Zinc chelation therapy mitigates muscle fibrosis and improves functional outcomes in a murine model of crush syndrome.

BACKGROUND: The survivors from crush syndrome are often disabled due to the affected limbs, with excessive fibrosis changes at damaged muscle tissue. We previously reported that a zinc chelator (N,N,N',N'-tetrakis-(2-pyridylmethyl)-ethylenediamine: TPEN) administration to the crush syndrome mice model inhibited neutrophil activation induced by ischemia-reperfusion, alleviating the inflammatory response. In the present study, we investigated the effects of TPEN administration on fibrosis in affected limb muscles. METHODS: Eight-week-old C57BL/6J mice were subjected to bilateral hindlimb compre

Baseline characteristics including blood and urine metal levels in the Trial to Assess Chelation Therapy 2 (TACT2).

BACKGROUND: The reduction in cardiovascular disease (CVD) events with edetate disodium (EDTA) in the Trial to Assess Chelation Therapy (TACT) suggested that chelation of toxic metals might provide novel opportunities to reduce CVD in patients with diabetes. Lead and cadmium are vasculotoxic metals chelated by EDTA. We present baseline characteristics for participants in TACT2, a randomized, double-masked, placebo-controlled trial designed as a replication of the TACT trial limited to patients with diabetes. METHODS: TACT2 enrolled 1,000 participants with diabetes and prior myocardial infarcti

Effects of iron chelation therapy on the clinical course of aceruloplasminemia: an analysis of aggregated case reports.

BACKGROUND: Aceruloplasminemia is a rare genetic iron overload disorder, characterized by progressive neurological manifestations. The effects of iron chelation on neurological outcomes have only been described in case studies, and are inconsistent. Aggregated case reports were analyzed to help delineate the disease-modifying potential of treatment. METHODS: Data on clinical manifestations, treatment and neurological outcomes of treatment were collected from three neurologically symptomatic Dutch patients, who received deferiprone with phlebotomy as a new therapeutic approach, and combined wi

Effect of chelation therapy on the neuropsychological and behavioral development of lead-exposed children after school entry.

OBJECTIVE: Some children in the United States continue to be exposed to levels of lead that increase their risk for lowered intellectual functioning and behavior problems. It is unclear whether chelation therapy can prevent or reverse the neurodevelopmental sequelae of lead toxicity. The objective of this study was to determine whether chelation therapy with succimer (dimercaptosuccinic acid) in children with referral blood lead levels between 20 and 44 microg/dL (0.96-2.12 micromol/L) at 12 to 33 months of age has neurodevelopmental benefits at age 7 years. METHODS: The Treatment of Lead-Exp

Genetic or pharmacological iron chelation prevents MPTP-induced neurotoxicity in vivo: a novel therapy for Parkinson's disease.

Studies on postmortem brains from Parkinson's patients reveal elevated iron in the substantia nigra (SN). Selective cell death in this brain region is associated with oxidative stress, which may be exacerbated by the presence of excess iron. Whether iron plays a causative role in cell death, however, is controversial. Here, we explore the effects of iron chelation via either transgenic expression of the iron binding protein ferritin or oral administration of the bioavailable metal chelator clioquinol (CQ) on susceptibility to the Parkinson's-inducing agent 1-methyl-4-phenyl-1,2,3,6-tetrapyridi

Toward a Rational Design of Polyamine-Based Zinc-Chelating Agents for Cancer Therapies.

In vitro viability assays against a representative panel of human cancer cell lines revealed that polyamines L1a and L5a displayed remarkable activity with IC50 values in the micromolar range. Preliminary research indicated that both compounds promoted G1 cell cycle arrest followed by cellular senescence and apoptosis. The induction of apoptotic cell death involved loss of mitochondrial outer membrane permeability and activation of caspases 3/7. Interestingly, L1a and L5a failed to activate cellular DNA damage response. The high intracellular zinc-chelating capacity of both compounds, deduced

Concomitant immune-related events in Wilson disease: implications for monitoring chelator therapy.

BACKGROUND AND AIMS: Current guidelines favor the use of chelating agents (d-penicillamine, trientine) in first line therapy of symptomatic Wilson disease patients. Development of chelator induced immunological adverse events are a concern especially under d-penicillamine therapy. This study assessed the prevalence of co-existing or therapy-related immune-mediated diseases in Wilson disease patients, and evaluated the role of antinuclear antibodies in therapy monitoring. METHODS: We retrospectively analyzed 235 Wilson disease patients. Medical regimens were classified and analyzed in relation

Restrictions on the applicability of mixed ligand chelate therapy (MLC) in acute cadmium intoxication.

An experimental study of the comparative effectiveness of single and mixed ligand chelates as antidotes for acute cadmium poisoning reveals few appreciable differences when the cadmium (as cadmium chloride or acetate) is administered ip. In each case the mixed ligand chelate system showed little or no improvement over the results obtained with the most effective of the individual components. This procedure, using mixed ligand chelate systems, may well be one which is limited to conditions more narrowly similar to those reported by Schubert and Derr (1978). It does not appear to possess the bro

Role of Oxidative Stress in the Worsening of Neurologic Wilson Disease Following Chelating Therapy.

Patients with neurologic Wilson disease (NWD) may worsen on treatment, but there is no study evaluating the role of oxidative stress. We report the role of plasma glutathione (GSH), total antioxidant capacity (TAC) and malondialdehyde (MDA) in the worsening of NWD following treatment. Fifty-one treatment-naïve NWD patients were subjected to detailed clinical evaluation. The severity of NWD was noted, and dystonia was measured by Burke-Fahn-Marsden (BFM) score. Their hematological, serum chemistry, ultrasound abdomen and cranial MRI changes were noted. Plasma GSH, TAC and MDA, serum free copper

Aluminum and other metals in Alzheimer's disease: a review of potential therapy with chelating agents.

Alzheimer's disease (AD) is characterized by the presence of neuritic plaques and neurofibrillary tangles in the brain. Although the causes of AD remain still unknown, it seems that certain environmental factors may be involved in the etiology and pathogenesis of the disease. While AD is associated with the abnormal aggregation of beta-amyloid protein in the brain, evidence shows that certain metals play a role in the precipitation and cytotoxicity of this protein. Among these metals, the potential role of aluminum as a possible ethiopathogenic factor in AD has been especially controversial. T

Utilization Patterns and Outcomes From Iron Chelation Therapy in Elderly Patients With Myelodysplastic Syndromes: A Population Analysis.

BACKGROUND: Iron overload from red blood cell (RBC) transfusions is a source of morbidity and mortality in myelodysplastic syndromes (MDS). Evidence supporting iron chelation therapy (ICT) is limited for elderly patients. MATERIALS AND METHODS: We analyzed real-world ICT utilization and outcomes in the U.S. Surveillance, Epidemiology, and End Results (SEER)-Medicare population. Between 2007 and 2017, MDS patients ≥66 years with ≥2-year survival was identified. Documented iron overload and/or cumulative ≥20 RBC within ≤2 years defined ICT eligibility. Survival, acute myeloid leukemia (AML), an

Changes in inflammatory cytokines, antioxidants and liver stiffness after chelation therapy in individuals with chronic lead poisoning.

BACKGROUND: Chronic exposure to lead causes lead to accumulate mainly in the liver. In vivo studies have shown that lead toxicity is related to alterations in the inflammatory response. We aimed to evaluate the association between lead poisoning and liver fibrosis as well as the change in the degree of liver fibrosis, levels of inflammatory mediators and glutathione (GSH) after chelation therapy. METHODS: Workers from a battery factory who were exposed to lead for > 12 months and had a blood lead level (BLL) > 70 μg/dL were enrolled (n = 86) in the study. Participants underwent chelation ther

What is known about deferasirox chelation therapy in pediatric HSCT recipients: two case reports of metabolic acidosis.

To date, in pediatric field, various hematological malignancies are increasingly treated with allogeneic hematopoietic stem cell transplantation (allo-HSCT). Iron overload and systemic siderosis often occur in this particular cohort of patients and are associated with poor prognosis. We describe herein the case of two allo-HSCT patients, on treatment with deferasirox; they showed histopathological elements compatible with venoocclusive disease or vanishing bile duct syndrome in ductopenic evolution before deferasirox started. The first patient developed drug-induced liver damage with metabolic

Acute cardiac decompensation in a patient with beta-thalassemia and diabetes mellitus following cessation of chelation therapy.

Patients with higher liver iron stores are likely to have a worse cardiac outcome following noncompliance with chelation. Cardiovascular magnetic resonance identifies myocardial siderosis allowing optimization of iron chelation regimes. Diabetes puts thalassemic patients at increased risk of myocardial fibrosis. Dual chelation therapy with deferoxamine and deferiprone offers improved cardiac outcomes.
