[Pathology of the "primary" cardiomyopathies (author's transl)].

The definition and clinical classification of the cardiomyopathies which the author uses are briefly mentioned, macroscopic, histologic, histochemical and electron microscopic appearances of the three clinical groups are described in detail. Classification is made into: 1. Congestive cardiomyopathy, 2a. hypertrophic cardiomyopathy with obstruction, 2b. hypertrophic cardiomyopathy without obstruction, 3. obliterative or restrictive cardiomyopathy. Löffler's endocarditis parietalis fibroplastica and endomyocardial fibrosis are examples which are compared with eosinophilic myocarditis and patholo

Inflammation in Cardiomyopathies: Cellular Mechanisms Across Cardiac Phenotype.

Cardiomyopathies are traditionally classified by structural and genetic phenotypes, but emerging evidence highlights chronic myocardial inflammation as a pivotal driver of disease progression across different etiologies. This review synthesizes the current literature on the cellular and molecular inflammatory mechanisms underlying hypertrophic cardiomyopathy, Anderson-Fabry disease, cardiac amyloidosis, arrhythmogenic cardiomyopathy, and dilated cardiomyopathy. Across these distinct conditions, endogenous triggers such as metabolic substrates, misfolded amyloid fibrils, mechanical stress, or v

Understanding inherited cardiomyopathies: clinical aspects and genetic determinants.

Cardiomyopathies (CMs) are a clinically heterogeneous group of cardiovascular diseases characterized by structural and functional abnormalities of the heart muscle in the absence of coronary artery disease, hypertension, valve disease, or congenital heart disease as a leading cause. The phenotypic spectrum of CMs ranges from silent heart failure to symptomatic heart failure and sudden cardiac death, and CMs are one of the leading causes of cardiovascular morbidity worldwide. CMs are highly heritable, although a clear distinction between inherited and acquired forms remains challenging, particu

Cardiac MRI Evaluation of Heart Failure and Cardiomyopathies.

Cardiac magnetic resonance imaging (CMR) is an exciting noninvasive imaging modality with increasing utilization in the field of cardiovascular medicine. In conjunction with echocardiogram, computed tomography, and invasive therapies, CMR has provided exceptional capability to further evaluate complex clinical cardiac conditions. CMR provides both anatomical and physiological information of a variety of tissue types, without the need for ionizing radiation. This article aims to first review basic principles of CMR and later discuss MRI's role in diagnosis of cardiomyopathy and treatment implic

Imaging Pitfalls in the Detection of Infiltrative Cardiomyopathies.

BACKGROUND: Despite increasing recognition of infiltrative cardiomyopathies, the heterogeneous group of diseases remains difficult to diagnose. CASE SUMMARY: We present an 82-year-old man with septal basal thickening initially thought to be hypertrophic cardiomyopathy. Further imaging was suspicious for sarcoidosis, but after several years without clinical improvement and the interval development of concentric hypertrophy, he was diagnosed with cardiac amyloidosis. DISCUSSION: Advanced imaging modalities are frequently used to evaluate suspected infiltrative cardiomyopathies. However, the ef

Molecular Basis of Cardiomyopathies in Type 2 Diabetes.

Diabetic cardiomyopathy (DbCM) is a common complication in individuals with type 2 diabetes mellitus (T2DM), and its exact pathogenesis is still debated. It was hypothesized that chronic hyperglycemia and insulin resistance activate critical cellular pathways that are responsible for numerous functional and anatomical perturbations in the heart. Interstitial inflammation, oxidative stress, myocardial apoptosis, mitochondria dysfunction, defective cardiac metabolism, cardiac remodeling, hypertrophy and fibrosis with consequent impaired contractility are the most common mechanisms implicated. Disease features and management of cardiomyopathies in women.

Over the last years, there has been a growing interest in the clinical manifestations and outcomes of cardiomyopathies in women. Peripartum cardiomyopathy is the only women-specific cardiomyopathy. In cardiomyopathies with X-linked transmission, women are not simply healthy carriers of the disorder, but can show a wide spectrum of clinical manifestations ranging from mild to severe manifestations because of heterogeneous patterns of X-chromosome inactivation. In mitochondrial disorders with a matrilinear transmission, cardiomyopathy is part of a systemic disorder affecting both men and women.

Concealed Inherited Cardiomyopathies Detected in Cardio-Oncology Screening.

INTRODUCTION: Basal cardiovascular risk assessment in cardio-oncology is essential. Integrating clinical information, ECG and transthoracic echocardiogram can identify concealed inherited cardiomyopathies (ICMPs) with potential added risk of cardiotoxicity. We aimed to evaluate the impact of our Cardio-Oncology Unit design in detecting concealed ICMPs. METHODS: We carried out a retrospective study of all consecutive breast cancer patients referred to the Cardio-Oncology Unit for cardiac evaluation (2020-2022). ICMPs diagnosis was provided according to ESC guidelines and underwent genetic test

A Review of Catecholamine Associated Cardiomyopathies and Channelopathies.

Conditions associated with states of catecholamine toxicity are pheochromocytoma, paraganglioma, Takotsubo's cardiomyopathy, and catecholaminergic polymorphic ventricular tachycardia. This article defines these conditions along with relevant diagnostic and prognostic factors. The role of catecholamines in these conditions are compared and contrasted. Emphasis is given to the role of endothelial dysfunction as a possible etiologic factor in cardiomyopathy associated with excess catecholamines.

Differentiating Athlete's Heart from Cardiomyopathies - The Right Side.

Arrhythmogenic right ventricular cardiomyopathy (ARVC) is a recognised cause of sudden cardiac death during exercise in young athletes. Competitive exercise is also known to accelerate progression of ARVC and exercise restriction is an important part of disease management. Regular endurance training can induce physiological changes detectable on electrocardiography and imaging which may overlap with pathological findings caused by ARVC, thus making differentiation of athlete's heart from ARVC difficult in some cases. This review will discuss the changes of athlete's heart, particularly as it a

Differentiating Athlete's Heart From Cardiomyopathies - The Left Side.

In athletes who undertake a high volume of high intensity exercise, the resultant changes in cardiac structure and function which develop as a result of physiological adaptation to exercise (so called "Athlete's Heart") may overlap with some features of pathological conditions. This chapter will focus on the left side of the heart, where left ventricular cavity enlargement, increase in left ventricular wall thickness and increased left ventricular trabeculation associated with athletic remodelling may sometimes be difficult to differentiate from conditions such as dilated cardiomyopathy, hyper

Cardiomyopathies in Noonan syndrome and the other RASopathies.

Noonan syndrome and related disorders (Noonan syndrome with multiple lentigines, Costello syndrome, cardiofaciocutaneous syndrome, Noonan syndrome with loose anagen hair, and other related traits) are autosomal dominant traits. Mutations causing these disorders alter proteins relevant for signaling through RAS. Thus, these traits are now collectively called the RASopathies. While the RASopathies have pleiomorphic features, this review will focus on the hypertrophic cardiomyopathy observed in varying percentages of all of these traits. In addition, inherited abnormalities in one pathway gene, R

Evolving Approaches to Genetic Evaluation of Specific Cardiomyopathies.

The understanding of the genetic basis of cardiomyopathy has expanded significantly over the past 2 decades. The increasing availability, shortening diagnostic time, and lowering costs of genetic testing have provided researchers and physicians with the opportunity to identify the underlying genetic determinants for thousands of genetic disorders, including inherited cardiomyopathies, in effort to improve patient morbidities and mortality. As such, genetic testing has advanced from basic scientific research to clinical application and has been incorporated as part of patient evaluations for su

Diagnosis and prognosis of fetal cardiomyopathies: a review.

Cardiomyopathies (CM) are a very rare disease in fetuses with a very poor outcome. Only isolated case reports and small case series were reported. According with published studies we will describe the fetal CM starting from their echocardiographic presentation: dilated cardiomyopathy (DCM) with dilatation of either or both ventricles and impaired ventricular function, and hypertrophic cardiomyopathy (HCM) with different degree of disproportionate hypertrophy of the myocardial walls. The term of the "noncompaction" of the left ventricular myocardium, is used in cases with DCM with evidence of n

Cardiac imaging in myocardial sarcoidosis and other cardiomyopathies.

PURPOSE OF REVIEW: The clinical role of emerging imaging technologies for diagnosing cardiac sarcoidosis and other cardiomyopathies is evolving. An up-to-date review of the role of various imaging modalities in the evaluation of cardiac sarcoidosis and other cardiomyopathies is presented. RECENT FINDINGS: No study prospectively established the accuracy of each of the various techniques for diagnosing myocardial involvement in patients with suspected cardiac sarcoidosis. Cardiac magnetic resonance imaging (CMR) is demonstrated to have a sensitivity of 100% and specificity of approximately 80%,

Cardiomyopathies in children. Clinical, epidemiological and prognostic evaluation.

In this article, the clinical and epidemiological characteristics of 137 children with cardiomyopathy admitted to the Hacettepe Pediatric Cardiology Unit were studied and the prognosis was evaluated after a follow-up period of 24 +/- 5 months. It was found that the highest proportion of patients were residents of Ankara, followed by the Northern and the Central Anatolian regions, and consanguinity between their parents was more common than the proportion for Turkey as a whole. Most of the patients had dilated cardiomyopathy (78.9%) and the age at which symptoms appeared varied according to the
