Residual sleepiness in sleep apnea patients treated by continuous positive airway pressure.

Hypoxic brain damage might explain persistent sleepiness in some continuous positive airway pressure-compliant obstructive sleep apnea called residual excessive sleepiness. Although continuous positive airway pressure may not be fully efficient in treating this symptom, wake-promoting drug prescription in residual excessive sleepiness is no longer allowed by the European Medicines Agency. The aim of this study is to describe residual excessive sleepiness phenotypes in a large prospective sample of patients with obstructive sleep apnea. Residual excessive sleepiness was defined by an Epworth Sl

Effects of nasal continuous positive-airway pressure on nutritive swallowing in lambs.

Current knowledge suggests that, to be successful, oral feeding in preterm infants should be initiated as soon as possible, often at an age where immature respiration still requires ventilatory support in the form of nasal continuous positive airway pressure (nCPAP). While some neonatologist teams claim great success with initiation of oral feeding in immature infants with nCPAP, others strictly wait for this ventilatory support to be no longer necessary before any attempt at oral feeding, fearing laryngeal penetration and tracheal aspiration. Therefore, the aim of the present study was to pro

A solution to the Jones tube continuous positive airway pressure (CPAP) dilemma?

A 49-year-old woman, who had previously undergone bilateral Jones tube placement, began nasal continuous positive airway pressure for obstructive sleep apnea. The patient's use of continuous positive airway pressure was limited by intolerance of the transfer of air through the Jones tube to her ocular surface resulting in irritation and discomfort. A change from nasal continuous positive airway pressure to a full face mask, including both Jones tubes in the pressure circuit, resolved the problem.

Aerosol delivery and humidification with the Boussignac continuous positive airway pressure device.

BACKGROUND: A simple method for effective bronchodilator aerosol delivery while administering continuing continuous positive airway pressure (CPAP) would be useful in patients with severe bronchial obstruction. OBJECTIVE: To assess the effectiveness of bronchodilator aerosol delivery during CPAP generated by the Boussignac CPAP system and its optimal humidification system. METHODS: First we assessed the relationship between flow and pressure generated in the mask with the Boussignac CPAP system. Next we measured the inspired-gas humidity during CPAP, with several humidification strategies, i

Early nasal continuous positive airway pressure and necrotizing enterocolitis in preterm infants.

BACKGROUND: The use of early nasal continuous positive airway pressure (ENCPAP) as the mode of initial respiratory support for very low birth weight (VLBW) infants has been increasing. The impact of CPAP and oxygen on gut mucosa and perfusion in premature infants is not known. The relation between ENCPAP and necrotizing enterocolitis (NEC) has not been adequately addressed. OBJECTIVE: To evaluate if the use of an individualized respiratory management strategy encouraging the use of ENCPAP is associated with an increased risk of NEC, and to determine risk factors for NEC in premature infants s

An empirical continuous positive airway pressure trial for suspected obstructive sleep apnea.

BACKGROUND: Standard practice in obstructive sleep apnea (OSA) management requires that a positive diagnostic, overnight polysomnography (PSG) test be obtained before initiating treatment. However, long waiting times due to lack of access to PSG testing facilities may delay the initiation of definitive treatment for OSA. OBJECTIVES: To evaluate the response of patients who had a high clinical suspicion for OSA and who were waiting for a PSG test to an empirical continuous positive airway pressure (CPAP) trial. METHODS: A retrospective study of all patients who had been offered empirical CPAP

[Long-term continuous positive airway pressure (CPAP) use in obstructive sleep apnea].

BACKGROUND: The use of CPAP is the conventional therapy for the Obstructive Sleep Apnea Syndrome (OSAS). AIM: To establish the proportion of OSAS patients with CPAP indication who use it in the long-term and to determine predictive factors of adherence. PATIENTS AND METHODS: Structured survey through personal interview or phone call, applied to all patients who had polisomnographically documented OSAS and CPAP titration at the Pontificia Universidad Católica de Chile Sleep Disorders Center between January 1998 and November 2001. Patients were divided in "Permanent Users" (Group I) and "Non U

Pulmonary interstitial emphysema and continuous positive airway pressure in a premature infant.

Pulmonary interstitial emphysema PIE is a recognized sequel of high pressure mechanical ventilation. Nevertheless, with the improvement in perinatal medical care, PIE started to be seen in spontaneously breathing infants. We present a 29-week-old girl who developed PIE on the first day of life, while she was on nasal continuous positive airway pressure, and was successfully managed with high frequency oscillatory ventilation. There is a paucity of these cases in the literature. We report on the occurrence of this entity, early reorganization, and the possible associated or causation factors.

Survival of veterans with sleep apnea: continuous positive airway pressure versus surgery.

OBJECTIVES: Continuous positive airway pressure (CPAP) improves sleep apnea survival. We tested whether CPAP is associated with better survival than uvulopalatopharyngoplasty (UPPP). STUDY DESIGN AND METHODS: This retrospective cohort database study included all sleep apnea patients treated with CPAP or UPPP in Veteran Affairs facilities from October 1997 through September 2001. Treatment groups were compared with Cox regression, adjusting for age, gender, race, year treatment was initiated, and comorbidity. Sleep apnea severity and CPAP use data were not available. RESULTS: By September 200

Blastomycosis-induced respiratory failure: the successful application of continuous positive airway pressure.

A patient diagnosed with pulmonary blastomycosis was seen initially by her primary care physician for severe pain in her right elbow and was treated with steroid therapy and analgesics. She also had been treated for chronic cough and congestion for approximately 2 months before admission. She showed no improvement at home. She was hospitalized for right epicondylitis, and progressively worsening restrictive pulmonary symptoms developed, including hypoxemia and dyspnea. She was transferred to the intensive care unit and treated with mask continuous positive airway pressure (CPAP) and intravenou

Continuous positive airway pressure as a promoter of laryngospasm during halothane anesthesia.

Twenty mongrel dogs were anesthetized with halothane 2.0%, 1.25%, 0.94%, and 0.63% in oxygen. Thyroarytenoid (TA) and posterior cricoarytenoid (PCA) electromyography (EMG) tracings were recorded with the animal at rest, following mechanical irritation of the glottis, and during 20 mm Hg continuous positive airway pressure (CPAP) following either airway occlusion or hyperventilation. Adductor laryngospasm was defined as continuous tonic TA EMG activity, silent PCA EMG, and vocal cord adduction. Abductor laryngospasm was defined as continuous tonic PCA EMG activity, silent TA EMG, and vocal cord

Treatment of the adult respiratory distress syndrome with continuous positive airway pressure.

Fifteen patients in the early stages of the adult respiratory distress syndrome with severe hypoxemia who were capable of maintaining adequate spontaneous ventilation were treated with continuous positive airway pressure (CPAP). The optimal level of CPAP was adjusted for each patient to achieve the highest oxygenation with the least adverse hemodynamic effects. The optimal intravascular volume, judged by pulmonary arterial occlusion pressure, was maintained by infusion of lactated Ringer's solution. Application of an optimal CPAP ranging between 10 and 25 cm H2O significantly reduced the intra

Spontaneous breathing with continuous positive airway pressure in adult respiratory distress syndrome.

The authors describe three systems of spontaneous respiration with positive end expiratory pressure (P.E.E.P.) : the system of continuous positive airway pressure (C.P.A.P.), the bubble and the intermittent mandatory ventilation with C.P.A.P. (I.M.V.-C.P.A.P.) by Servo-Ventilator 900 B. The results are conclusive with the C.P.A.P. unit used for post-traumatic adult patients in acute respiratory distress and for patients who have been anesthetized for more than three hours. The PaO2 significantly increase in subjects whose PaO2 in the ambient air without P.E.E.P. is lower than 70 mm Hg. The bub

Continuous positive airway pressure and mechanical ventilation by facemask in newborn infants.

During a nine-month period 24 newborn infants were treated with continuous positive airway pressure (CPAP) or mechanical ventilation delivered through a facemask. The mask was held in place in a way that minimised trauma and distortion of the head. The median birth weight of the infants was 1096 g and their median gestational age 29 weeks. The usual reason for treatment was hyaline membrane disease or recurrent apnoea due to inadequate control of breathing. Twenty-one of the infants survived. The technique was simple to apply and complications were minimal. We suggest that it may have advantag

Structured continuous positive airway pressure weaning standardizes discontinuation and reduces instability events.

BACKGROUND/OBJECTIVES: Continuous positive airway pressure (CPAP) is a cornerstone of neonatal respiratory support, yet weaning practices remain highly variable and largely experience-based. This study evaluated whether implementing a structured, stage-based CPAP-weaning protocol could standardize clinical decision-making and reduce exposure to poorly tolerated CPAP-off periods, using safety-relevant instability markers. METHODS: In this single-centre interventional study with a historical control group, all neonates reaching ≥30 + 0 weeks corrected gestational age who received CPAP between M

Continuous positive airway pressure-treated patients' behaviours during the COVID-19 crisis.

The #COVID19 pandemic has had limited impact on CPAP use by obstructive sleep apnoea patients. Mainly only those with suspected or proven COVID-19 stopped CPAP use, sometimes without a physician's advice, and were more likely to move to a separate bedroom. https://bit.ly/2RxQDZO.
