Talking About the Anti-Trans Sociopolitical Climate with Transgender and Gender Diverse Children and Their Caregivers.

BACKGROUND: Transgender and gender diverse (TGD) children and adolescents in the United States are currently experiencing an oppressive sociopolitical climate that is likely impacting their mental wellbeing. TGD children, defined as 5- to 10-year-olds, have the capacity to be aware of the anti-trans sociopolitical climate and, within the context of developmental readiness, TGD children and their caregivers may benefit from having conversations about the sociopolitical climate with a mental health provider. However, there are no clinical guidelines for how mental health providers may approach c

[More recent aspects of treatment for hypothyroidism and goitre in children and adolescents (author's transl)].

According to more recent knowledge L-thyroxine alone is recommended for thyroid hormone replacement therapy, no longer the combination of L-thyroxine and triiodothyronin, since L-thyroxine is converted to triiodothyronine in the periphery of the body. Additional application of triiodothyronine causes unphysiological elevations of blood levels of triiodothyronine. When changing from the combined thyroid hormone therapy to treatment with L-thyroxine alone it is enough in most cases to replace the previous amount of L-thyroxine only; previous recommendations for the dosage of thyroid hormone were

[Current status of orthotic management of children with meningomyelocele].

The supplying of Spina bifida children with orthotic devices leads to a remarkable success only, if it is based on the preexisting possibilities of the overall development and the present situation of the child. To realize the aim of maximum autonomy in standing and walking this overall analytic perspective is a must for all therapeutic planing and decisions. Therefore, static and dynamic orthoses like hip guidance-combined with above-knee orthoses, standing and walking-aids like reciprocating gait orthoses etc. are decisive assumptions. The most important characteristics of all orthoses are,

Protocol for Infant Massage in Home Settings: An e-Delphi Approach for Consensus Guidance Integrating Traditional Wisdom with Modern Medicine.

Infant massage is a highly prevalent traditional practice in India and other parts of Asia. Clear guidance on safe and effective uses of infant massage is lacking especially in the contemporary times when the traditional knowledge is on the verge of extinction and preparations may differ from in the past. This paper presents a consensus guidance in the form of a standardized protocol for routine massage of infants in home settings. Furthermore, a feasible method to develop an integrative protocol involving traditional and modern medicine experts is described. A modified e-Delphi method was use

[Studies on the socio-medical and psychological situation of persons suffering from osteogenesis imperfecta tarda (author's transl)].

A group of 22 patients suffering from osteogenesis imperfecta tarda were evaluated on their school and vocational education, sport activities as well as personality development with the help of follow-up studies, questionnaires or medical records. 21 patients had at least received elementary school education and had been average or good pupils. Five of six patients who had meanwhile reached adulthood had received higher education and vocational training. Owing to their disease five of six patients had to give up their original jobs or occupational plans. There was only one female patient who h

Divergent approaches in the vaccination of recently arrived migrants to Europe: a survey of national experts from 32 countries, 2017.

BackgroundMigrants within the European Union and European Economic Area (EU/EEA) may be underimmunised and lack documentation on previous vaccinations. We investigated approaches to vaccination in recently arrived adult and child migrants, and guideline availability and implementation. Methods: Between March and May 2017, a national vaccination expert from every EU/EEA country and Switzerland completed an electronic questionnaire. We used descriptive analyses to calculate percentages, and framework analysis to synthesise free-text responses. Results: We approached 32 countries (response rate 1

[Therapy of childhood schizophrenia].

Due to the multifacet genesis and variability of clinical phenomenology, the therapy of childhood schizophrenia must be multidimensional. Formerly applied techniques like electroshock- and insulin-therapy are now replaced by pharmaco-therapy, primarily with phenothiazines, butyrophenones and chlorprothixens. The dosage depends on age, body weight or body surface. Because of extrapyramidal motor side effects, combinations with anticholinergic drugs may be necessary. Psychopharmaco-therapy alone, however, is insufficient. High emphasis must be placed on psychotherapy and educational guidance and

The Role of Early Maladaptive Schemas in the Appearance of Psychological Symptomatology in Adult Women Victims of Child Abuse.

Child abuse affects people's ways of thinking, feeling, and observing the world, resulting in dysfunctional beliefs and maladaptive schemas. Thus, consequences of child abuse may persist during adulthood. Therefore, the aim of this study was to analyze the psychological consequences (anxiety, phobic anxiety, depression, and hopelessness) of different types of maltreatment (physical, sexual, and emotional abuse and physical and emotional neglect) and to study the role of early maladaptive schemas in the onset of symptomatology in adult female victims of child abuse. The sample consisted of 75 w

Intravenous paracetamol overdose: two case reports and a change to national treatment guidelines.

Two cases of 10-fold accidental overdose with intravenous paracetamol are presented. Case 1: A 5-month-old child with intussusception received 90 mg/kg intravenous paracetamol over an 8 h period. She was not initially treated with an antidote and developed hepatic impairment. Case 2: A 6-month-old child received a single dose of 75 mg/kg intravenous paracetamol. The child was treated with N-acetylcysteine and remained well without hepatic impairment. Therapeutic errors such as 10-fold overdosing are relatively common in children. Case 1 demonstrates that intravenous paracetamol is a potentiall

Dyspnea in Children with Serious Illness.

Dyspnea is a common and distressing symptom in children living with serious illnesses, particularly near the end of life, and requires thoughtful, family-centered assessment and management. A stepwise nursing approach to pediatric dyspnea is explored, which includes clinical assessment, identification of potentially reversible causes, and shared decision-making grounded in the child's symptoms, goals of care, and family context. Using a case study of a child with metastatic osteosarcoma, practical strategies are highlighted for symptom management, including nonpharmacologic interventions, comm

Clinical guidance for diagnosis and management of suspected Pediatric Acute-onset Neuropsychiatric Syndrome in the Nordic countries.

Pediatric acute-onset neuropsychiatric syndrome is a clinical concept used to describe a subgroup of children with sudden onset of psychiatric and somatic symptoms. The diagnostic term and especially management of children differs depending on the clinical setting to which they present, and the diagnosis and management is controversial. The aim of this paper is to propose a clinical guidance including homogenous diagnostic work-up and management of paediatric acute onset neuropsychiatric syndrome within the Nordic countries. The guidance is authored by a Nordic-UK working group consisting of p

Technology-enabled anticipatory guidance and peer support in empowering parents to prevent early childhood caries: a randomized controlled trial.

INTRODUCTION: Dental caries is one of the most prevalent chronic diseases of childhood. Early childhood caries (ECC), defined as onset of dental caries before 6 years of age, is preventable and reversible in the early stages with proper interventions. The purpose of the study is to assess the clinical effectiveness and cost-effectiveness of technology-enabled anticipatory guidance and peer support in empowering parents to establish good diets, feeding and oral hygiene practices to prevent ECC. METHODS AND ANALYSIS: Mother-child dyads will be recruited from pre-natal clinics and post-natal war

Fractures in suspected child abuse: A critical review of the evidence cited by the American Academy of Pediatrics and Royal College of Paediatrics and Child Health.

We critically evaluated the evidence cited by the American Academy of Pediatrics (AAP) and the Royal College of Paediatrics and Child Health (RCPCH) supporting the diagnostic significance of fractures in suspected physical child abuse. Primary studies cited by the AAP Clinical Report and the RCPCH Systematic Review were reviewed with respect to study design, methods used to classify cases as abuse, and the potential for incorporation bias and outcome misclassification. Sixty-seven studies were identified, of which 35 reported measures of diagnostic utility. All were observational, predominantl

Preventive health services for young children in Israel: historical development and current challenges.

PURPOSE: Pediatric preventive health services are delivered in many different formats throughout the world. In Israel, these services for young children are provided in designated Maternal Child Health Clinics (MCHC). The history and operation of Israel's MCHC have been described primarily in the Hebrew language medical literature with most of these reports being over a decade old. This paper provides an up to date summary of the evolution and current care in Israel's one-hundred-year old model for the provision of preschool preventive health services. As these clinics have been recognized by

Safeguarding children: why an issue for pediatric anesthetists?

Child maltreatment and the responsibility of heath care professionals to assist in prevention and diagnosis is currently center stage in the United Kingdom. This subject has a very large literature base. Anesthetists were one of the first groups to have specific guidance on this subject, and key competencies are now part of the core curriculum for our trainees. This article seeks to briefly define maltreatment and provide statistics that outline the scale of the problem, and includes discussion of risk factors and recognition. We have focused on physical abuse and have provided a separate sect

[An asymmetrical face due to torticollis in two young children].

In 2 girls, 4 years and 8 months of age, respectively, torticollis had been diagnosed a few weeks after birth. Kinesiotherapy, given for a period of 1 year and 6 months, respectively, had little effect. Both girls were then referred to the Orthopaedic Outpatient Clinic where a shortened sternocleidomastoid muscle and an asymmetrical face were diagnosed. The shortened muscle was cleaved surgically. Over half a year later, the face of the older child was still asymmetrical but that of the younger child symmetric. It is recommended that children with torticollis due to a shortened sternocleidomas

Does the use of ultrasound guidance during percutaneous Achilles tendon tenotomy improve outcomes of clubfoot management? A randomized controlled trial.

Achilles tenotomy is a crucial aspect in the non-operative management of clubfoot as per Ponseti's casting protocol. Achilles tenotomy is routinely carried out percutaneously in a blind manner without any image guidance. This study aimed to determine the role of ultrasound-guided percutaneous Achilles tenotomy (PAT) in improving outcomes in clubfoot management. There are no previous studies that have explored the role of image guidance in PAT. This prospective, single-center randomized control trial included 50 clubfoot cases (74 feet) planned for PAT at a mean age of 30.2 months. A single exp

Managing pediatric-onset multiple sclerosis in an austere setting: A case report.

Pediatric-onset multiple sclerosis (POMS) is the most common demyelinating disease in children. Patients suffer from physical disability, cognitive impairment, and psychosocial challenges. Management requires a multidisciplinary care team. Here we present a case of an 11-year-old boy with POMS who relocated to Guam prior to initiation of a disease-modifying treatment and who experienced a flare without immediate access to an MRI or a child neurologist. Care required the combined efforts of ophthalmology, pediatrics, and emergency medicine in Guam, real-time remote guidance by child neurology,

Skin care of the healthy newborn.

Routine care of a newborn may be an intimidating task for new parents. This routine care includes tending to the skin of the infant. Maintaining a healthy, intact cutaneous barrier is important psychologically for the parent and medically for the child. Clinicians should be able to offer guidance concerning the basics of skin care, to dispel any misconceptions concerning baby products, and to optimize cutaneous integrity for the comfort and well being of the infant.
