regarding radiation use in the cardiac catheterization laboratory. METHODS: We handed out a Radiation Questionnaire to cardiology trainees in February 2020 at the Irish Cardiac Society "Spring Meeting". The questionnaire assessed radiation protection use amongst trainees and tested knowledge of X-ray basics. RESULTS: Many trainees report inadequate access to properly sized lead protection, and infrequent dosimeter usage. Over one-third of trainees report musculoskeletal issues from

Cardiologist's approach to the diabetic patient: No further delay for a paradigm shift.

Type 2 diabetes mellitus (DM) is constantly increasing worldwide and its most critical determinant of morbidity and mortality is still represented by cardiovascular (CV) complications. For years, cardiologists' approach to diabetic patients has been focused on risk factors optimization, with positive results. However, the management of DM per se was never truly considered in order to obtain prevention from major CV events, because medications used for glycemic control were not expected to gain CV benefit. Early trials concerning intensive versus conventional glycemia control did not prove usef

[The adult with tetralogy of fallot: what the clinical cardiologist needs to know].

Tetralogy of Fallot (TOF) is the most common cyanotic congenital heart disease. After more than seven decades of the first palliative surgery, TOF prognosis has changed dramatically. The prevalence of TOF is approximately 3 per 10 000 births, representing 7 to 10% of congenital heart disease. With a higher survival into adulthood, the clinical cardiologist faces challenges in the management of this population, from severe pulmonary regurgitation to heart failure and ventricular arrhythmias. Its prevalence is approximately 3 per 10 000 live births, representing 7 to 10% of congenital heart dise

Hybrid Procedures. Opening Doors for Surgeon and Cardiologist Close Collaboration.

Background: Collaboration between cardiac surgeons and cardiologists can offer interventions that each specialist may not be able to offer on their own. This type of collaboration has been demonstrated with the hybrid Stage I in patients with hypoplastic heart syndrome. Since that time, a hybrid approach to cardiac interventions has been expanded to an incredible variety of potential indications. Methods: Seventy-one patients were scheduled for a hybrid procedure along 8 years. This was defined as close collaboration between surgeon and cardiologist working together in the same room, either ca

Pregnancy in Women with Congenital Heart Disease: A Guide for the General Cardiologist.

Remarkable advances in the care and survival of congenital heart disease (CHD) patients have led to increasing numbers of young women with CHD who carry a pregnancy with significant risk. The profound hemodynamic changes that naturally occur during gestation may unmask CHD or exacerbate an existing condition and place both the woman and fetus in jeopardy. The caring cardiologist should be familiar with the specific lesion and anticipate complications. Pregestational counseling and a multidisciplinary team approach during pregnancy are key for a successful pregnancy and favorable outcomes. In t

Cardiovascular magnetic resonance: applications and practical considerations for the general cardiologist.

Cardiovascular magnetic resonance (CMR) is a rapidly evolving non-invasive imaging modality offering comprehensive, multiparametric assessment of cardiac structure and function in a variety of clinical situations. Cine imaging with CMR is the gold standard non-invasive imaging technique for the quantification of ventricular volumes and systolic function. It also affords superior visualisation of apical and right ventricular morphological abnormalities. In coronary artery disease, CMR stress perfusion imaging identifies functionally significant coronary artery disease with high sensitivity and

Neonatal Aortic Stenosis is a Surgical Disease: An Interventional Cardiologist View.

The application of balloon valvotomy as primary treatment for neonatal congenital aortic stenosis is contentious. In this debate, we discuss data comparing outcomes of a percutaneous and surgical strategy between two tertiary centers that have adopted opposite therapeutic strategies. The outcomes with surgical and balloon therapies appear comparable. These contemporaneous data validate the empiric switch to primary balloon valvotomy in the modern era.

Reversible cerebral vasoconstriction syndromes: what the cardiologist should know.

Over the past decade, the reversible cerebral vasoconstriction syndromes (RCVS) have emerged as a group of conditions with easily recognizable clinical-angiographic features and a usually benign prognosis. The RCVS affect young individuals, mostly women, and the majority present with recurrent, severe, 'thunderclap' headaches. Vascular imaging studies show dynamic and reversible narrowing and dilatation of multiple intracerebral arteries. Brain imaging usually shows no parenchymal lesions, however, approximately one-third of patients develop ischemic or hemorrhagic strokes or reversible brain
