Alteration of cortical excitability and its modulation by Miglustat in Niemann-Pick disease type C.

Niemann-Pick type C (NP-C) is a rare, neurodegenerative, lysosomal storage disease. Cortical excitability using different transcranial magnetic stimulation (TMS) protocols together with clinical and neuropsychological testing was longitudinally assessed in a patient with NP-C. Cerebellar inhibition, a measure for the integrity of the cerebello-thalamo-cortical network, was impaired. Short-latency afferent inhibition, a measure for cholinergic transmission, and cognitive functions were also impaired and improved under Miglustat treatment. Short interval intracortical facilitation, a marker for

Motor and visual cortical excitability in migraineurs patients with or without aura: transcranial magnetic stimulation.

BACKGROUND AND PURPOSE OF STUDY: Controversy surrounds measures of visual and motor cortical excitability in migraineurs: some authors report that interictal excitability is increased, others decreased. The aim of this work was to provide further evidence about motor and visual cortical excitability in migraineurs between attacks. PATIENTS AND METHODS: Twenty-eight migraineurs patients, 18 with aura and 10 patients without aura and 20 healthy right-handed, age and sex matched volunteers were included in the study. Each subject underwent transcranial magnetic stimulation and was submitted to t

Periodic motor cortical excitability changes associated with PSDs of EEG in Creutzfeldt-Jakob disease (CJD).

OBJECTIVE: To assess the temporal relationship between excitability changes of the motor cortex and periodic synchronous discharges (PSDs) of electroencephalograms (EEGs) in Creutzfeld-Jakob disease (CJD) patients. METHODS: In four patients with classical type CJD we gave transcranial magnetic stimulation (TMS) randomly in time while simultaneously recording their EEGs and electromyograms (EMGs). The intensity of TMS was fixed to evoke 0.3-0.5 mV EMG in the abductor pollicis brevis muscle (APB) during periods in which there were no PSDs. We assessed the relation between the timing of TMS and

Individual and Sex Differences in Behavior and Cortical Excitability in the Rat Valproate Model of Autism.

The rodent prenatal valproate (VPA) treatment is a widely used animal model of idiopathic autism spectrum disorder (ASD). However, the presence of autistic-like symptoms is highly variable in treated offspring. The disruption of the excitation-inhibition balance of certain brain areas has been proposed as a main feature in both human ASD and the VPA model. The current study presents a detailed analysis of neural development, diverse behaviors, and neocortical excitability in a high number of individually identified VPA-treated rat offspring of both sexes. Neocortical excitability was assessed

Acute changes in cortical excitability in the cortex contralateral to focal intracerebral hemorrhage in the swine.

Injury to the cerebral cortex results in functional deficits not only within the vicinity of the lesion but also in remote brain regions sharing neuronal connections with the injured site. To understand the electrophysiological basis of this phenomenon, we evaluated the effects of a focal intracerebral hemorrhage (ICH) on cortical excitability in a remote, functionally connected brain region. Cortical excitability was assessed by measuring the somatic evoked potential (SEP) elicited by electrical stimulation of the swine snout, which is somatotopically represented in the rostrum area of the pr

Abnormalities of the balance between inhibition and excitation in the motor cortex of patients with cortical myoclonus.

Patients with cortical myoclonus may have purely focal or multifocal jerks, or they may have additional bilateral or generalized jerks, suggesting the spread of excitatory myoclonic activity between the cerebral hemispheres and across the sensorimotor cortex. The factors contributing to this spread of activity were investigated in 10 patients with multifocal cortical myoclonus and eight patients with multifocal and bilateral or generalized cortical myoclonus. The two groups were termed 'non- spreaders' and 'spreaders' respectively. Eight of the patients were also epileptic. Motor thresholds to

Task-specific impairment of motor cortical excitation and inhibition in patients with writer's cramp.

Abnormalities in motor cortical excitation and inhibition have been reported in patients with writer's cramp, at rest and during muscle activation. We were interested in whether such abnormalities might be task-specific and depended on the type of movement task used to activate the dystonic hand. We therefore assessed motor-evoked potentials (facilitation/rest MEP amplitude ratio) and duration of the cortical silent period (CSP) from the right first dorsal interosseus (FDI) muscle to transcranial magnetic stimulation (TMS) in 10 patients with writer's cramp and in 10 healthy volunteers perform

Altered motor cortical excitability to magnetic stimulation in a patient with a lesion in globus pallidus.

A 27-year-old man with Behçet's disease presented with dyskinesia of the right hand and leg, but without paresis. MRI showed a discrete lesion in the internal segment of the left globus pallidus. SPECT (single photon emission tomography) disclosed increased perfusion in the hand and leg area of the left motor cortex and ipsilateral thalamus compared to that of the corresponding regions on the right. The dyskinesia was considered to be produced by disinhibition of the motor cortex due to loss of inhibitory inputs from the ipsilateral pallidum via the thalamus. Double transcranial magnetic stimu

Propagating motor cortical patterns of excitability are ubiquitous across human and non-human primate movement initiation.

A spatiotemporal pattern of excitability propagates across the primary motor cortex prior to the onset of a reaching movement in non-human primates. If this pattern is a necessary component of voluntary movement initiation, it should be present across a variety of motor tasks, end-effectors, and even species. Here, we show that propagating patterns of excitability occur during the initiation of precision grip force and tongue protrusion in non-human primates, and even isometric wrist extension in a human participant. In all tasks, the directions of propagation across the cortical sheet were bi

Excessive excitability of inhibitory cortical circuit and disturbance of ballistic targeting movement in degenerative cerebellar ataxia.

This study aimed to investigate abnormalities in inhibitory cortical excitability and motor control during ballistic-targeting movements in individuals with degenerative cerebellar ataxia (DCA). Sixteen participants took part in the study (DCA group [n = 8] and healthy group [n = 8]). The resting motor-threshold and cortical silent period (cSP) were measured in the right-hand muscle using transcranial magnetic stimulation over the left primary motor cortex. Moreover, the performance of the ballistic-targeting task with right wrist movements was measured. The Scale for the Assessment and Rating

Repetitive Peripheral Magnetic Stimulation of Wrist Extensors Enhances Cortical Excitability and Motor Performance in Healthy Individuals.

Repetitive peripheral magnetic stimulation (rPMS) may improve motor function following central nervous system lesions, but the optimal parameters of rPMS to induce neural plasticity and mechanisms underlying its action remain unclear. We examined the effects of rPMS over wrist extensor muscles on neural plasticity and motor performance in 26 healthy volunteers. In separate experiments, the effects of rPMS on motor evoked potentials (MEPs), short-interval intracortical inhibition (SICI), intracortical facilitation (ICF), direct motor response (M-wave), Hoffmann-reflex, and ballistic wrist exten

Impaired Cortical Cytoarchitecture and Reduced Excitability of Deep-Layer Neurons in the Offspring of Diabetic Rats.

Maternal diabetes has been related to low verbal task scores, impaired fine and gross motor skills, and poor performance in graphic and visuospatial tasks during childhood. The primary motor cortex is important for controlling motor functions, and embryos exposed to high glucose show changes in cell proliferation, migration, and differentiation during corticogenesis. However, the existing studies do not discriminate between embryos with or without neural tube defects, making it difficult to conclude whether the reported changes are related to neural tube defects or other anomalies. Furthermore

Cortical reflex myoclonus. A study of the relationship between giant somatosensory evoked potentials and motor excitability.

The excitability cycles of the N1-P1-N2 waveforms of the scalp-recorded somatosensory evoked potential (SEP) and of the long-latency, cortical loop reflex electromyographic (EMG) activity were studied in two patients with cortical reflex myoclonus. Long-latency cortical loop reflex EMG activity in the thenar muscles and giant SEPs occurred following median nerve stimulation. The excitability cycle of the EMG paralleled that of the SEP. There was an initial period of attenuation of SEP and EMG amplitude at interstimulus intervals (ISIs) of less than 40 ms followed by a period of amplitude enhan

C9orf72-derived dipeptide repeat proteins poly-PR disrupt membrane excitability and synaptic function in cortical neurons.

Amyotrophic lateral sclerosis (ALS) is one of the most fatal neurodegenerative disease, with the most common genetic form of the ALS is associated with hexanucleotide GGGGCC repeat expansions in the first intron of C9orf72 gene. Cortical hyperexcitability is one of the symptoms reported in several forms of ALS and implicated as a cause of neuronal death, however, the underlying mechanisms are still unclear. The dipeptide repeat (DPR) proteins produced from hexanucleotide repeat expansion have been shown toxic to neurons and induce cellular damages. In this study, we explore relationships betwe

Parietal rTMS Induced Changes in Cortical Excitability in Patients With Minimally Conscious State Using TMS-EEG.

OBJECTIVE: To verify the effectiveness of the parietal repetitive transcranial magnetic stimulation (rTMS) and take advantage of TMS-EEG to assess cortical excitability in patients with minimally conscious states (MCS). METHODS: We enrolled 10 MCS patients who received 10 sessions of 10 Hz rTMS on the parietal cortex for 10 consecutive days and then 10 days of sham stimulation after a 14-day wash-out period. The Coma Recovery Scale-Revised (CRS-R) and TMS-EEG were used to assess the levels of consciousness and cortical excitability before and after active and sham stimulation, respectively.

Reduced cortical excitability is associated with head impact severity and cognitive symptoms in adolescent football players.

American tackle football is associated with high rates of concussion, leading to neurophysiological disturbances and debilitating clinical symptoms, but previous investigations have largely ignored aperiodic neurophysiological activity. In this study, we use pre- and post-season resting-state magnetoencephalography (MEG) to examine whether concussion during a season of football is related to changes in neurophysiology and whether any such changes are associated with clinical outcomes. Concussion was associated with increased aperiodic exponents in superior frontal cortices, indicating a relati
