Disrupted white matter in language and motor tracts in developmental stuttering.

White matter tracts connecting areas involved in speech and motor control were examined using diffusion-tensor imaging in a sample of people who stutter (n=29) who were heterogeneous with respect to age, sex, handedness and stuttering severity. The goals were to replicate previous findings in developmental stuttering and to extend our knowledge by evaluating the relationship between white matter differences in people who stutter and factors such as age, sex, handedness and stuttering severity. We replicated previous findings that showed reduced integrity in white matter underlying ventral prem

Crossed cerebellar atrophy in patients with precocious destructive brain insults.

OBJECTIVE: To analyze the frequency and pathogenetic factors of crossed cerebellar atrophy (CCA) in adult patients with epilepsy secondary to destructive brain insults of early development. METHODS: We studied 51 adult patients with epilepsy and precocious destructive lesions. Patients were divided into 3 groups according to the topographic distribution of their lesions on magnetic resonance imaging: group A, hemispheric (n = 9); group B, main arterial territory (n = 25); and group C, arterial border zone (n = 17). We evaluated the presence of CCA visually and with cerebellar volumetric measu

Cerebral haemorrhage in Moyamoya disease at autopsy.

Nineteen fatal cases of occlusion of the circle of Willis, so-called Moyamoya disease, were examined clinicopathologically. Fresh and massive cerebral haemorrhage was confirmed in 14 and cerebral infarcts of 4 of 19 patients. Among these 14 patients, massive haemorrhage was found in the basal ganglia, thalamus and hypothalamus of 9, and in the thalamus, cerebral peduncle and midbrain of 5. Pathologically, fibrosing stenoses or occlusions involved the circle of Willis and its major branches in all cases. In 13 of 17 patients numerous collateral channels, muscular in type, paralleled the circle,

Excitatory Repetitive Transcranial Magnetic Stimulation Induces Contralesional Cortico-Cerebellar Pathways After Acute Ischemic Stroke: A Preliminary DTI Study.

Background: Repetitive transcranial magnetic stimulation (rTMS) is proved to be effective in facilitating stroke recovery. However, its therapeutic mechanism remains unclear. The present study aimed to investigate changes in white matter fractional anisotropy (FA) after excitatory rTMS to better understand its role in motor rehabilitation. Materials and Methods: Acute stroke patients with unilateral subcortical infarction in the middle cerebral artery territory were recruited. The patients were randomly divided into an rTMS treatment group and a sham group. The treatment group received a 10-da

Cerebellar White Matter Microstructure Is Associated With Age, Cerebrospinal Fluid Amyloid Beta Levels, and Cognition in Cognitively Unimpaired Older Adults.

Structural changes in the cerebellum contribute to cognitive decline due to aging and Alzheimer's disease (AD). However, it is unclear whether age and AD pathology are associated with structural alterations in the cerebellum among cognitively unimpaired individuals and how these alterations relate to cognition. This study examined the association of age and cerebrospinal fluid (CSF) AD biomarkers (amyloid beta [Aβ42/Aβ40], phosphorylated tau [p-tau181]) with cerebellar gray matter (GM) and white matter (WM) volumes and cerebellar WM microstructure, measured via magnetic resonance imaging (MRI)

An Eyebrow, Supracarotid Triangle Approach for Lesions at the Ventral Thalamopeduncular Junction: A Technical Report.

BACKGROUND: Lesions arising at the ventral thalamopeduncular junction are difficult to resect. In addition to being relatively inaccessible, these lesions are located in one of the most sensitive areas of the brain. A critical question is whether new approaches could be developed to allow surgeons to adequately resect these lesions with reasonable outcomes. In the present report, we describe our approach to resect lesions in this region of the brain using an eyebrow craniotomy approach with a trajectory through the supracarotid triangle. METHODS: Through retrospective data collection, we pres

[Unilateral persistent hyperhidrosis after ischemic stroke].

A 64-year-old right hemiplegic woman, who had been treated for hypertension for 15 years, was admitted to our hospital. Neurologic examination on admission disclosed right hemiplegia and motor aphasia; however, ophthalmoparesis, pupillary abnormality, and blepharoptosis were not evident. Excessive sweating on the right side of the body, which was most marked on the face, was observed. Amount of sweating on the left side of the body was normal. Unilateral hyperhidrosis persisted for more than 2 months. MRI revealed hemorrhagic infarctions in the left basal ganglia, internal capsule, thalamus, h

The segmentation of the posterior cerebral artery: a microsurgical anatomic study.

There are still different descriptions of the segmentation of the posterior cerebral artery, although there is a radiological and anatomical consensus on the segmentation of the anterior and the middle cerebral artery. This study aims to define the most appropriate localization for origin and end points of the segments through reviewing the segmentation of the posterior cerebral artery. The segments and the cortical branches originating from those segments of the 40 posterior cerebral arteries of 20 cadaver brains were examined under operating microscope. In this research, the P1, P2, P3, P4,

AIDS-related primary central nervous system lymphoma: prolonged remission associated with highly active antiretroviral therapy.

A 36-year-old HIV-seropositive man developed progressive confusion and unilateral tremor of the hand. His medical history included cryptococcal meningitis and CMV colitis. CT scan revealed a single hyperdense mass with minimal peripheral enhancement at the region of the cerebral peduncle and pons, causing obstructive hydrocephalus. He was treated with ventriculo-peritoneal shunt and cranial radiotherapy. He also received treatment with highly active antiretroviral therapy (HAART). A CD4+ cell count was increased from 2 to 345 cells/mm3. He returned to normal function for about 32 months after

Two mesencephalic lacunar infarcts presenting as Claude's syndrome and pure motor hemiparesis.

Two exceptional cases of mesencephalic lacunar infarcts located both in the anterior vascular territory are reported. In patient 1, the infarct selectively involved the red nucleus, thus resulting in a Claude's syndrome. In patient 2, the lesion was limited to the external 2/3 of the cerebral peduncle, and was responsible for a pure motor hemiplegia (PMH). CT scan easily demonstrated the lesion in both cases. Claude's syndrome is very unusual, and PMH has only been reported once before in a mesencephalic infarct. The reasons why these lesions are so uncommon are discussed.

An unusual cause of fixed dilated pupils after cardiac surgery.

BACKGROUND: The presence of fixed, dilated pupils after cardiac surgery is an ominous sign, typically indicating severe diffuse hypoxic-ischemic brain injury. Rarely, however, this finding can be seen as a result of focal midbrain ischemia. It is important to differentiate between these syndromes, as the latter might be amenable to acute stroke treatments, and because they affect consciousness very differently. CASE: A 46-year-old man with diffuse atherosclerosis underwent coronary artery bypass grafting and closure of an incidentally discovered patent foramen ovale. He underwent neurologic e

Age-related white matter lesions are associated with reduction of the apparent diffusion coefficient in the cerebellum.

Cerebellar apparent diffusion coefficient (ADC) was found to be increased after acute cerebral hemispheric stroke. There are no data on cerebellar ADC changes in patients with chronic, age-related white matter lesions (ARWML). We aimed to determine longitudinal ADC variations on cerebral hemispheric and cerebellar white matter regions of patients with ARWML in order to study relations between ADC changes in both regions. ADC was measured serially (1-year interval) on lesioned periventricular frontal white matter, frontal and parietoccipital normal appearing white matter and middle cerebellar p

A case of Tourette's syndrome with midbrain involvement.

Several hypotheses have attempted to incriminate a particular anatomical region responsible for the complex symptomatology of Tourette's syndrome (TS). Recently, Devinsky speculated that damage to the peri-aqueductal gray matter (PAG) and midbrain tegmentum may represent the major anatomical site of dysfunction in TS. A patient with TS in whom radiological evidence indicated midbrain involvement may support Devinsky's hypothesis. A 7-year-old boy exhibited motor and vocal tics since the age of four. These were associated with hyperactive behavior, stereotype body movements, abnormal sexual beh

MR of craniocerebral hemiatrophy.

The magnetic resonance (MR) findings of three patients with cerebral hemiatrophy, the so-called Dyke-Davidoff-Masson syndrome, which is characterized by variable degrees of unilateral loss of cerebral volume and compensatory changes of the calvarium are presented. The condition was due to middle cerebral artery stroke in all patients. The pathologic alterations of cerebral tissue and the brainstem were reflected in detail on the MR studies. MR findings in addition to the primary vascular insult included prominence of the cortical sulci and perimesencephalic cistern in one subject with acquired

Brain structural damage in spinocerebellar ataxia type 2. A voxel-based morphometry study.

Voxel-based morphometry (VBM) enables an unbiased in-vivo whole-brain quantitative analysis of differences in gray matter (GM), white matter (WM) and cerebro-spinal fluid (CSF) volumes. We assessed with VBM 20 spinocerebellar ataxia Type 2 (SCA2) patients with mild or moderate cerebellar deficit and 20 age and sex-matched healthy controls. SCA2 patients showed a significant (P < 0.05 corrected for multiple comparison) symmetric loss of GM in the cerebellar vermis and hemispheres sparing lobules I,II, Crus II,VII, and X, and of the WM in the peridentate region, middle cerebellar peduncles, dors

Xylometazoline abuse induced ischemic stroke in a young adult.

INTRODUCTION: substance abuse is an important cause of ischemic stroke in the young. This includes over-the-counter dietary supplements and cough and cold remedies, which were reported to be an independent risk factor for hemorrhagic stroke. CASE REPORT: this article describes a young male patient with acute ischemic infarctions in the posterior inferior cerebellar and posterior cerebral artery territories bilaterally, the right cerebral peduncle, the left pontine tegmentum, and lateral pons following abuse of xylometazoline-containing nasal decongestant for 10 years. CONCLUSION: this is the

Involvement of the dentate nucleus in the pathophysiology of amyotrophic lateral sclerosis: A multi-center and multi-modal neuroimaging study.

Amyotrophic lateral sclerosis (ALS) is characterized primarily by motor neuron but also frontotemporal lobar degeneration. Although the cerebellum is involved in both motor and cognitive functions, little is known of its role in ALS. We targeted the dentate nucleus (DN) in the cerebellum and the associated white matter fibers tracts connecting the DN to the rest of the brain using multimodal imaging techniques to examine the cerebellar structural and functional connectivity patterns in ALS patients and hypothesized that the DN is implicated in the pathophysiology of ALS. A cohort of 127 partic

Magnetic resonance imaging of cerebral hemiatrophy.

Twenty-one cases of cerebral hemiatrophy were studied using 1.0 T magnetic resonance imaging (MRI). In addition to showing the generalized decrease in volume of the cerebral hemisphere, three patterns of cerebral hemiatrophy were noted: pattern I, diffuse cortical and subcortical atrophy (six cases):pattern II, diffuse cortical atrophy associated with expanded porencephalic cysts (10 cases); pattern III, old infarction with necrosis in the territory of the middle cerebral artery (five cases). Pattern I and pattern II were believed to have resulted from hypoxic-ischemic encephalopathy with subc
