Laboratory diagnosis of community-acquired lower respiratory tract infection.

This article has focused on the evaluation of outpatients with lower respiratory illness. In large part, the need for microbiological work-up is host-dependent. Healthy patients usually do well, and laboratory data are often unnecessary. The abnormal host requires a different approach and, in general, the more compromised the host, the more aggressive the laboratory evaluation. A renal transplant patient with respiratory symptoms often follows the dictum that "common things happen commonly;" however, the clinician needs that extra level of assurance in this case. Some transplant patients may h

Proteomic, metabolomic and lipidomic profiles in community acquired pneumonia for differentiating viral and bacterial infections.

Community-acquired pneumonia (CAP) has a significant impact on public health, especially in light of the recent SARS-CoV-2 pandemic. To enhance disease characterization and improve understanding of the underlying mechanisms, a comprehensive analysis of the plasma lipidome, metabolome and proteome was conducted in patients with viral and bacterial CAP infections, including those induced by SARS-CoV-2. Lipidomic, metabolomic and proteomic profiling were conducted on plasma samples of 69 patients suffering either from viral or bacterial CAP. Lipid and metabolite analyses were LC-MS-based, while p

Co-infections with Multiple Viruses: A Frequent cause of Community-Acquired Pneumonia in Sarawak Malaysia.

OBJECTIVES: Equatorial Sarawak, Malaysia, has been the site of important novel respiratory virus detections. During the COVID-19 pandemic, we sought to determine viral causes of pneumonia that were not SARS-CoV-2. METHODS: Using an informed consent process, we enrolled patients from four hospitals in Sarawak for this cross-sectional study. Patients permitted a nasopharyngeal (NP) swab collection and completed a risk factor questionnaire. We studied NP swabs with molecular diagnostics for previously recognized respiratory viruses such as influenza A and D viruses, and pan-species assays for ad

Exploring multidrug resistance patterns in community-acquired Escherichia coli urinary tract infections with machine learning.

While associations of antibiotic resistance traits are not random in multidrug-resistant (MDR) bacteria, clinically relevant resistance patterns remain underexplored. This study used association-set mining to explore resistance associations within E. coli isolates from community-acquired urinary tract infection isolates collected from 2018 to 2022 by France's national surveillance system. Association-set mining was applied separately to extended-spectrum beta-lactamase-producing E. coli (ESBL-EC) and non-ESBL-EC. MDR patterns with expected support (reflecting pattern frequency) and conditional

Current Challenges in the Management of Skin and Soft Tissue Infections and Community-Acquired Pneumonia.

Skin and soft tissue infections (SSTI) and community-acquired pneumonia (CAP) are major public health problems that are commonly encountered in the primary care setting. Establishing the severity of disease is an important step in the diagnosis of SSTI and CAP, because this can affect decisions about optimal management, including level of care. Due to antibiotic resistance, allergies, and adverse effect profiles of current therapies, there is a need for new treatment options for both SSTI and CAP. Improved utilization of oral outpatient antibiotic treatments can also minimize the risk of serio

Prevalence, etiology and antibiotic resistance patterns of community-acquired urinary tract infections in Dhaka, Bangladesh.

Urinary tract infection (UTI) accounts for a significant morbidity and mortality across the world and is a leading cause for antibiotic prescriptions in the community especially in developing countries. Empirical choice of antibiotics for treatment of UTI is often discordant with the drug susceptibility of the etiologic agent. This study aimed to estimate the prevalence of community-acquired UTI caused by antibiotic resistant organisms. This was a cross-sectional study where urine samples were prospectively collected from 4,500 patients at the icddr,b diagnostic clinic in Dhaka, Bangladesh dur

A prospective observational study of community-acquired bacterial bloodstream infections in Metro Manila, the Philippines.

Community-acquired bacterial bloodstream infections are caused by diverse pathogens with changing antimicrobial-resistance patterns. In low-middle income countries in Southeast Asia, where dengue fever is endemic and a leading cause of fever, limited information is available about bacterial bloodstream infections due to challenges of implementing a blood culture service. This study describes bacterial bloodstream pathogens and antimicrobial-resistance patterns in Metro Manila, the Philippines. We aimed to identify the proportion of patients with a positive blood culture, the bacteria isolated

Parainfluenza Virus Types 1-3 Infections Among Children and Adults Hospitalized With Community-acquired Pneumonia.

BACKGROUND: Parainfluenza virus (PIV) is a leading cause of lower respiratory tract infections. Although there are several distinct PIV serotypes, few studies have compared the clinical characteristics and severity of infection among the individual PIV serotypes and between PIV and other pathogens in patients with community-acquired pneumonia. METHODS: We conducted active population-based surveillance for radiographically confirmed community-acquired pneumonia hospitalizations among children and adults in 8 US hospitals with systematic collection of clinical data and respiratory, blood, and s

Cefazolin versus fluoroquinolones for the treatment of community-acquired urinary tract infections in hospitalized patients.

Literature for the treatment of hospitalized patients with community-acquired urinary tract infections (UTI) is limited. Previous outpatient studies do not support the use of oral beta-lactams compared with oral fluoroquinolones (FQ) due to poor clinical cure rates. However, recent studies evaluating intravenous (IV) beta-lactams in more complicated cases demonstrate promising cure rates. In addition, the use of more narrow-spectrum beta-lactams may be preferable when possible, due to a lower incidence of "collateral damage" compared with FQ. This was a retrospective, non-inferiority, single-c

Predictive factors for Enterococcus faecalis in complicated community-acquired urinary tract infections in older patients.

AIM: Risk factors for complicated community acquired Enterococcus faecalis urinary tract infection (UTI) in older patients are not well known. METHODS: We identified the predictive factors for E. faecalis on a cohort of 659 older patients admitted to hospital with complicated UTI. We also examined the adequacy of empirical antimicrobial therapy and outcomes in E. faecalis UTI. Multivariable logistic regression was used to identify predictors of E. faecalis UTI. RESULTS: A total of 87 (13.2%) patients had E. faecalis UTI; of these 63.2% were men, their mean age was 82.3 years and they had a g

Community-acquired respiratory infections are common in patients with non-Hodgkin lymphoma and multiple myeloma.

PurposeAvailable data suggest that respiratory infections are associated with increased morbidity and mortality in patients hospitalized due to acute leukemia and allogeneic stem cell transplantation (allo-SCT). However, the precise incidence, risk factors, and severity of respiratory infection, mainly community-acquired, in patients with lymphoma and multiple myeloma (MM) are not fully determined. The current study aimed to investigate risk factors for respiratory infections and their clinical significance in patients with B cell non-Hodgkin lymphoma (NHL) and multiple myeloma (MM) in the fir

Association between prior antibiotic therapy and subsequent risk of community-acquired infections: a systematic review.

BACKGROUND: Antibiotic use can have negative unintended consequences including disruption of the human microbiota, which is thought to protect against pathogen overgrowth. We conducted a systematic review to assess whether there is an association between exposure to antibiotics and subsequent risk of community-acquired infections. METHODS: We searched MEDLINE, EMBASE and Web of Science for studies published before 30 June 2017, examining the association between antibiotic use and subsequent community-acquired infection. Infections caused by Clostridium difficile and fungal organisms were excl

Antimicrobial Resistance Among Uropathogens That Cause Childhood Community-acquired Urinary Tract Infections in Central Israel.

In this retrospective study 829 positive urine cultures were analyzed. Escherichia coli bacterium was the leading uropathogen (86%). Almost 60% were resistant to ampicillin and first generation cephalosporins, and about 30% of them resistant to amoxicillin-clavulanic acid and trimethoprim-sulfamethoxazole. Almost none of them were resistant to second and third generation cephalosporins, aminoglycosides, ciprofloxacin or nitrofurantoin.

Staphylococcal colonization of caretakers of children who have community-acquired skin and soft tissue infections.

PURPOSE: Colonization with Staphylococcus aureus is considered a risk factor for the rising incidence of pediatric community-acquired skin and soft tissue infections (CA-SSTIs), and intrafamily spread is thought to be the source of colonization. METHODS: A prospective study was conducted to determine skin and nasal staphylococcal colonization rates among the caretakers of CA-SSTI patients and those of nonabscess controls. A questionnaire regarding risk factors was administered to all participants. Fisher's Exact test and the chi(2) test were used for statistical analysis. RESULTS: Forty-six
