Clinico-cytopathological subcategorization in thyroid nodules of atypia of undetermined significance/follicular lesion of undetermined significance using the TIRADS and Bethesda classifications.

INTRODUCTION: Bethesda category III - atypia of undetermined significance/follicular lesion of undetermined significance (AUS/FLUS) is a heterogeneous class of the Bethesda system for thyroid nodules. In order to clarify the therapeutic road for clinicians, this category was subclassified based on the cytopathological features. In this study, we evaluated the risk of malignancy, surgical outcome, demographic characteristics, and correlation of ultrasound features with the final outcome in patients with thyroid nodules based on AUS/FLUS subclassification. METHOD: After evaluating 867 thyroid n

[Cerebellar degeneration in chronic alcoholism: with special reference to an autopsied case showing the restricted form of cerebellar cortical degeneration (Victor)].

The brains of 8 patients with chronic alcoholism were neuropathologically examined. The findings obtained in the cerebellum of case 1 (male, aged 52) were in agreement with the restricted form of cerebellar cortical degeneration which was described by Victor et al (1958). The clinicopathological report on the cerebellar degeneration of this type was very rare in Japan. The cerebellar changes were more pronounced in the anterior vermis than in the posterior vermis and hemisphere. Furthermore, some old lesions of Wernicke's encephalopathy were found in this brain. 7 other cases showed very mild

From back to front: A functional model for the cerebellar modulation in the establishment of conditioned preferences for cocaine-related cues.

It is now increasingly clear that the cerebellum may modulate brain functions altered in drug addiction. We previously demonstrated that cocaine-induced conditioned preference increased activity at the dorsal posterior cerebellar vermis. Unexpectedly, a neurotoxic lesion at this region increased the probability of cocaine-induced conditioned preference acquisition. The present research aimed at providing an explanatory model for such as facilitative effect of the cerebellar lesion. First, we addressed a tracing study in which we found a direct projection from the lateral (dentate) nucleus to t…

Glucose metabolism in the human cerebellum: anatomical-functional correlations.

Functional maturation of human cerebellar structures was previously difficult to evaluate with positron emission tomography (PET) because of scanner resolution. Only vermis and cerebellar hemispheres could be reliably identified with 2-deoxy-2[18F]fluoro-D-glucose (FDG) PET. In the present study, we have used a new high resolution PET system, the Siemens EXACT-HR, to study, in depth, the patterns of glucose metabolism in various cerebellar structures. Due to the orientation of the cerebellar fissures which are important landmarks, the largest number of regions could be identified when the cere

Clinical study of medial area infarction in the region of posterior inferior cerebellar artery.

Our objective is to study the neurological characteristics of medial area infarction in the caudal cerebellum. Medial area of the caudal cerebellum is supplied with 2 branches of the posterior inferior cerebellar artery (PICA). The medial hemispheric branch of the PICA distributes to the medial area of the caudal cerebellar hemisphere. The medial branch of the PICA (mPICA) distributes to the inferior vermis. We studied the neurological characteristics of 18 patients with medial area infarction of the caudal cerebellum. The infarction was located in the medial area of the cerebellar hemisphere

Posterior fossa abnormalities in major depression: a controlled magnetic resonance imaging study.

High-field magnetic resonance (MR) images were used to study posterior fossa morphology in 27 patients with major depression and 36 normal control subjects. Depressed patients demonstrated smaller brain stem and cerebellar vermis than controls. These differences were highly significant for the anterior cerebellar vermis and medulla. There was also a striking age-related decline in midbrain size in depressed patients as well as in controls. Our results are consistent with several lines of evidence implicating a role for the cerebellar vermis in affective disorders and, in addition, provide the …

Cerebellum Involvement in Visuo-vestibular Interaction for the Perception of Gravitational Direction: A Repetitive Transcranial Magnetic Stimulation Study.

Accurate perception of the direction of gravity relies on the integration of multisensory information, particularly from the visual and vestibular systems, within the brain. Although a recent study of patients with cerebellar degeneration suggested a cerebellar role in visuo-vestibular interaction in the perception of gravitational direction, direct evidence remains limited. To address this gap, we conducted two experiments with 42 healthy participants to evaluate the impact of repetitive 1 Hz transcranial magnetic stimulation (rTMS) over the posterior cerebellar vermis on visual dependency, q…

Evidence for a link among cognition, language and emotion in cerebellar malformations.

We compared the neurobehavioral profiles of children with Joubert syndrome (JS participants), a rare autosomal recessive condition characterized on magnetic resonance imaging (MRI) by hypoplasia of the cerebellar vermis and midbrain-hindbrain malformations, and children with malformations confined to the cerebellar vermis and one or both hemispheres (Cerebellar malformations--CM participants). We aimed at investigating the influence of anatomo-clinical similarities (vermian malformation) and differences (intact cerebellar hemispheres vs sparing of the pons, respectively) with respect to cognit…

Dandy-Walker syndrome successfully treated with cystoperitoneal shunting--case report.

A neonate presented with Dandy-Walker syndrome manifesting as a large posterior cranial fossa cyst, aplasia of the lower cerebellar vermis, and elevation of the confluence of the sinuses but without hydrocephalus. A cystoperitoneal shunt was placed at one month after birth. The cyst diminished in size, and marked development of the cerebellar hemispheres and descent of the confluence of sinuses were observed, but not vermis development. The primary pathology of Dandy-Walker syndrome is posterior cranial fossa cyst formation due to passage obstruction in the fourth ventricle exit area and aplas…

The prevalence of alcoholic cerebellar atrophy. A morphometric and histological study of an autopsy material.

The Purkinje cell densities in the cerebellar vermis were determined in 31 male alcoholics and 34 non-alcoholic controls under 70 years of age. In addition, all cases were examined histologically for atrophy of the superior vermis. All analyses were performed independently of each other and on randomized slides. The alcoholics had significantly lower Purkinje cell densities than the controls in the superior and middle segments of the vermis. Histologically verified atrophy of the superior vermis was found in 13 of the alcoholics (42%) and in 3 of the controls (9%). There was a good accordance …

Resection of fourth ventricle tumors without splitting the vermis: the cerebellomedullary fissure approach.

OBJECTIVE: Standard surgical practice for excision of fourth ventricle tumors entails splitting the inferior vermis, but incision of the vermis and lateral retraction on the dentate nuclei and their outflow tracts have been implicated in the development of the so-called 'cerebellar mutism syndrome'. We describe a surgical approach in which the cerebellar vermis is preserved. METHODS: Clinical experiences with 11 patients harboring fourth ventricle tumors were supplemented by fixed and fresh cadaver dissections. Anatomic illustrations, prosections and intraoperative photographs are presented. …

[Prenatal diagnosis of Joubert syndrome:one case report and literature review].

A 25-year-old nulliparity underwent prenatal ultrasonography, and the fetal cerebellar abnormality was suspected. The fetal MRI showed 'molar tooth sign' in midbrain and cerebellar vermis hypoplasia. The fetal cerebellar vermis hypoplasia was confirmed by MRI imaging and autopsy after induced abortion. The next-generation sequencing showed that the fetus had a heterozygous mutation of CC2D2A gene (c.2728C > T and c.4598T > C), which might be the cause of the disease.

Neuroarchitecture of the central complex in the Madeira cockroach Rhyparobia maderae: Pontine and columnar neuronal cell types.

Insects have evolved remarkable abilities to navigate over short distances and during long-range seasonal migrations. The central complex (CX) is a navigation center in the insect brain that controls spatial orientation and directed locomotion. It is composed of the protocerebral bridge (PB), the upper (CBU) and lower (CBL) division of the central body, and a pair of noduli. While most of its functional organization and involvement in head-direction coding has been obtained from work on flies, bees, and locusts that largely rely on vision for navigation, little contribution has been provided b

SIFT-GVF-based lung edge correction method for correcting the lung region in CT images.

Juxtapleural nodules were excluded from the segmented lung region in the Hounsfield unit threshold-based segmentation method. To re-include those regions in the lung region, a new approach was presented using scale-invariant feature transform and gradient vector flow models in this study. First, the scale-invariant feature transform method was utilized to detect all scale-invariant points in the binary lung region. The boundary points in the neighborhood of a scale-invariant point were collected to form the supportive boundary lines. Then, we utilized a Fourier descriptor to obtain a character

Ecchordosis Physaliphora: Does It Even Exist?

The term ecchordosis physaliphora (EP) has been used historically to describe a benign notochordal remnant with no growth potential, most commonly occuring in the central clivus. Unfortunately, the radiologic appearance of EP overlaps considerably with the appearance of low-grade chordomas, which do have the potential for growth. In this article, we review new pathologic terminology that better describes this family of diseases, and we propose new radiologic terms that better address the uncertainty of the radiologic diagnosis. The surgical importance of accurate terminology and the implicatio

[Paroxysmal cerebellar ataxia--an evaluation using the findings from magnetic resonance imaging, positron emission tomography, and acetazolamide treatment].

Paroxysmal cerebellar ataxia (PCA) is a specific disease which exhibits spasmodic cerebellar ataxia but rarely shows abnormal neurological findings in the intermission. Verger first described an isolated case. Subsequent reports of the disease included mostly cases with autosomal dominant inheritance, but the reports have been limited to about 20 families. Although both the lesion and the cause have not been clearly identified, since Vighetto et al. demonstrated the atrophy of the anterosuperior region of the cerebellar vermis using magnetic resonance imaging (MRI), the lesion of PCA captured …
