Evaluating penicillin allergy clinical decision-making tools to enhance penicillin allergy delabeling.

BACKGROUND: Patient-reported penicillin allergy is associated with adverse outcomes. OBJECTIVE: We aimed to evaluate clinical decision-making tools used to risk-stratify reported penicillin allergies for delabeling. METHODS: Retrospective chart review of patients with reported penicillin allergies who underwent testing from October 9, 2020, to October 8, 2022 was completed at a tertiary referral health care system. Follow-up surveys were conducted to assess penicillin use after testing. The PEN-FAST clinical decision rule, 1-1-1 rule, Siew score, and Vanderbilt criteria were evaluated. RESU

Molecular residual disease (MRD) testing: advancing clinical decision-making in breast cancer.

Circulating tumor DNA (ctDNA) is emerging as a clinically meaningful biomarker across multiple solid tumors, including breast cancer. Advances in personalized, tumor-informed whole-genome sequencing have enabled highly sensitive detection of ctDNA, allowing for more precise assessment of tumor burden. Across treatment settings, ctDNA testing has demonstrated consistent prognostic value in patients with breast cancer. In the neoadjuvant setting, ctDNA status is strongly prognostic at baseline and following completion of therapy. After definitive surgery, detection of molecular residual disease

HealthCare Complexity Index (HCIn): A Clinical Decision-Making Tool for Primary Care.

BACKGROUND: Population ageing, multimorbidity, and chronic conditions have increased the demand for person-centred care in Primary Health Care. Current stratification systems rely on diagnoses or resource utilization and fail to capture the multidimensional nature of care complexity, which also depends on functional, social, environmental factors. OBJECTIVE: To develop a HealthCare Complexity Index using routinely collected electronic health record (EHR) data and the Person-Centred Care Knowledge Model. METHODOLOGY: A retrospective, cross-sectional observational study was conducted in Primar

CIRRHOSIS-ASSOCIATED SARCOPENIA: INTEGRATING IMAGING, FUNCTION, AND METABOLISM FOR CLINICAL DECISION-MAKING.

BACKGROUND: Sarcopenia is a highly prevalent and clinically impactful complication of cirrhosis, affecting 30-70% of patients depending on diagnostic criteria and disease stage. It is strongly associated with increased mortality, hepatic decompensation, infections, waitlist dropout, and inferior posttransplant outcomes. Despite growing recognition, the field remains limited by heterogeneous definitions, lack of standardized cutoffs, and inconsistent integration of functional and structural assessments. OBJECTIVE AND METHODS: This narrative review synthesizes mechanistic pathways linking cirrh

Clinical decision-making skill in diagnosing of oral diseases among medical interns.

BACKGROUND: Oral mucosal diseases are frequently misdiagnosed due to overlapping clinical presentations. Given the important role of physicians in the initial assessment of oral conditions, evaluating diagnostic reasoning among medical trainees is essential. This study aimed to assess the clinical decision-making skills of medical interns in diagnosing oral diseases. METHODS: This cross-sectional analytical study was conducted among 66 medical interns at Guilan University of Medical Sciences using a census sampling approach. Data were collected using a two-part questionnaire consisting of dem

Minimally Invasive Management of a Mullerian Anomaly: A Clinical Decision-Making Tree.

OBJECTIVE: To describe the presentation and management of a symptomatic Müllerian duct abnormality in a teenager and propose an evidence based clinical decision-making tree on when to treat based on a thorough literature review. CASE PRESENTATION: A 17-year-old nulligravid woman presented with a 5-year history of menorrhagia, dysmenorrhea and pelvic pain with medical history significant for a congenitally absent left kidney and ureter. Pelvic ultrasound suggested a bicornuate uterus, but a pelvic MRI with/without contrast diagnosed her with a unicornuate uterus with a noncommunicating left ru

Digital case-based learning for improving clinical decision-making in tuberculosis care.

This study compares a digital case-based learning module with standard training for improving guideline-concordant clinical decisions in tuberculosis (TB) care. Evidence on whether digital training improves TB decision accuracy is limited when both groups complete the same scored case assessment. In a two-arm parallel trial, clinicians or trainees were assigned to either the digital module or standard lecture or continuing medical education training, then completed a standardized vignette test within 7 days. Guideline-concordant decision accuracy was higher with the digital module, with an int

Neoplasms of the Cervical Spine: Diagnostic Workup and Clinical Decision-making Algorithms.

STUDY DESIGN: Narrative review. OBJECTIVE: To synthesize current evidence on diagnostics and clinical decision-making for patients with cervical spine neoplasms. BACKGROUND: Although the cervical spine is disproportionately affected by primary spinal tumors and is the predominant site for intramedullary spinal cord tumors, there remains a relative paucity of reviews focused specifically on cervical spine neoplasms. METHODS: PubMed and Scopus were searched using a combination of terms related to cervical spine neoplasms, classification schemas, diagnostic evaluation, staging, and clinical de

Refining the Clinical Pathway for Nasotracheal Intubation: An Updated Decision Making Algorithm.

Nasotracheal Intubation (NTI) is a common route of airway management in many situations. Over the years, numerous techniques and approaches have been described in performing NTI safely and effectively, including many innovative techniques being published in recent years. However, there hasn't been a summary of the recent approaches to NTI, especially in an easy, clinically applicable decision making format. In fact, the last algorithmic approach to nasal intubation in the literature was published in 2008. This comprehensive review details an updated analysis of NTI techniques along with a new

Antithrombotic Therapy in Peripheral Artery Disease: Risk Stratification and Clinical Decision Making.

Patients with peripheral artery disease (PAD) are an underrecognised group with significant thrombotic risk. This risk is modifiable with the use of aggressive secondary preventative efforts, including optimisation of antithrombotic therapy. Appropriate antithrombotic selection for patients with PAD requires appropriate assessment of thrombotic and bleeding risk. Recent Canadian guidelines have recommended dual pathway therapy initiation for stable PAD and post-revascularisation patients. However, there is ongoing discussion about how to identify PAD patients who stand to benefit most from the

Variability in clinical decision-making for ventricular assist device implantation in pediatrics.

BACKGROUND: Minimal data exist on clinical decision-making in VAD implantation in pediatrics. This study aims to identify areas of consensus/variability among pediatric VAD physicians in determining eligibility and factors that guide decision-making. METHODS: An 88-item survey with clinical vignettes was sent to 132 pediatric HT cardiologists and surgeons at 37 centers. Summary statistics are presented for the variables assessed. RESULTS: Total respondents were 65 (72% cardiologists, 28% surgeons) whose centers implanted 1-5 (34%), 6-10 (40%), or >10 (26%) VADs in the past year. Consensus va

First-time Glenohumeral Dislocations: Current Evidence and Considerations in Clinical Decision Making.

The decision to manage first-time shoulder dislocations conservatively or operatively has become increasingly complex because of conflicting literature. Although shoulder dislocations have traditionally been managed with reduction and immobilization, recent evidence has suggested high rates of subsequent recurrence. Surgical intervention is thought to better restore stability and decrease recurrence rates; however, it also has the potential for additional morbidity and financial cost. As such, recent literature has sought to better define patient risk profiles to identify optimal candidates fo
