Not all chronic urticaria is "idiopathic"!

The aetiology of chronic urticaria in the majority of patients is elusive. The cause of physical urticarias (dermographism, delayed pressure urticaria, cold urticaria) is unknown. We have identified a subset of chronic "idiopathic" urticaria patients, representing approximately 30% of the total in which the disease is caused by the presence of IgG autoantibodies against the high affinity IgE receptor (FcepsilonRIalpha). This functional autoantibody stimulates normal (albeit vicarious) activation of mast cells and basophils via FcepsilonRI, causing whealing and angioedema. Anti-FcepsilonRI-posi

Chronic Urticaria and Angioedema: Masqueraders and Misdiagnoses.

Chronic urticaria is a common condition presenting with intensely pruritic wheals. Although individual lesions resolve within 24 hours, by definition, chronic urticaria lasts for a duration of at least 6 weeks. Both spontaneous and inducible forms exist. In the spontaneous variant, chronic urticaria occurs in the absence of clearly identifiable triggers. In chronic inducible urticaria, specific triggers may include dermatographism, cholinergic (heat), cold, exercise, delayed pressure, and solar. Extensive laboratory evaluation for chronic spontaneous urticaria is not required unless indicated

[Usefulness of photochemotherapy in allergic diseases of the skin].

PUVA treatment was applied in 16 patients with chronic urticaria (7 with solar urticaria, 2 with cold urticaria and 7 with urticaria of unknown etiology), 26 with chronic disseminated eczema and 41 with atopic dermatitis. In all 9 patients with physical urticaria, a clear-cut improvement was observed. The results from the treatment of chronic urticaria of unknown etiology were considerably poorer (improvement in 2 out of 7 cases). In patients with chronic disseminated eczema, a full remission was present in 5 and considerable improvement in 20 cases. In group of 41 patients with atopic dermati

Neutrophilic urticaria: clinical features, histological changes and possible mechanisms.

Neutrophilic urticaria (NU) is a histologically defined entity, but its clinical and pathogenetic aspects are poorly understood. We investigated 22 NU patients whom we identified by examining 118 biopsies of weals. The patients comprised 11 of 20 with acute urticaria, nine of 49 with chronic urticaria, one of 10 with cold urticaria and one of 10 controls undergoing prick tests. Clinically, NU patients had a shorter mean duration of disease than other urticaria patients and significantly increased erythrocyte sedimentation rate and leucocytosis. Histologically, not only neutrophil counts, but t

Physical urticaria/angioedema: an experimental model of mast cell activation in humans.

Urticaria and angioedema may occur in skin and mucus membranes when mast cells are activated by various physical stimuli, including trauma, pressure, vibration, light, cold, heat, and (in rare cases) water. Experimental challenge of patients with cold-induced and cholinergic urticaria/angioedema in particular provides an in vivo model of mast cell activation in humans. This model synthesizes observations of the evolution of clinical manifestations, histologic analysis of tissue alterations, measurement of mediators released into the circulation, and assessment of leukocyte motility. The model

Case Report: de novo in-frame deletion in PLCG2 gene: a case report of B-cell lymphopenia, pulmonary bullae, and cutis laxa.

Phospholipase C gamma 2 (PLCG2) gene mutations might cause PLCG2-associated antibody deficiency and immune dysregulation (PLAID)/autoinflammation and PLCG2-associated antibody deficiency and immune dysregulation (APLAID) syndrome. They are two forms of autosomal-dominant immune dysregulation (ID). APLAID patients are usually characterized by skin lesions, pulmonary involvement, and musculoskeletal, ophthalmic, and gastrointestinal tract symptoms, but unlike PLAID patients, these patients do not present with cold urticaria or autoimmunity. Here, we report a 25-year-old man with B-cell lymphopen

Cold, Wind, and Sun Exposure: Managing-and Preventing-Skin Damage.

Outdoor athletes, through the course of their training and competition, are constantly battling environmental effects. Cold and wind exposure can lead to frostbite, which may not be immediately apparent if the skin grows numb. Other common cold-induced conditions include Raynaud's phenomenon, cold-induced urticaria, and chilblains. Summer athletes and athletes participating at higher altitudes need to be especially vigilant for sunburn. Physicians should know when to withhold athletes from play to prevent further damage. By teaching prevention strategies, clinicians can help their patients saf

[Sudden death in urticaria pigmentosa].

A case of sudden death of a 25-year-old man, with a six years history of Urticaria pigmentosa, is reported. On a cold morning in November he was found lying dead on the lawn. Histology revealed no signs of systemic mastocytosis. The biochemical investigations (high free histamine level in urine) in addition to a BAC of 0,72 mg/g and the scene conditions (cold irritation) explained the mechanism of death, a systemic anaphylactoid reaction.

Plasma protease inhibitor and anaphylatoxin inactivator levels in chronic urticaria/angioedema and in patients experiencing anaphylactoid reactions to radiographic contrast media.

Since a protease inhibitor or anaphylatoxin inactivator deficiency might explain why certain individuals are prone to develop chronic urticaria/angioedema or anaphylactoid reactions to radiographic contrast media, serum alpha 1-protease inhibitor, alpha 1-antichymotrypsin, alpha 2-macroglobulin, inter-alpha-inhibitor, antithrombin III, alpha 2-plasmin inhibitor, C1 inhibitor, and serum carboxypeptidase N were assessed by immunologic or functional methods. These values all were within normal limits in both groups of patients except for a low mean alpha 1-protease inhibitor level in chronic idio

Mediator release in local heat urticaria.

We described the sixteenth reported case of local heat urticaria, in a 59-yr-old woman with erythema and angioedema upon contact with hot water or outdoor heat exposure. Immersing her hand in 39 degrees to 40 degrees C heated water resulted in an erythematous, angioedematous response sharply demarcated by the line of immersion and was associated with immediate increases in histamine concentration (18 to 135 ng/ml) and high molecular weight neutrophil chemotactic activity (two to five times prechallenge levels) in venous blood draining the challenge site. We suggest that the local heat urticari

Clinical and etiologic evaluation of the children with chronic urticaria.

BACKGROUND: Chronic urticaria (CU) is a skin disorder defined as daily or almost daily exhibition of pruritic and transient wheals that last for 6 weeks. CU is divided into two subtypes: chronic spontaneous urticaria (CSU) and chronic inducible urticaria (CIndU). OBJECTIVES: To evaluate the clinical features, possible causes, associated findings, and laboratory results of different subtypes of CU in children according to a new classification. METHODS: In this study, we evaluated the clinical features, laboratory investigations, and provocation tests of children with different subtypes of CU

Physical allergies and exercise. Clinical implications for those engaged in sports activities.

There are several allergic responses that may occur in susceptible individuals as a result of exposure to physical stimuli. Most of these conditions are mediated by vasoactive substances and usually result in symptoms of urticaria and/or angioedema. There are 2 such conditions that may occur as a direct result from exercise. The first of these is cholinergic urticaria. Patients with cholinergic urticaria experience punctate (2 to 4mm) hives which occur reproducibly with exercise or with passive warming, such as might occur in a steam bath or hot pool. Life-threatening hypotension or angioedema

Evidence for histamine release in chronic inducible urticaria - A systematic review.

BACKGROUND: Chronic inducible urticaria (CIndU) constitutes a group of nine different CIndUs in which pruritic wheals and/or angioedema occur after exposure to specific and definite triggers. Histamine released from activated and degranulating skin mast cells is held to play a key role in the pathogenesis of CIndU, but evidence to support this has, as of yet, not been reviewed systematically or in detail. We aim to characterize the role and relevance of histamine in CIndU. METHODS: We systematically searched 3 electronic databases (PubMed, Scopus, and Embase) for studies that reported increas

Histamine and the skin.

The role of histamine in urticaria is reviewed with particular reference to experiments on cold contact urticaria. In this model, histamine release clearly has a central role, but the failure of drugs which suppress histamine release to alleviate symptoms, other than itching, suggests that other pharmacological agents must be involved. Evidence is presented that prostaglandin E may play a role in some of these patients. The discovery that human blood vessels possess H2 receptors as well as H1 receptors is discussed with particular reference to the role of combined H1 and H2 pharmacological blo

Key differences between chronic inducible and spontaneous urticaria.

INTRODUCTION: The latest international EAACI/GA²LEN/EuroGuiDerm/APAAACI guideline for urticaria recommends limited laboratory testing for chronic spontaneous urticaria (CSU) and selective testing for only certain chronic inducible urticaria (CIndU) subtypes, though the rationale for these recommendations is poorly explained. This study aimed to improve the understanding of CIndU subtypes by comprehensively comparing their demographic, clinical, and laboratory characteristics with those of the better-characterized CSU. METHODS: We conducted a retrospective analysis of 567 patients (median age

Pathomechanisms in physical urticaria.

Various exogenous stimuli, e.g., rubbing, pressure, cold, heat, or electromagnetic waves, have been described to elicit whealing reactions, the so-called physical urticarias. They may occur as isolated diseases or in association with other types of urticaria. In many cases, the respective physical factors can be defined exactly, e.g., the degree of temperature changes or the range of eliciting ultraviolet wavelengths. In contrast, the underlying pathomechanisms are mostly still obscure. In the past, often contradictory results have been reported regarding the role of IgE, complement factors, h

Bruton's tyrosine kinase inhibition for food-induced anaphylaxis.

PURPOSE OF REVIEW: The recent approval of the Bruton's tyrosine kinase inhibitor (BTKi) remibrutinib for the treatment of chronic spontaneous urticaria opened a new therapeutic class of drugs for the allergy space. This review summarizes findings from recent BTKi clinical trials, with a focus on practical considerations for the practicing allergist. RECENT FINDINGS: In its phase 2 trial, 4  weeks of remibrutinib treatment demonstrated remarkable efficacy in preventing food-induced anaphylaxis to peanut in adults. Additionally, in a phase 3 trial for inducible urticarias, it improved symptoms

Assessment of urticaria and angio-oedema.

There are many types of urticaria and the principal form of assessment is by clinical history and examination. Urticarial weal formation involves acute, reversible vasodilatation and increased vascular permeability. If the process is deeper the more diffuse swelling is termed angio-oedema. The major types of urticaria include allergic, physical and idiopathic forms. In allergic urticaria, IgE-mediated degranulation of mast cells results in weals of short duration which typically respond well to antihistamines. Physical urticarias are induced by physical insults including pressure, scratch, col
