OBJECTIVE: To review the contemporary surgical issues in paediatric cochlear implantation (CI) based on published evidence.
DESIGN: Narrative literature review.
RESULTS: Surgical challenges in paediatric CI are discussed, with respect to post meningitic labyrinthitis ossificans; cochlear malformation; cochlear implantation in infants; auditory neuropathy and cochlear nerve deficiency; bilateral cochlear implantation; hearing preservation; otitis media; and device failure.
CONCLUSION: Early CI is recommended if bacterial meningitis causes profound sensorineural hearing loss (SNHL). CI in coc

We report a case of intermittent dysfunction in a 10-years-old boy, implanted with MedEL(®) cochlear implant. Few weeks after the surgery the boy described short and intermittent episodes of implant dysfunction with rapid return to a normal function. No evidence for any electric or neural dysfunction was found. After few weeks, a clinical link was discovered to episodes of sneeze or nose blowing. Clinical and surgical implications are discussed.

OBJECTIVES: Few studies have addressed the benefits of cochlear implantation for the small group of patients with bilateral, end-stage Meniere's disease, or unilateral disease with contralateral hearing loss from another cause. Our retrospective study evaluates the effectiveness and post-operative performance in these Meniere's disease patients and discusses these findings relative to other post-lingually deafened adults.
METHODS: Among 456 adults who received cochlear implants, we identified eight (1.7%) patients with Meniere's disease who met clinical indications for implantation and a cont

Cochlear implantation is associated with deterioration in hearing. Despite the fact that the damage is presumed to be of sensory origin, residual hearing is usually assessed by air-conduction thresholds alone. This study sought to determine if surgery may cause changes in air- and bone-conduction thresholds producing a mixed-type hearing loss. The sample included 18 patients (mean age 37 years) with an air-bone gap of 10 dB over three consecutive frequencies and measurable masked and reliable bone-conduction thresholds of operated and non-operated ears who underwent cochlear implant surgery. OBJECTIVE AND IMPORTANCE: To describe the role of cochlear implantation in patients with 3p deletion syndrome. Additionally, this paper will review non-mastoid approaches to the cochlea potentially used in cases of mastoid abnormalities.
CLINICAL PRESENTATION: A 23-month-old child with 3p deletion syndrome (3pDS) presented with suspected hearing loss and characteristic craniofacial abnormalities. Bilateral sensorineural hearing loss (SNHL) was confirmed. Temporal bone computed tomography showed poorly developed mastoids bilaterally.
INTERVENTION: Cochlear implantation was successfully undert

OBJECTIVE: Susac syndrome comprises a triad of vestibulocochlear dysfunction, retinopathy and multifocal encephalopathy, which is characterised pathophysiologically by microangiopathy of the ear, retina and brain. Diagnosis is confirmed by magnetic resonance imaging of the brain and ophthalmological examination, which reveals branch retinal artery occlusion. Hearing loss persists in 90 per cent of patients. We present a case of successful hearing rehabilitation by cochlear implantation in a young woman with this syndrome.
CLINICAL PRESENTATION: A 36-year-old woman presented with neurological

INTRODUCTION: Constant histological changes in otosclerosis lead to progressive hearing loss which may end up in a profound hearing loss and then be treated by means of cochlear implants. These progressive changes could be followed by changes in cochlear implants fitting and speech discrimination results over the years.
OBJECTIVES: The aim of the study is to correlate the progressive histological changes to the cochlear implant clinical outcomes (fitting and speech discrimination results). Also main complications (facial nerve stimulation and difficulties at insertion) and new complications w

OBJECTIVE: Sensorineural hearing loss resulting from otosclerosis, meningitis, chronic otitis media, autoimmune ear disease, and trauma can be associated with partial or total obstruction of the cochlear scalae. Multichannel cochlear implantation may be difficult in a cochlea with an obstructed scala tympani. The purpose of this study is to determine the safety and efficacy of scala tympani electrode insertion.

OBJECTIVE: Cochlear implantation is now a well-accepted procedure in the paediatric population. The objective of this study was to evaluate the use of small postaural incision for cochlear implantation in children.
METHODS: This study is a retrospective review of patients who had cochlear implantation using small postaural incision. Patient records of 34 patients who were implanted using this incision are reviewed. The technique involves drilling the bony well for the implant inside a small subperiosteal pocket standing opposite the operated ear. The rest of the steps were the same as for sta

OBJECTIVE: To describe the outcome of cochlear implantation in a cohort of pediatric patients with a history of otitis media.
STUDY DESIGN: Retrospective chart review and follow-up phone survey.
METHODS: The medical records of patients younger than 48 months of age who underwent cochlear implantation were reviewed for demographics, complications, and history of otitis media and bilateral myringotomy and ventilation tubes. A follow-up phone survey was performed to determine the long-term natural history of otitis media.
RESULTS: The records of 126 profoundly deaf pediatric patients were revi

The aim of this work was to determine the influence of cochlear implantation (CI) on vestibular canal and otolithic function. Between 1995 and 1999, 15 patients (6 females, 9 males; 9-77 years old) underwent a vestibular examination before and after CI. Electronystagmography was performed between 5 and 8 days after CI in 9 patients, and with a time delay of 2-24 months in 10 patients. Pre- and postoperative evaluation included electronystagmography with caloric (44 degrees C, 30 degrees C, ice-water) and pendular rotatory testing. Otolithic function was measured postoperatively using off-verti

The aim of the study was to review the indications, techniques and results of cochlear implant (CI) treatment with the Nucleus CI multi-channel electrode for the first children operated on in Denmark. The study material comprised 16 children (5 girls, 11 boys). Their median age at the time of CI operation was 27 months. Anacusis was caused by pneumococcal meningitis in 31% of the children in this relatively young prelingual material. Obliteration of the cochlea was found in two children, indicating the need for surgery as soon as possible after the verification of post-meningitis, total deafne

High-resolution computed tomography (HRCT) is the standard imaging technique used in cochlear implantation. However, cochlear and retrocochlear soft-tissue abnormalities may not be detected with HRCT alone. To determine whether magnetic resonance imaging (MRI) provides clinically significant information in addition to HRCT in the evaluation of candidates for cochlear implants, we performed a prospective study of 13 consecutive patients with cochlear implant patients receiving preoperative, high-resolution fast spin-echo T2-weighted MRI scans of the temporal bone. MRI identified unanticipated c

In 1981, our group, as part of a Food and Drug Administration investigator team directed by William House, performed Illinois' first cochlear implantation. We have since performed cochlear implantations in ten other persons. Since May 1980, approximately 35 severely to profoundly deaf persons per year have been referred to our clinic for evaluation. A small number of these patients had far-advanced otosclerosis. In these cases, exploratory tympanotomy and stapedectomy followed by rehabilitation with a hearing aid was a more appropriate management than cochlear implantation. Case histories and

OBJECTIVE: To understand the family's experience of a child who uses a cochlear implant (CI). Specifically, to identify the difficulties, changes, and feelings entailed by deafness and the use of the CI; the coping strategies; and to understand the role of the family for the child with a CI.
METHOD: Qualitative research, using Symbolic Interactionism and Straussian Grounded Theory as the theoretical and methodological frameworks, respectively. Data collection instrument: semi-structured interview. A total of 9 families (32 individuals) participated in the study. The children's ages ranged fro
