BACKGROUND: Cervical insufficiency (CI) is thought to be responsible for 8% of miscarriages and preterm births. Cervical cerclage is the main treatment for CI. There are different approaches to cervical cerclage, and it is particularly important to find a method of cervical cerclage that is simple to perform and results in better pregnancy outcomes.
OBJECTIVE: This study investigates the clinical efficacy of transvaginal hemiperitoneal cervical cerclage in the treatment of CI and its impact on maternal and fetal outcomes.
METHODS: We performed a descriptive retrospective single-center study,

OBJECTIVES: To evaluate whether cervical funneling influences pregnancy outcome in women with short cervical length (CL) after cerclage, and to identify funneling parameters associated with pregnancy outcome.
METHODS: Medical records of women identified to have a short CL with or without funneling after cerclage were reviewed. Women with short CL as well as funneling were defined as cases (Funneling group), and those with short CL but no funneling were the controls (No Funneling group). We compared perinatal outcome between the two groups and analyzed the relationships between funneling param

Forty-seven antenatal patients with clinical diagnosis of cervical incompetence were subjected to a structured scoring system before insertion of McDonald Stitch. All other possible causes of midtrimester abortion were ruled out and the patients followed up to the end of their pregnancies. Significantly there were fewer preterm deliveries in the high score group (P less than 0.01) and no abortions. The mean cerclage-delivery interval was shown to rise with increasing scores. Significantly better fetal salvage ratio was also seen in the high score group (P less than 0.05). The scoring system sh

INTRODUCTION: In terms of prophylactic cerclage, the simplest and most commonly used technique is the McDonald cervical cerclage. Cervico-isthmic cerclage techniques may have better results, but are more difficult to achieve. A simplified Shirodkar technique would have the advantage of being "cervical high" while remaining easy to achieve.
OBJECTIVE: To compare the results of high cervical cerclage, according to a simplified Shirodkar technique, with those of a classic McDonald cerclage, in the case of women at high risk of prematurity in an exploratory study.
METHODS: A comparative, retrosp

BACKGROUND: Although there have been significant improvements in prenatal and neonatal care in recent years, twin pregnancies account for 17-20% of all preterm births and are associated with an increased risk of perinatal mortality and morbidity.
AIMS: To evaluate the contribution of cerclage to the continuation of pregnancy in twin pregnancy cases required cervical cerclage.
METHODS: Cases of twin pregnancies who performed cervical cerclage with follow-up and deliveries between January 2010 and January 2022 in the Department of Obstetrics and Gynecology, Ege University Faculty of Medicine H

Transabdominal cerclage (TAC) is a recognised treatment for recurrent spontaneous late miscarriage or preterm birth due to cervical weakness. This can be performed via an open procedure before and during pregnancy, or a laparoscopic technique preconception. Complications include cerclage failure and suture migration. We present a case highlighting these complications where laparoscopic removal of an open TAC and replacement led to two successful term deliveries. A woman in her thirties with a fibroid uterus, adenomyosis and a history of three spontaneous mid-trimester losses, had an open TAC a

OBJECTIVE: Cervical cerclage, when performed in twin gestation, has been reported to be associated with poor outcome. However, the role of first-trimester history-indicated cerclage among women with a twin pregnancy and a history of preterm birth has not been evaluated. The aim of this study was to assess pregnancy outcomes among women with a twin pregnancy who underwent first-trimester history-indicated cervical cerclage compared with outcomes in those managed expectantly.
METHODS: This was a retrospective matched case-control study. The study group comprised all women with a twin pregnancy

BACKGROUND: A cervical stitch has been used to prevent preterm deliveries in women with previous second trimester pregnancy losses, or other risk factors such as short cervix on digital or ultrasound examination.
OBJECTIVES: To assess effectiveness and safety of prophylactic cerclage (before the cervix has dilated), emergency cerclage (where cervices have started to shorten and dilate) and then labour halted, and to determine whether a particular technique of stitch insertion is better than others.
SEARCH STRATEGY: We searched the Cochrane Pregnancy and Childbirth Group specialised register

Fifty-one patients underwent emergency cervical cerclage using Shirodkar or McDonald suture for incompetent cervix diagnosed during pregnancy. Fifteen patients had only sonographic evidence of cervical incompetence (group I). Eighteen patients had cervical dilation of less than 3 cm (group II), and 18 patients had cervical dilation of 3 cm or more (group III). Perinatal mortality was 0%, 50%, and 55.6%, respectively. The median length of time gained with the cerclage was 16 weeks in group I, 1.8 weeks in group II, and 1 week in group III. Twelve infants required prolonged stay in the neonatal

Twenty-nine cases of cervical incompetence were found among 98 women diagnosed as having a congenital uterine anomaly on hysterosalpingography, a high incidence of 30%. The highest incidence was found in the bicornuate uterus group--38%. The incidence of premature delivery and late abortion was higher in this group than in the rest of the patients with uterine anomalies (55% and 68%, vs. 45% and 32%). An obvious improvement in obstetrical performance was noted after cerclage. In the cervical incompetence group, term deliveries increased from 26% to 63%. Premature deliveries and late abortions

The incidence of cervical incompetence is 0.1-1%. A retrospective study of a six year period in the University Hospital of Copenhagen, Hvidovre is presented. This includes 37 women, all of whom were treated surgically on 44 occasions with cerclage on account of incompetence of the cervix. The duration of the pregnancy after operation was longer in women operated in a quiet phase, without cervical effacement and with an internal os smaller than 3 cm. Women with an internal os dilated to more than 3 cm at the time of operation all terminated their pregnancies within three days. This study showed
