Impact of Obstructive Sleep Apnea on Arrhythmia and Quality-of-Life Outcomes After Catheter Ablation of Atrial Fibrillation.

BACKGROUND: Catheter ablation is a first-line therapy for atrial fibrillation (AF). We evaluated the impact of obstructive sleep apnea (OSA) on arrhythmia and postablation quality-of-life outcomes among AF subtypes. METHODS: Consecutive patients undergoing AF ablation (2013-2022) at a tertiary center were enrolled and assessed for arrhythmia recurrence over 3-year follow-up. A subset of patients enrolled in an automated patient-reported outcomes platform were assessed for quality of life, including Atrial Fibrillation Severity Score and AF burden (AF duration and frequency scores). RESULTS:

Chronic obstructive pulmonary disease is associated with higher recurrence rates of atrial fibrillation following catheter ablation.

BACKGROUND: Patients with atrial fibrillation (AF) and chronic obstructive pulmonary disease (COPD) are at increased risk of cardiovascular mortality compared to patients with AF alone. Consequently, employing rhythm control strategies such as AF catheter ablation could offer substantial benefits to patients with COPD. However, the impact of COPD on AF ablation outcomes is not well established. METHODS: In this single-center case control study, we retrospectively analyzed 200 patients with AF and COPD, 52 of whom underwent AF catheter ablation. Those who underwent ablation were matched with a

The Value of Urethral Pressure Profile Using Air-Charged Catheters in the Diagnosis of Female Bladder Neck Obstruction.

OBJECTIVES: Rarely have studies focused on the utilization of urethral pressure profile (UPP) in bladder neck obstruction (BNO). This study aimed to evaluate the parameters of UPP in urodynamic testing (UDS) using air-charged catheters (ACC) and to investigate the value of UPP as a diagnostic tool for female BNO. PATIENTS AND METHODS: We retrospectively collected records of 165 female patients with lower urinary tract symptoms (LUTS) who received UDS at our center from September 2020 to September 2022. The whole patient group was divided into BNO group (n = 92) and non-BNO group (n = 73). The

Septal Ablation with Radiofrequency Catheters Guided by Echocardiography for Treatment of Patients with Obstructive Hypertrophic Cardiomyopathy: Initial Experience.

BACKGROUND: Hypertrophic cardiomyopathy (HCM) can cause obstruction in the left ventricular outflow tract (LVOT), and be responsible for the onset of limiting symptoms, such as tiredness. When such symptoms are refractory to pharmacological treatment, interventionist alternative therapies can be useful, such as septal ablation through the infusion of alcohol in the coronary artery or through myectomy. Recently, the use of a radiofrequency (RF) catheter for endocardial septal ablation guided by electroanatomic mapping has proven to be efficient, despite the high incidence of complete atrioventr

A multicenter retrospective study of heterogeneous tissue aggregates obstructing ventricular catheters explanted from patients with hydrocephalus.

BACKGROUND: Implantation of ventricular catheters (VCs) to drain cerebrospinal fluid (CSF) is a standard approach to treat hydrocephalus. VCs fail frequently due to tissue obstructing the lumen via the drainage holes. Mechanisms driving obstruction are poorly understood. This study aimed to characterize the histological features of VC obstructions and identify links to clinical factors. METHODS: 343 VCs with relevant clinical data were collected from five centers. Each hole on the VCs was classified by degree of tissue obstruction after macroscopic analysis. A subgroup of 54 samples was analy

Overnight catheter drainage in children with poorly compliant bladders improves post-obstructive diuresis and urinary incontinence.

PURPOSE: Overnight catheter drainage (OCD) has been suggested as a treatment for boys with valve bladder syndrome as well as those with polyuric renal failure. We report our experience using overnight catheter drainage in children with poorly compliant bladders. MATERIALS AND METHODS: Between 1999 and 2004 OCD was initiated in 6 boys and 5 girls (median age 7.5 years) with poorly compliant bladders in whom aggressive daytime intermittent catheterization and anticholinergic use had failed. Etiologies of bladder dysfunction included posterior urethral valves in 1 case, The Hinman-Allen syndrome

Knotted bowel: small-bowel obstruction from coiled peritoneal shunt catheters. Report of two cases.

Knotting of a peritoneal catheter around a loop of bowel is a rare occurrence, which may lead to bowel obstruction. The incomplete removal of two ventriculoperitoneal shunts resulted in two cases of iatrogenically knotted peritoneal catheters. One patient underwent a laparotomy for relief of obstruction and the other was successfully treated by uncoiling the catheter by means of a wire passed into its lumen. A plan for management of a knotted peritoneal catheter is outlined.

Suprapubic catheter placement through ileal mesentery causes mechanical small bowel obstruction six years later in a female patient with turners syndrome.

Percutaneous suprapubic cystostomy (SPC) is a procedure performed to manage urinary retention when urethral catheterization is contraindicated or to improve the quality of life in cases such as neurogenic bladder. Although a simple procedure, it is associated with serious complications, increasing the morbidity and mortality. This case study demonstrates a delayed presentation of small bowel obstruction caused by a suprapubic catheter traversing through the ileal mesentery in a patient with no prior bowel surgeries. Few cases report this complication and this is possibly the first case to be r

A new self-retaining intraurethral device. An alternative to an indwelling catheter in patients with urinary retention due to infravesical obstruction.

A new self-retaining intraurethral catheter, designed as an alternative to an indwelling catheter, has been used in 14 patients. It was left in place for 2 to 18 weeks; 11 of the 14 patients were able to void freely, were continent and had neither residual urine nor urinary tract infection. This device may be used instead of an indwelling Foley catheter in patients with retention due to infravesical obstruction and may be an alternative to prostatectomy in patients who are at high risk for surgery.

Acute and late obstruction of a modified Blalock-Taussig shunt: a two-center experience in different catheter-based methods of treatment.

Modified Blalock-Taussig (B-T) shunt occlusion results in a sudden reduction of pulmonary vascular perfusion, causing dramatic saturation drop and cyanosis which pose a direct hazard to a child's life. The results of percutaneous local r-tpa infusion, balloon angioplasty and additionally stent implantation in obstructed modified B-T shunts were studied to assess their role as an alternative to a re-do surgery. We outline two pediatric centers' experience (period 2004-2008) regarding the effectiveness of various emergency treatment methods for occlusion or critical stenosis of systemic-to-pulmo

Feasibility and clinical utility of stone extraction balloon catheter-assisted endoscopic stenting for malignant distal duodenal and proximal jejunal obstruction (with video).

BACKGROUND: Endoscopic stenting for malignant distal duodenal and jejunal obstructions is challenging because the length of conventional gastroscopes is insufficient to reach the obstruction point. A fully inflated balloon catheter freely navigates the duodenum and jejunum without causing bowel injury by burying the catheter tip. It easily changes its direction for guidewire relocation, conforming to the angulation of the small bowel flexures. This study evaluated the feasibility and efficacy of stone extraction balloon catheter-assisted endoscopic stenting for malignant distal duodenal and pr

Fluorescent Ureteral Catheter-Assisted Laparoscopic Nephrectomy for a Pediatric Patient With Obstructed Hemivagina and Ipsilateral Renal Anomaly Syndrome.

Obstructed hemivagina and ipsilateral renal anomaly (OHVIRA) syndrome is a rare congenital disorder characterized by Müllerian duct malformation and ipsilateral renal abnormalities. Variant forms associated with hypoplastic kidneys and ectopic ureteral insertion in the vagina may require nephrectomy because of persistent urinary incontinence. In such cases, urinary drainage through a vaginal opening permits direct ureteral access via vaginoscopy without the size limitations associated with transurethral cystoscopy. Therefore, retrograde catheter placement is technically feasible in patients wi

Obstructive Uropathy Secondary to Accidental Ureteric Placement of an Indwelling Urinary Catheter at the Time of Caesarean Section: A Case Report.

This case describes a rare complication of Foley catheter insertion where accidental instrumentation of a ureter occurred at the time of caesarean section with the subsequent development of bladder obstruction and acute kidney injury. Rapid identification and management can minimise the associated long-term implications; this therefore necessitates greater awareness of this possible complication. The current understanding of this condition recognises neurogenic bladder and female sex as risk factors; however, physiological changes in pregnancy may also act as a significant risk factor for this

[A Case of Sheath-Like Obstruction of the Entire Catheter in a Short Period of Time after Placement of an Intraperitoneal Access Port].

Intraperitoneal chemotherapy, in which an anticancer drug is administered directly into the abdominal cavity through an intraperitoneal access port(IP port), is one of the treatment options for advanced gastric cancer with peritoneal metastasis. Herein, we report a case of sheath-like obstruction of the entire catheter of the IP port due to tissue reaction within a short period of time after IP port implantation. The case was a 35-year-old woman with advanced type 4 gastric cancer with peritoneal dissemination. The IP port was placed and intravenous and intraperitoneal chemotherapy using S-1 p

Predictive value of obstructive sleep apnoea assessed by the Berlin Questionnaire for outcomes after the catheter ablation of atrial fibrillation.

AIMS: Obstructive sleep apnoea (OSA) is a risk factor for atrial fibrillation (AF) recurrence after catheter ablation. The purpose of this study is to determine whether OSA assessed by the simple tool, the Berlin Questionnaire (BQ), is useful in predicting the efficacy of catheter ablation of AF. METHODS AND RESULTS: The patient population consisted of 210 consecutive patients [mean age 58 +/- 10 years, 167 (80%) male, 119 (57%) paroxysmal] who underwent catheter ablation of AF and completed the BQ. Clinical success was defined as at least 90% reduction in AF burden after 3-month blanking per

Obstructive effect of a closed 12F urethral catheter during the emptying phase of cystometry in patients with a spinal reflex bladder.

In spinal cord injured patients with a reflex urinary bladder urodynamic evaluation of the detrusor pressure during the emptying phase is important, but the methods are not yet standardised. The aim of this study was to examine whether the detrusor pressure is significantly affected by the presence of a catheter in the urethra. In seven patients with a spinal reflex bladder, the maximum detrusor pressure and the duration of detrusor contractions in cystometry with a suprapubic technique were compared with the corresponding measurements when a closed 12F catheter was added to mimic a transureth
