Intersecting gender, ethnicity, and sexual orientation identities and HIV stigma: results from the People Living with HIV Stigma Index study in three provinces in Canada.

Stigma remains a significant burden for people living with HIV and while studies have examined the impacts of gender, ethnicity, and sexual orientation on stigma separately, little is known about how these factors may intersect and potentially exacerbate levels of stigma. This study examines how these intersecting social positions may relate to levels of internalised, enacted and anticipated HIV stigma. Participants were recruited in Ontario, Alberta, and Québec (n = 1040) as part of the People Living with HIV Stigma Index study in Canada. Three-way interaction models were constructed by creat

Clinical, Phenotypic and Genotypic Characteristics of Von Willebrand Disease in Afro-Caribbeans: Results From a Study in Martinique Island, French West Indies.

BACKGROUND: Several cohort studies have investigated the molecular basis of von Willebrand disease (VWD); very few have focused on the Afro-Caribbean population. OBJECTIVES: To determine the genotypic and phenotypic characterization of VWD in a large cohort of Afro-Caribbean patients living in Martinique. MATERIALS AND METHODS: A total of 31 families comprising 63 Afro-Caribbean patients with VWD were enrolled. A standardized questionnaire and blood samples were collected for biological and molecular genetic analyses of von Willebrand factor (VWF). The impact of new missense variants has bee

Incidence and mortality of primary liver cancer in England and Wales: changing patterns and ethnic variations.

AIM: To explore recent trends, modes of diagnosis, ethnic distribution and the mortality to incidence ratio of primary liver cancer by subtypes in England and Wales. METHODS: We obtained incidence (1979-2008) and mortality (1968-2008) data for primary liver cancer for England and Wales and calculated age-standardised incidence and mortality rates. Trends in age-standardised mortality (ASMR) and incidence (ASIR) rates and basis of diagnosis of primary liver cancer and subcategories: hepatocellular carcinoma, intrahepatic bile duct and unspecified liver tumours, were analysed over the study per

Positioning palliative oncology in health care in Caribbean and Sub-Saharan African countries: Leveraging social and economic opportunities.

Each year, millions of people around the world experience serious health-related suffering from life-threatening and life-limiting illnesses and require palliative care. Palliative care is a human right to adequate and timely interventions for the well-being of patients with serious illnesses and their families. The World Health Assembly incorporated palliative care as a component of universal health care, reinforcing the World Health Organization's mandate to support governments in integrating the service into primary health care. Yet, fewer than half of the countries across the globe have re

Behaviorally informed digital campaigns and their association with social media engagement and COVID-19 vaccine uptake in Belize.

BACKGROUND: Increasing vaccination coverage was key to curbing the COVID-19 pandemic globally. However, lack of trust in the vaccine and fear of side effects in regions like the Caribbean resulted in a low uptake despite enough vaccine supply. METHODS: We conducted two correlational analyses and one experiment between five sequential behaviorally informed Facebook campaigns, social media performance outcomes, and district-level vaccination data. First, we ran multivariate linear regression models to estimate the mean differences between the campaigns in (i) social media performance ("Clicks"

SARS-CoV-2 in a tropical area of Colombia, a remarkable conversion of presymptomatic to symptomatic people impacts public health.

BACKGROUND: The ability of SARS-CoV-2 to remain in asymptomatic individuals facilitates its dissemination and makes its control difficult. OBJECTIVE: To establish a cohort of asymptomatic individuals, change to the symptomatic status, and determine the most frequent clinical manifestations.  METHODS: Between April 9 and August 9, 2020, molecular diagnosis of SARS-CoV-2 infection was confirmed in 154 asymptomatic people in contact with subjects diagnosed with COVID-19. Nasopharyngeal swabs were performed on these people in different hospitals in Córdoba, the Caribbean area of Colombia. The gen

HIV-related stigma in the UK then and now: to what extent are we on track to eliminate stigma? A qualitative investigation.

BACKGROUND: The introduction of effective antiretroviral treatment in the late 1990s led to the perception that HIV was a chronic but manageable condition. Nevertheless, stigma remains one of the major hurdles for people living with HIV (PLWH) to accessing healthcare and biomedical preventions. Thus, Fast Track Cities has set a target of zero HIV discrimination by 2030 as part of its strategy to end HIV transmission. METHODS: Fifty-three participants from the United Kingdom, including PLWH (n = 21, 40%), health and social care workers (n = 24, 45%), and charity workers and activists (n = 13,

Skin of color: biology, structure, function, and implications for dermatologic disease.

People with skin of color constitute a wide range of racial and ethnic groups-including Africans, African Americans, African Caribbeans, Chinese and Japanese, Native American Navajo Indians, and certain groups of fair-skinned persons (eg, Indians, Pakistanis, Arabs), and Hispanics. It has been predicted that people with skin of color will constitute a majority of the United States and international populations in the 21st century. There is not a wealth of data on racial and ethnic differences in skin and hair structure, physiology, and function. What studies do exist involve small patient popu

Endocrinopathies in paediatric-onset neuromyelitis optica spectrum disorder with aquaporin 4 (AQP4) antibody.

The involvement of the diencephalic regions in neuromyelitis optica spectrum disorder (NMOSD) may lead to endocrinopathies. In this study, we identified the following endocrinopathies in 60% (15/25) of young people with paediatric-onset aquaporin 4-Antibody (AQP4-Ab) NMOSD: morbid obesity ( n = 8), hyperinsulinaemia ( n = 5), hyperandrogenism ( n = 5), amenorrhoea ( n = 5), hyponatraemia ( n = 4), short stature ( n = 3) and central hypothyroidism ( n = 2) irrespective of hypothalamic lesions. Morbid obesity was seen in 88% (7/8) of children of Caribbean origin. As endocrinopathies were prevale

Reproductive health history and later life cognition in African Caribbean women.

INTRODUCTION: Dementia prevalence is higher in women, in particular those of African ancestry. Reproductive health may influence dementia risk, yet data from the Caribbean are lacking. We assessed the association of reproductive health with cognition in the Tobago Women's Health Study. METHODS: This population-based study recruited middle to older aged women from the Caribbean Island of Tobago. Cognitive performance was assessed using the Digit Symbol Substitution Test (DSST). Seven indicators of reproductive health were collected via interviews. Multivariable regression models estimated the

Acute lymphoblastic leukemia mortality in Hispanic Americans.

Higher incidence and poorer outcomes of acute lymphoblastic leukemia (ALL) in Hispanic Americans have been attributed to high-risk molecular markers associated with Native American (NA) ancestry. However, the diverse Hispanic populations in the United States differ substantially in ancestry. Continental Hispanics have a high proportion of NA ancestry while Caribbean Hispanics have a lower proportion of NA ancestry. Here, we analyzed mortality data of 2428 children and adults with ALL. Mortality rates were age-adjusted and compared by race and ethnicity using negative binomial regression with p

Long term mortality trends in people with severe mental illnesses and how COVID-19, ethnicity and other chronic mental health comorbidities contributed: a retrospective cohort study.

BACKGROUND: People with schizophrenia-spectrum and bipolar disorders (severe mental illnesses; 'SMI') experience excess mortality. Our aim was to explore longer-term trends in mortality, including the COVID-19 pandemic period, with a focus on additional vulnerabilities (psychiatric comorbidities and race/ ethnicity) in SMI. METHODS: Retrospective cohort study using electronic health records from secondary mental healthcare, covering a UK region of 1.3 million people. Mortality trends spanning fourteen years, including the COVID-19 pandemic, were assessed in adults with clinician-ascribed ICD-

Nephronophthisis due to a novel DCDC2 variant in a patient from African-Caribbean descent: A case report.

Nephronophthisis-19 (NPHP19) due to truncating mutations in the DCDC2 gene has only been described previously in two patients. We describe a new case in a patient from the island country of Saint Vincent and the Grenadines, in the West Indies. This condition is a renal-hepatic ciliopathy with phenotypic characteristics that include hepatosplenomegaly, hepatic fibrosis with bile cholestasis, increased kidney echogenicity, and end-stage renal disease. Here, we report a 13-year-old African-Caribbean female with areas of absence of heterozygosity suggesting parental consanguinity or identity by dec

Screening for obstructive sleep apnea in an Afro-Caribbean Population: Diagnostic performance of common questionnaires compared with respiratory polygraphy - The MASOS Study A prospective, cross-sectional, descriptive, and analytical study.

BACKGROUND: Obstructive sleep apnea hypopnea syndrome (OSAHS) is a major public health issue. Its prevalence is likely underestimated in many parts of the world, and experts project a substantial future increase. In settings with limited access to diagnostic tools, clinical screening questionnaires may improve the efficiency of patient management. This study aimed to evaluate the diagnostic performance of the main OSAHS screening questionnaires in an Afro-Caribbean population. METHODS: We conducted a cross-sectional, descriptive, and analytical study, in a high-risk population of patients refe

Renal dysfunction and diastolic impairment among British ethnic minorities with hypertension: the Ethnic-Echocardiographic Heart of England Screening Study.

Renal dysfunction is frequently associated with left ventricular (LV) hypertrophy and diastolic dysfunction in hypertensive patients. Limited data exist on renal dysfunction and diastolic impairment among British ethnic minorities with hypertension. We studied associations between renal impairment and diastolic dysfunction in hypertensive subjects of African-Caribbean and South Asian origin. Five hundred and ten hypertensive subjects with ejection fraction ⩾55% and with no history of ischaemic heart disease/valve pathology were included from the original population of the Ethnic-Echocardiograp

A Scoping Review of Footwear Worn by People With Diabetes in Low- and Middle-Income Countries: Implications for Ulcer Prevention Programs.

International guidelines advise people with diabetes to wear close-toed, thick-soled footwear to protect against foot ulceration. However, this type of footwear is incompatible with some of the cultures, climates, and socioeconomic conditions in many low- and middle-income countries (LMICs). This scoping review aims to summarize what is known about footwear used by people with diabetes in LMICs and consider whether international diabetic foot guidelines are practical and actionable in these contexts, given current practices. General and abdominal adiposity and hypertension in eight world regions: a pooled analysis of 837 population-based studies with 7·5 million participants.

BACKGROUND: Adiposity can be measured using BMI (which is based on weight and height) as well as indices of abdominal adiposity. We examined the association between BMI and waist-to-height ratio (WHtR) within and across populations of different world regions and quantified how well these two metrics discriminate between people with and without hypertension. METHODS: We used data from studies carried out from 1990 to 2023 on BMI, WHtR and hypertension in people aged 20-64 years in representative samples of the general population in eight world regions. We graphically compared the regional dist

Racial variation in the number of spontaneous abortions before a first successful pregnancy, and effects on subsequent pregnancies.

OBJECTIVE: To assess the relationship between race and spontaneous abortion, whether the relationship varies by risk factors, and the effect of spontaneous abortions on subsequent pregnancies. METHODS: A retrospective study was undertaken using data collected in London, UK, between 1988 and 2000. Logistic regression evaluated histories of spontaneous abortion and associations with small-for-gestational-age and preterm births in black African, black Caribbean, and South Asian women relative to white European women. Interactions with risk factors were assessed. RESULTS: Overall, 196 040 women
