Ensuring high-quality patient care: the role of accreditation, licensure, specialty certification and revalidation in medicine.

CONTEXT: The accreditation of medical school programmes and the licensing and revalidation (or recertification) of doctors are thought to be important for ensuring the quality of health care. Whereas regulation of the medical profession is mandated in most jurisdictions around the world, the processes by which doctors become licensed, and maintain their licences, are quite varied. With respect to educational programmes, there has been a recent push to expand accreditation activities. Here too, the quality standards on which medical schools are judged can vary from one region to another. OBJEC

Arnold Max Meirowsky: champion of the American mobile neurosurgical unit during the Korean War.

Dr. Arnold Max Meirowsky (1910-1984) was enormously influential to military neurosurgery during the Korean War, introducing to the American military the concept of the mobile neurosurgical unit. After implementation of the neurosurgical detachment, meningocerebral infections saw a decrease from 41% to less than 1%, with similar improvements in mortality and complication rates. Additionally, Meirowsky developed many techniques and improvements in neurosurgery, specifically in the field of neurosurgical trauma, which he dedicated himself to even after reentering civilian practice. Furthermore, h

BACKGROUND: This study examined associations between anxiety and work-related outcomes in an anxiety disorders clinic population, examining both pretreatment links and the impact of anxiety change over 12 weeks of treatment on work outcomes. Four validated instruments were used to also allow examination of their psychometric properties, with the goal of improving measurement of work-related quality of life in this population. METHODS: Newly enrolled adult patients seeking treatment in a university-based anxiety clinic were administered four work performance measures: Work Limitations Question

The evolving landscape of doctor and nurse mobility in the OECD since 2000 with a focus on Canada.

BACKGROUND: OECD health systems face rising demand for health workers, increasingly relying on international recruitment of doctors and nurses. Growing dependence raises "brain drain" concerns, particularly when recruits originate from WHO Health Workforce Support and Safeguards List countries. OBJECTIVE: This study examines how mobility to OECD countries has evolved over two decades, focusing on Canada, and considers implications for destination and origin countries. METHODS: Using harmonised data on foreign-born and foreign-trained doctors and nurses, we analyse stocks and annual inflows a

Measuring Carer Outcomes in an Economic Evaluation: A Content Comparison of the Adult Social Care Outcomes Toolkit for Carers, Carer Experience Scale, and Care-Related Quality of Life Using Exploratory Factor Analysis.

Background. To incorporate the spillover effects experienced by carers providing informal care in health policy decisions, new carer-related preference-based measures have been developed for use in economic evaluation, which include the Adult Social Care Outcomes Toolkit for Carers (ASCOT-Carer), Carer Experience Scale (CES), and Care-Related Quality of Life (CarerQoL). The aim of this study was to investigate the extent to which these 3 instruments measure complementary or overlapping constructs. Methods. Data were derived from an online survey undertaken with carers residing in Australia. An

The Impact of Biases for Early-Career Women in Medicine: "It's the Little Day-to-Day Microaggressions".

BACKGROUND: Women physicians face multiple systemic and social biases in the clinical setting that erode mental and physical well-being. Gender-based discrimination was reported by 76% of early-career women physicians, decreasing to 57% for mid-career, and 36% for late-career. While these trends suggest that these biases may diminish with career progression, they remain a persistent burden of women physician's careers. This article offers a unique contribution by centering early-career women family physicians, a group underrepresented in qualitative research on workplace bias. This article foc

OBJECTIVE: Based on self-regulation theory, this study aimed to examine the association between future career clarity and bedtime procrastina

The subtle suspension of backlash: A meta-analysis of penalties for women's implicit and explicit dominance behavior.

Previous research suggests that women, more than men, experience negative outcomes when they display dominance. A closer look, however, reveals ambiguity about the specific forms of dominance proscribed for women. Here, we suggest that negative reactions to women's dominance, a counter-stereotypical behavior, may require that the behavior be clearly encoded as counter-stereotypical-which is less likely when the behavior is expressed implicitly. This hypothesis was tested with a meta-analysis of studies on the evaluation of individuals behaving dominantly, including articles not directly invest

The career success of an adult with a learning disability: a psychosocial study of amnesic-semantic aphasia.

B.I. is a 39-year-old, intellectually gifted (IQ = 130) man with learning disabilities who, without known cause, demonstrated symptoms of amnesic-semantic aphasia at age 13. This led to placement in a public school class for students with mild mental retardation and to his dropping out of school after repeating Grade 9. His aphasia is associated with a severe deficit in speech comprehension, poor reading and writing, spatial confusion, and episodic memory loss. We studied the remarkable behavioral and cognitive adjustments that have enabled him to lead a fulfilling life and become a highly suc

Determinants of women's employee engagement in the Bangladeshi workforce.

This study examines the determinants of women's employee engagement in Bangladesh, focusing on socio-cultural influences, organizational culture, leadership support, work-life integration, and career development opportunities. Drawing on a cross-sectional survey of 242 women employees across four industries, the study applies partial least squares structural equation modeling (PLS-SEM) to estimate the associations between these five determinants and women's employee engagement. Organizational culture, leadership support, and work-life integration are each positively associated with women's emp

Practical Considerations for the Academic Physician Moving to a New State.

Moving from one state to another for a new position or opportunity is a common event for academic physicians. Although moving can be personally and professionally disruptive for everyone, it can be particularly challenging for academic physicians. From practical considerations such as applying for a new state medical license to professional challenges such as minimizing disruption to educational or research projects, there are numerous challenges associated with relocating to an academic institution in a new state. Despite the frequency with which academic physicians move between institutions

Publication pressure and burn out among Dutch medical professors: a nationwide survey.

BACKGROUND: Publication of scientific research papers is important for professionals working in academic medical centres. Quantitative measures of scientific output determine status and prestige, and serve to rank universities as well as individuals. The pressure to generate maximum scientific output is high, and quantitative aspects may tend to dominate over qualitative ones. How this pressure influences professionals' perception of science and their personal well-being is unknown. METHODS AND FINDINGS: We performed an online survey inviting all medical professors (n = 1206) of the 8 academi
