[Pharmacology of oral contraceptives].

Oral contraceptives include two types of steroids; ethinyl-estradiol as the main estrogenic component which dose vary from 20 to 50 micrograms per tablet (mostly 30 to 35 micrograms) and progestins essentially derivatives of 19 nortestosterone. Derivatives of 19 norprogesterone such as nomegestrol acetate or ST 1435 are not used as oral contraceptives but are being evaluated through parenteral administration, e.g. implants or transdermal systems. The assessment of the pharmacological properties of these progestins indicate a high antigonadotropic and a high antiestrogenic properties for levono

Oral contraceptive use and prevalence of infection with Chlamydia trachomatis in women.

One thousand eight non-pregnant women aged 16-34 years, presenting for the first time at a clinic for sexually transmitted diseases (STD), were examined and screened for infection with Chlamydia trachomatis, Neisseria gonorrhoeae, Trichomonas vaginalis, and Candida species. The respective prevalence rates were 21.1%, 20.7%, 13.4%, and 27.8%. Isolation rates for C trachomatis, either occurring alone or in association with other genital infections, were significantly greater in women using oral contraceptive agents. This was not because oral contraceptive users were more promiscuous. The finding

Considerations for Menstrual Suppression in Patients With Hematologic Malignancies.

Patients who menstruate during treatment for hematologic malignancies have a higher risk of heavy vaginal bleeding due to thrombocytopenia caused by malignancy and myelosuppressive chemotherapy. Heavy menstrual bleeding is associated with significant morbidity in this patient population, and menstrual suppression is a standard of supportive care. Options for prophylactic menstrual suppression include gonadotropin-releasing hormone agonists, progestin-only therapy, and combined hormonal contraception. Various factors impact the choice of menstrual suppression agent, including efficacy, presence

Recent advancements and future applications of intrauterine drug delivery systems.

INTRODUCTION: Intrauterine devices (IUDs) are drug-device combination products that are placed inside the uterus above the endometrium. IUDs contain a therapeutic agent used for contraception and have been shown to have other therapeutic benefits. Over the past century, intrauterine drug delivery systems (IUDDS) have revolutionized women's health, and they hold immense growth potential in the future for new developments in the therapeutic areas beyond contraception. AREAS COVERED: The aim of this review is to offer a comprehensive description of the history, development, and current state of

Contraception use differs across solid organ transplant types among women transplanted in reproductive age.

The number of reproductive-age solid organ transplantation (SOT) recipients is growing, yet comparative data on contraception use across organ types are limited. We performed a retrospective cohort study of women who underwent first-time liver, kidney, heart, or lung transplantation between the ages of 18 and 45 years from 2015 and 2025 using TriNetX (n = 10 155). Contraception uptake within 2 years was assessed by organ, including patterns of initial contraceptive method and subsequent method changes, and clinical outcomes, including osteoporosis, thromboembolism, and graft rejection, were co

Clinical trials in male hormonal contraception.

Research has established the principle of hormonal male contraception based on suppression of gonadotropins and spermatogenesis. All hormonal male contraceptives use testosterone, but only in East Asian men can testosterone alone suppress spermatogenesis to a level compatible with contraceptive protection. In Caucasians, additional agents are required of which progestins are favored. Clinical trials concentrate on testosterone combined with norethisterone, desogestrel, etonogestrel or depot-medroxyprogesterone acetate. The first randomized, placebo-controlled clinical trial performed by the ph

Synthetic androgens for male contraception.

High global rates of unintended pregnancy (>40% of all pregnancies) are relatively stagnant despite a broad array of female contraceptive options. Data suggest many men are enthusiastic to utilize novel, reversible, male contraceptives, particularly those that may be administered orally. Hormonal male contraception utilizes androgens or androgen-progestin combinations to exploit endocrine negative feedback, interrupting the hypothalamic-pituitary-testicular axis to suppress spermatogenesis. Modifications of testosterone allow for tailoring of steroid activity and show promise as single agent h

The influence of maternal exposure history to virus and medicine during pregnancy on congenital heart defects of fetus.

Congenital heart disease (CHD) is the most common birth defect. It is due to dysfunction of the heart and great vessels during embryo development stage, or the channel was not closed after birth. This study focuses on investigating the influence of virus infection and medicine history during pregnancy on the incidence rate of CHD of fetus. We conducted a retrospective birth cohort study of infant born in the maternal and child health hospital of Fanyu district in Guangzhou. Five thousand three hundred eighty one cases with complete medical records, including mothers, fathers, and infants, were

Etonorgestrel concentrations in morbidly obese women following Roux-en-Y gastric bypass surgery: three case reports.

BACKGROUND: Pregnancy should be avoided for 12 to 18 months after Roux-en-Y gastric bypass (RYGB) surgery. The etonorgestrel (ENG)-releasing implant (Implanon®) may represent a safe and effective contraceptive method in morbidly obese women who are candidates for bariatric surgery. In addition, the subcutaneous delivery of steroid is unaffected by malabsorptive surgery. METHODS: Three cases of young women with ENG-releasing implant are reported. The device was inserted 1-2 months prior to RYGB. RESULTS: Their initial weights were 130 to 176 kg, and the mean weight loss was 33.6 kg at 6 month

A randomized community trial of enhanced family planning outreach in Rakai, Uganda.

A randomized community trial of a family planning outreach program was conducted in Rakai District, Uganda. Five communities received standard services; six intervention communities received additional family planning information, counseling, and contraceptive methods from government service providers and community-based volunteer agents using social marketing and other strategies. Condom use was promoted in all of the communities. The community-based family planning outreach program was implemented in two phases--1999-2000 (early) and 2001(late)--and its impact was evaluated by means of popul

Impact of contraception on gynecologic cancers.

In considering the appropriate contraceptive method for a particular woman, the potential effect of that method on her risk of developing cancer of the breast, cervix, endometrium, or ovary is crucial. Among the most closely studied of the risk factors for gynecologic neoplasm has been the potential role of contraceptives, especially oral contraceptives, intrauterine devices, and injectable progestins. Physicians need to consider the potential impact of these agents on the disease process, therapy for the disease, future fertility, and the health of the fetus. Although much of the epidemiologi

Promoting emergency contraception.

The use of available oral contraceptive agents for emergency contraception has been judged safe and effective by the U. S. Food and Drug Administration and other agencies. Yet implementation by physicians has been limited, and only a small percentage of women take advantage of this option each year. Lack of a specially packaged--and marketed--product for this indication has been a major barrier.

[Male contraception-scientific foundations and contemporary progress].

Despite decades of assiduous inquiry and burgeoning societal demand, contraceptive options for men remain circumscribed. Women have benefitted from highly efficacious hormonal modalities, but the burden of contraceptive responsibility continues to rest almost exclusively upon them. In light of the potential sequelae of female hormonal agents and the inviolable right to bodily autonomy, the quest for safe, reversible male methods is assuming urgency.Hormonal contraception for men relies on suppression of spermatogenesis through exogenous testosterone combined with progestins.
