Unsupervised Approach for the Identification of the Predominant Site of Upper Airway Collapse in Obstructive Sleep Apnoea Patients Using Snore Signals.

Knowledge regarding the site-of-collapse in the upper airway in obstructive sleep apnoea (OSA) has implications for treatment options and their outcomes. However, current methods to identify the site-of-collapse are not suitable for clinical practice due to the invasive nature, the time/cost of the tests and the inconsistency of the obstruction site identified with natural and drug-induced sleep. In this study, we adopted an unsupervised algorithm to identify the predominant site-of-collapse of the upper airway during natural sleep using nocturnal audio recordings. Nocturnal audio was recorded

Sleep apnea patients with epiglottic collapse elevate their larynx more with swallowing; videofluoroscopic swallowing study of 80 patients.

OBJECTIVE: The epiglottis plays an integral role in the swallowing mechanism and is also implicated as an obstruction site in obstructive sleep apnea (OSA). The underlying causes of epiglottic collapse during sleep remain unclear. This study aimed to investigate the cognitive functions using the Loewenstein Occupational Therapy Cognitive Assessment (LOTCA) and the neurophysiological and anatomical factors using videofluoroscopic swallowing studies (VFSS). We compared patients with OSA exhibiting epiglottic collapse to those without, assessing differences in anatomical or neurophysiological cha

Surgical treatment of pulmonary tuberculosis; a decade of change.

To observe trends in the surgical therapy of pulmonary tuberculosis, the records of patients treated during the last ten years at the Tuberculosis Division of the San Diego County General Hospital (Vauclain Home) were reviewed. In this decade, a chemotherapeutic revolution permitted more patients to be treated with fewer beds, lower mortality and shorter hospitalization. Pneumoperitoneum has replaced other forms of temporary collapse. Pneumothorax, phrenic nerve interruption and pneumonolysis have been abandoned in favor of extraperiosteal plombage, particularly in older, poor risk patients. T

Surgical management of the hypopharyngeal airway in sleep disordered breathing.

The etiology of sleep disordered breathing is collapse or obstruction of the upper airway during sleep. This obstruction may be localized to one or two areas or may encompass the entire upper airway passages to include the nasal cavity, nasopharynx, oropharynx, hypopharynx, and larynx. The presurgical evaluation, which includes polysomnography, a comprehensive head and neck physical examination, fiberoptic nasopharyngoscopy, and lateral cephalometric analysis is essential in directing surgical therapy in a site specific approach. The surgical procedures available to address hypopharyngeal and

Evidence-Based Management and Functional Outcomes of Femoral Head Avascular Necrosis: A Systematic Review.

Avascular necrosis of the femoral head (AVNFH), also known as osteonecrosis of the femoral head, is a progressive musculoskeletal disorder caused by compromised blood supply to the femoral head, causing osteocyte death, structural collapse, pain, and functional disability. Despite advances in diagnostic imaging and treatment modalities, optimal management remains challenging because of multifactorial etiology and variable clinical presentation of the disease. To systematically evaluate the etiopathogenesis, evidence-based management strategies, and functional outcomes associated with AVNFH. A

Myocardial depression induced by severe sepsis: successful rescue using extracorporeal cardiopulmonary resuscitation from initial phase of severe sepsis.

A 60-year-old man was diagnosed with severe sepsis caused by pyelonephritis. During transfer to the hospital room, he suddenly developed ventricular fibrillation and the patient recovered after electrical defibrillation. After this cardiac event, his haemodynamics collapsed despite administration of crystalloid fluid. Transthoracic echocardiography was immediately performed showing the oedema and reduced left ventricular wall motion. Since the haemodynamic collapse was too severe to maintain with conventional septic shock therapy, we introduced extracorporeal cardiopulmonary resuscitation, bri

Iatrogenic lesions of the upper airway in the newborn.

A retrospective study of 140 babies who survived the first week of life and who had endotracheal intubation showed that 65 of them had either or both of the following complications of endotracheal tube therapy: (a) Collapse of one or more lobes with the tube in place or within the first two days following extubation and lasting less than 24 hours. When collapse lasted more than 48 hours, it often required repeated intubation for tracheal cleansing. The right lung, especially its upper lobe, was predominantly affected. (b) Tracheal lesions and nasal necrosis. A statistical comparison with intub

Exercise-associated collapse: an evidence-based review and primer for clinicians.

Exercise-associated collapse (EAC) commonly occurs after the completion of endurance running events. EAC is a collapse in conscious athletes who are unable to stand or walk unaided as a result of light headedness, faintness and dizziness or syncope causing a collapse that occurs after completion of an exertional event. Although EAC is perhaps the most common aetiology confronted by the medical provider attending to collapsed athletes in a finish-line tent, providers must first maintain vigilance for other potential life-threatening aetiologies that cause collapse, such as cardiac arrest, exert

Relapsing polychondritis.

Relapsing polychondritis (RP) is a rare disease causing inflammation and destruction of cartilage and other connective tissues. Specific laboratory aberrations are lacking. Predominant clinical manifestations include auricular chondritis, polyarthritis, nasal chondritis, ocular inflammation, audiovestibular damage, and respiratory tract chondritis. A relapsing course is characteristic. Airways are involved in 50% of patients and may cause dyspnea, stridor, wheezing, hoarseness, aphonia, and laryneal or tracheal tenderness. Airflow obstruction may result from RP involving the tracheobronchial t

Unmasking Upper Airway Obstruction: A Case of Tracheobronchial Amyloidosis.

We present a rare case of a 76-year-old woman with severe tracheobronchial amyloidosis (TBA) causing airway collapse, in which the predominant underlying mechanism was expiratory central airway collapse (ECAC) secondary to both TBA and severe obstructive sleep apnoea (OSA). CPAP therapy effectively treated severe OSA, which likely contributed to a reduction in ECAC severity and alleviation of breathlessness. This case suggests that positive airway pressure ventilation may represent a valuable adjunctive treatment in patients with TBA exhibiting features of OSA, particularly when endobronchial

No air leak on PPV does not exclude tracheobronchial injury after blunt chest trauma.

Tracheobronchial injuries are commonly associated with persistent air leak with pneumothoraces especially when on positive pressure ventilation (PPV). Injuries with absence of these features together with collapse of the lung and consequent low arterial oxygen tension while on PPV are less well recognised. We present a patient with traumatic aortic dissection and preoperatively undiagnosed complete transaction of the left main bronchus following blunt chest trauma. He had no persistent air leak with lower lung lobe collapse despite undergoing PPV and had low arterial oxygen tension which faile

[Pulmonary re-expansion edema. Description of two cases and observations about its management].

Two cases of Reexpansion pulmonary edema (RPE), an uncommon complication of the treatment of chronic lung collapse secondary to pneumothorax or pleural effusion, are described. RPE is generally unilateral and occurs when the lung is rapidly reexpanded by active evacuation of large amounts of air or fluid. Nevertheless, both cases observed confirm that RPE can be seen when the pulmonary collapse is of short duration and the lung is reexpanded without suction. The pathogenesis of RPE is still unclear and is probably multifactorial. Implicated in the etiological process of RPE are chronicity of c

Lower airway collapse: Revisiting the definition and clinicopathologic features of canine bronchomalacia.

Bronchomalacia (BM) is an inconsistently defined term in dogs, impairing understanding of clinical presentation, therapeutic response and prognosis. Herein the authors propose to clarify the definition of canine bronchomalacia (CBM) as regional to diffuse dynamic airway collapse of segmental and/or subsegmental bronchi with associated clinical signs due to airflow limitation. In contrast to tracheal collapse, mainstem bronchial collapse, and in some cases lobar collapse, CBM requires advanced imaging. Common co-morbid conditions (e.g., chronic bronchitis, mitral valve degenerative disease, etc

Collapsing glomerulopathy in 19 patients with systemic lupus erythematosus or lupus-like disease.

BACKGROUND AND OBJECTIVES: Collapsing glomerulopathy is a podocytopathy with segmental or global wrinkling and collapse of capillary walls and overlying epithelial cell proliferation. Idiopathic collapsing glomerulopathy is a distinct clinicopathologic entity with significant proteinuria, poor response to immunosuppressive therapy, and rapid progression to renal failure. Collapsing glomerulopathy is associated with viral infections, autoimmune disease, and drugs. This work presents the largest group of collapsing glomerulopathy in patients with SLE. Tubulointerstitial nephritis in active tuberculosis - a single center experience.

BACKGROUND: Treatment for osteonecrosis of the femoral head (ONFH) remains controversial. Current reviews include low-level evidence studies evaluating the treatment of both pre-collapse and collapse stages of the disease. QUESTIONS/PURPOSES: The purpose of the current study is to systematically review the literature evaluating core decompression (CD) with bone marrow mesenchymal cells (BMMCs), CD alone, and bisphosphonate treatment in pre-collapse ONFH by focusing just on randomized clinical trials (RCTs) reporting functional and radiologic outcomes. We aim to determine if the literature pro

Home sleep study nasal airflow patterns and epiglottic collapse during drug induced sleep endoscopy.

PURPOSE: Obstructive sleep apnea (OSA) is characterized by collapse of various portions of the pharynx. Epiglottic collapse is difficult to diagnose and may affect treatment tolerance. Previous research shows a distinct nasal airflow pattern during epiglottic obstruction characterized by signal discontinuities. It was hypothesized that primary epiglottic collapse characterized during drug-induced sleep endoscopy (DISE) correlates with nasal airflow discontinuity signals seen on home sleep studies. METHODS: Patients with clinical records of undergoing both DISE and home portable sleep studies

Using Computed Tomography-Based Three-dimensional Modeling and Computer Navigation for Minimally Invasive Core Decompression and Adjuvant Orthobiologic Therapy of Femoral Head Avascular Necrosis.

Avascular necrosis of the femoral head is a debilitating condition that can lead to femoral head collapse. Core decompression with adjuvant cellular therapies, such as bone marrow aspirate concentrate, delays disease progression and improves outcomes. However, inconsistent results in the literature may be due to limitations in surgical technique and difficulty in targeting the necrotic lesions. Here, we present a surgical technique utilizing computed tomography-based three-dimensional modeling and instrument tracking to guide the therapy to the center of the lesion. This method minimizes the n
