Chronic Electrical Stimulation of the Otolith Organ: Preliminary Results in Humans with Bilateral Vestibulopathy and Sensorineural Hearing Loss.

INTRODUCTION: Bilateral vestibulopathy is an important cause of imbalance that is misdiagnosed. The clinical management of patients with bilateral vestibular loss remains difficult as there is no clear evidence for an effective treatment. In this paper, we try to analyze the effect of chronic electrical stimulation and adaptation to electrical stimulation of the vestibular system in humans when stimulating the otolith organ with a constant pulse train to mitigate imbalance due to bilateral vestibular dysfunction (BVD). METHODS: We included 2 patients in our study with BVD according to Criteri

Impact sites representing potential bruising locations associated with rearward falls in children.

Children presenting multiple unexplained bruises can be an early sign of physical abuse. Bruising locations on the body can be an effective indicator of abusive versus accidental trauma. Additionally, childhood falls are often used as falsely reported events in child abuse, however, characterization of potential bruising locations associated with these falls does not exist. In our study we used a 12-month old pediatric anthropomorphic test device (ATD) adapted with a custom developed force sensing skin to predict potential bruising locations during rearward falls from standing. The surrogate b

Content Validity of the K-CAT® Assessing Mental Health Challenges in Autism: A Mixed Methods Analysis of Perspectives from Autistic Youth, Caregivers, and Clinicians.

PURPOSE: Mental health (MH) challenges in autistic youth are often under-identified due in part to limited availability of assessment tools developed or adapted for the autism population. The K-CAT®, a battery of computerized adaptive tests assessing up to nine MH domains in the general population, shows promise in addressing this need. Because these general instruments may not be valid or acceptable for use with autistic individuals, we evaluated the K-CAT®'s content validity for autistic youth with input from the autism community. METHODS: Feedback was obtained from autistic youth, caregive

Intellectual impairment in neurofibromatosis 1.

Intellectual problems are a recognized feature of neurofibromatosis 1 (NF1) but their aetiology is unknown. We investigated the frequency, nature, severity and cause of intellectual impairment in NF1. We undertook neurological and psychometric assessments on 103 patients with NF1 and 105 controls equated for age, sex and socio-economic status. The mean full scale IQ was significantly lower in the NF1 than the control group, 88.6 (SD 14.6) compared with 101.6 (SD 14.2). However, the degree of intellectual impairment was mild and only 8% of NF1 patients had an IQ < 70. The NF1 patients also had

Simplified follow-up after medical abortion using a low-sensitivity urinary pregnancy test and a pictorial instruction sheet in Rajasthan, India--study protocol and intervention adaptation of a randomised control trial.

BACKGROUND: The World Health Organisation suggests that simplification of the medical abortion regime will contribute to an increased acceptability of medical abortion, among women as well as providers. It is expected that a home-based follow-up after a medical abortion will increase the willingness to opt for medical abortion as well as decrease the workload and service costs in the clinic. METHODS/DESIGN: This study protocol describes a study that is a randomised, controlled, non-superiority trial. Women screened to participate in the study are those with unwanted pregnancies and gestationa

Exploring the psychogastroenterology patterns of functional dyspepsia and irritable bowel syndrome comorbidities based on latent profile analysis.

BACKGROUND: Comorbidities between different subtypes of functional gastrointestinal disorders (FGIDs) are highly prevalent in clinical practice. The heterogeneity of clinical appearance of these comorbidities causes difficulties with individualized diagnosis and comprehensive treatment of FGIDs. In this study, we aimed to estimate and interpret the psychology-gastroenterology interaction patterns of FGIDs for heterogeneous clinical appearances against the traditional anatomic diagnosis criteria. METHODS: In a retrospective cross-sectional study, the intensities of clinical appearances coverin

Minimal Clinically Important Differences for PROMIS Physical Function, Upper Extremity, and Pain Interference in Carpal Tunnel Release Using Region- and Condition-Specific PROM Tools.

PURPOSE: Uncertainty exists about what change in Patient-Reported Outcomes Measurement Information System (PROMIS) scores represents a clinically relevant improvement (minimal clinically important difference [MCID]) in hand surgery care. Using a region-specific patient-reported outcome measure (PROM) (Michigan Hand Question [MHQ]) and a condition-specific PROM (Boston Carpal Tunnel Questionnaire [BCTQ]), MCID values were determined for PROMIS Physical Function (PF), Upper Extremity (UE), and Pain Interference (PI) computerized adaptive testing among patients undergoing carpal tunnel release (C

Computerized Tailored Interventions to Enhance Prevention and Screening for Hepatitis C Virus Among People Who Inject Drugs: Protocol for a Randomized Pilot Study.

BACKGROUND: Hepatitis C virus (HCV) infection is a growing problem among people who inject drugs. Strategies to reduce disease transmission (eg, syringe exchange programs) and facilitate HCV screening and linkage are available but are under-utilized in many communities affected by injection drug use. Novel approaches to increasing the use of these strategies are needed. OBJECTIVE: The goals of this project are to (1) develop and pilot test a computerized tailored intervention for increasing HCV screening and decreasing risky drug use behavior among people who inject drugs and (2) determine th

Classification and Regression Tree and Computer Adaptive Testing in Cardiac Rehabilitation: Instrument Validation Study.

BACKGROUND: There is a need for shorter-length assessments that capture patient questionnaire data while attaining high data quality without an undue response burden on patients. Computerized adaptive testing (CAT) and classification and regression tree (CART) methods have the potential to meet these needs and can offer attractive options to shorten questionnaire lengths. OBJECTIVE: The objective of this study was to test whether CAT or CART was best suited to reduce the number of questionnaire items in multiple domains (eg, anxiety, depression, quality of life, and social support) used for a

Comparison of one-sample two-sided sequential t-tests for application in epidemiological studies.

In epidemiological prospective cohort studies, exposure levels of cases with disease and disease-free control subjects can be measured by laboratory analysis of previously stored biological specimens. In such studies, a sequential t-test can be used for preliminary evaluations, at the expense of the smallest possible number of specimens, of whether a new aetiological hypothesis is worth further investigation or whether specimens should rather be spared to test other, more fruitful, hypotheses. For this purpose, we recently compared two sequential probability ratio tests (SPRTs), in which the l

Factors Associated With a Discretionary Upper-Extremity Surgery.

PURPOSE: Surgery for nontraumatic upper-extremity problems is largely discretionary and preference-sensitive. Psychological and social determinants of health correlate with greater symptoms and limitations and might be associated with discretionary operative treatment. METHODS: We used routinely collected patient-reported outcome measures from patients with de Quervain tendinopathy, ganglion cyst, trapeziometacarpal arthritis, trigger digit, and carpal tunnel syndrome to study factors associated with discretionary surgery using multiple logistic regression. Patients completed a measure of the

Increased parietal activity after training of interference control.

Recent studies suggest that computerized cognitive training leads to improved performance in related but untrained tasks (i.e. transfer effects). However, most study designs prevent disentangling which of the task components are necessary for transfer. In the current study, we examined whether training on two variants of the adaptive dual n-back task would affect untrained task performance and the corresponding electrophysiological event-related potentials (ERPs). Forty three healthy young adults were trained for three weeks with a high or low interference training variant of the dual n-back t

Methylphenidate and galantamine in patients with vascular cognitive impairment-the proof-of-principle study STREAM-VCI.

BACKGROUND: To date, no symptomatic treatment is available for patients with vascular cognitive impairment (VCI). In the proof-of-principle study Symptomatic Treatment of Vascular Cognitive Impairment (STREAM-VCI), we investigated whether a single dose of a monoaminergic drug (methylphenidate) improves executive functioning and whether a single dose of a cholinergic drug (galantamine) improves memory in VCI patients. METHODS: STREAM-VCI is a single-center, double-blind, three-way crossover trial. We included 30 VCI patients (Mini-Mental State Examination (MMSE) ≥ 16 and Clinical Dementia Rati

Method for Measuring Mucociliary Clearance and Cilia-generated Flow in Mice by ex vivo Imaging.

Ex vivo biophysical measurements provide valuable insights into understanding both physiological and pathogenic processes. One critical physiological mechanism that is regulated by these biophysical properties is cilia-generated flow that mediates mucociliary clearance, which is known to provide protection against foreign particles and pathogens in the upper airway. To measure ciliary clearance, several techniques have been implemented, including the use of radiolabeled particles and imaging with single-photon emission computerized tomography (SPECT) methods. Although non-invasive, these tests

Automated identification of abnormal metaphase chromosome cells for the detection of chronic myeloid leukemia using microscopic images.

Karyotyping is an important process to classify chromosomes into standard classes and the results are routinely used by the clinicians to diagnose cancers and genetic diseases. However, visual karyotyping using microscopic images is time-consuming and tedious, which reduces the diagnostic efficiency and accuracy. Although many efforts have been made to develop computerized schemes for automated karyotyping, no schemes can get be performed without substantial human intervention. Instead of developing a method to classify all chromosome classes, we develop an automatic scheme to detect abnormal

The Long and Winding Road to Real-Life Experiments: Remote Assessment of Executive Functions with Computerized Games-Results from 8 Years of Naturalistic Interventions.

Mate Marote is an open-access cognitive training software aimed at children between 4 and 8 years old. It consists of a set of computerized games specifically tailored to train and evaluate Executive Functions (EF), a class of processes critical for purposeful, goal-directed behavior, including working memory, planning, flexibility, and inhibitory control. Since 2008, several studies were performed with this software at children's own schools in interventions supervised in-person by cognitive scientists. After 2015, we incorporated naturalistic, yet controlled, interventions with children's ow
