A 28 year old veterinary medical student experienced spiking fever, cough, peripheral blood eosinophilia and an eosinophilic pulmonary infiltrate. Corynebacterium spp. are gram-positive bacteria increasingly recognized as pathogens. This study evaluates the MICs of omadacycline, a tetracycline, against 40 clinical Corynebacterium isolates using two methodologies: broth microdilution (BMD) and Liofilchem omadacycline MIC Test Strip (MTS). By BMD, the MIC50, MIC90, and MIC range were 0.5 µg/mL, 1 µg/mL, and 0.12-2.0 µg/mL, respectively. Comparing BMD to MTS, essential agreement (EA, within ±1 doubling dilution of the reference BMD MIC) was 87.5% (95% CI: 73.0%-95.4%).

We present the case of a 46-year-old immunocompetent male diagnosed with meningitis caused by Corynebacterium striatum, a rare pathogen typically found in immunocompromised individuals. Corynebacterium striatum has developed into a new community-acquired and hospital-acquired multi-drug resistance (MDR) bacterium, and is a potential target pathogen for infection control and antibacterial management projects. Severe necrotizing fasciitis was diagnosed in a 53-year-old man in Germany in 2012. Toxigenic Corynebacterium ulcerans was grown from a wound swab sample. One of the patient's 2 dogs was found to harbor a toxigenic C. ulcerans strain. We report a case of endocarditis of a transcatheter pulmonary valve-in-valve in a 14-year-old boy with tetralogy of Fallot. He presented with recurrent low-grade fevers, lethargy, and anorexia. Multiple blood cultures grew a gram-positive rod, Corynebacterium pseudodiphtheriticum. He was taken to the operating room for removal of the vegetative endocarditis and pulmonary valve replacement.

BACKGROUND: Corynebacterium pseudodiphtheriticum is of increasing importance because of the rising number of immunocompromised patients. Pneumonia, but also endocarditis, urinary tract infections or keratitis can be caused by this bacteria in case of immunosuppression. Taking corynebacterium pseudodiphtheriticum into consideration as causitive agent provides for a fast onset of targeted antibiotic therapy.
HISTORY AND FINDINGS: A 69-year-old man with immunoglobulin deficiency due to a chronic lymphocytic leukemia presented with typical clinical, laboratory and imaging evidence of pneumonia.
