OBJECTIVE: This study aimed to compare the efficacy of topical 5% Sodium Chloride (NaCl) and 10% Mannitol agents in managing corneal edema following uncomplicated cataract surgery.
METHODS: Patients with postoperative corneal edema were enrolled, and they were divided into three groups for comparison. Anterior chamber depth (ACD), axial length (AL), intraocular lens (IOL) power, cumulative dissipated energy (CDE), best-corrected visual acuity (BCVA), central corneal thickness (CCT), and endothelial cell count (ECC) were assessed among the groups.

Penetrating keratoplasties were performed in 100 patients with bullous keratopathy and aphakia. In 85% of cases, edema was due to Fuchs' dystrophy. Cases were divided into eyes with good, fair, and poor prognosis, depending on the presence of glaucoma, anterior segment pathology and corneal vascularization. A 56-year-old Hispanic male individual with moderate primary open-angle glaucoma on maximal tolerated topical therapy with recent visual field progression presented with excessive glare due to cataract in the right eye. The patient underwent uncomplicated phacoemulsification with supraciliary microstent implantation. Ten weeks postoperatively, he presented with global Descemet folds and focal corneal edema overlying the CyPass implant despite apparent adequate implant position (1 retention ring visible). We present a case of retained intraocular lens (IOL) haptic segment in the anterior chamber, diagnosed seven months following a reportedly uneventful cataract surgery due to manifestation of inferior corneal edema. In a prospective study of 89 patients the postoperative corneal edema and endothelial cell count was studied after cataract extraction. A double blind investigation was performed as the patients during the first 6 postoperative days were treated with three different types of eye-drops: Maxidex, Pred-Forte 1% or placebo, all containing Benzalkonium Chloride 0.004%. The content of steroid in Maxidex and Pred-Forte was equivalent. We found no significant difference in estimates of endothelial cells in the three groups postoperatively. Corneal edema was less in the two groups treated with steroid.

The presence of Descemet membrane scrolls is an uncommon finding reported in syphilitic interstitial keratitis. We herein describe a novel surgical strategy for this entity; this report demonstrates that Descemet membrane endothelial keratoplasty surgery can be performed with excellent visual outcomes in the event of corneal edema as a late sequela of syphilitic keratitis.
