Fatal respiratory disease due to Corynebacterium diphtheriae: case report and review of guidelines for management, investigation, and control.

Dramatic reductions in the incidence of diphtheria and high levels of childhood vaccination in recent decades have led the United States to establish the goal of diphtheria elimination among persons < or = 25 years of age by the year 2000. In 1990, an unimmunized 25-month-old child died of respiratory diphtheria in Dade County, Florida, before treatment with diphtheria antitoxin could be instituted. Twenty-three asymptomatic household contacts and other close contacts of the child were identified, cultured for Corynebacterium diphtheriae, given antimicrobial prophylaxis, and vaccinated with di

Genetic relationships of Corynebacterium diphtheriae strains isolated from a diphtheria case and carriers by restriction fragment length polymorphism of rRNA genes.

In the present study we report the results of an analysis, based on ribotyping of Corynebacterium diphtheriae intermedius strains isolated from a 9 years old child with clinical diphtheria and his 5 contacts. Quantitative analysis of RFLPs of rRNA was used to determine relatedness of these 7 C.diphtheriae strains providing support data in the diphtheria epidemiology. We have also tested those strains for toxigenicity in vitro by using the Elek's gel diffusion method and in vivo by using cell culture method on cultured monkey kidney cell (VERO cells). The hybridization results revealed that the

Multidrug-resistant Corynebacterium diphtheriae in people with travel history from West Africa to France, March to September 2023.

We describe 10 unlinked cases of Corynebacterium diphtheriae infection (nine cutaneous, one respiratory) in France in 2023 in persons travelling from Guinea, Mali, Senegal, Niger or Nigeria and Central African Republic. Four isolates were toxigenic. Seven genomically unrelated isolates were multidrug-resistant, including a toxigenic respiratory isolate with high-level resistance to macrolides and beta-lactams. The high rates of resistance, including against first-line agents, call for further microbiological investigations to guide clinical management and public health response in ongoing West

Genomic epidemiology of nontoxigenic Corynebacterium diphtheriae from King County, Washington State, USA between July 2018 and May 2019.

Between July 2018 and May 2019, Corynebacterium diphtheriae was isolated from eight patients with non-respiratory infections, seven of whom experienced homelessness and had stayed at shelters in King County, WA, USA. All isolates were microbiologically identified as nontoxigenic C. diphtheriae biovar mitis. Whole-genome sequencing confirmed that all case isolates were genetically related, associated with sequence type 445 and differing by fewer than 24 single-nucleotide polymorphisms (SNPs). Compared to publicly available C. diphtheriae genomic data, these WA isolates formed a discrete cluster

[The characteristics of the lymphocyte subpopulation composition and its clinical significance in bacterial carriers of toxigenic Corynebacterium diphtheriae].

In 43 carriers of toxigenic C. diphtheriae the antigenic picture of lymphocyte membranes was studied by the method of flow cytofluorometry. The study revealed that at the time of the hospitalization of the carriers a decrease in the number of CD21+ and HDL-DR+ lymphocytes was a prognostically important criterion for the formation of carrier state, while a decrease in the content of natural killers (CD16+) was characteristic of prolonged forms of carrier state. The necessity of the study of cellular immunity factors in carriers at the time of their hospitalization and the expediency of the incl

Biosynthesis of a novel 3-oxo-2-tetradecyloctadecanoate-containing phospholipid by a cell-free extract of Corynebacterium diphtheriae.

We have isolated, purified, and identified by chemical analyses and mass spectrometry, a novel 3-oxo-2-tetradecyloctadecanoate (dehydrocorynomycolate)-containing phospholipid (PL-1) from the chloroform-methanol extract of Corynebacterium diphtheriae. This phospholipid was separated from all of the other known dehydrocorynomycolate and 3-hydroxy-2-tetradecyloctadecanoate (corynomycolate)-containing lipids and found to be unstable even at -20 degrees C. It was present in trace amounts as a homologous series (molecular weights of 1400 and 1404 as the methyl esters) and composed of a dehydrocoryno

Cloning, sequence, and footprint analysis of two promoter/operators from Corynebacterium diphtheriae that are regulated by the diphtheria toxin repressor (DtxR) and iron.

DtxR is an iron-dependent sequence-specific DNA-binding protein that binds to the tox operator, an inverted-repeat nucleotide sequence located upstream from the diphtheria toxin gene. In this study, two additional iron-regulated promoter/operator sequences (IRP1 and IRP2) that are controlled by DtxR were cloned from the chromosome of Corynebacterium diphtheriae and characterized. Operon fusions to lacZ were used to analyze expression from IRP1 and IRP2 in Escherichia coli. Transcription from both promoters was strongly repressed in high-iron medium in the presence of the cloned dtxR gene; howe

Native Aortic and Mitral Valve Endocarditis Caused by Nontoxigenic Corynebacterium diphtheriae in a Young Male Patient: A Case Report.

Non-toxigenic Corynebacterium diphtheriae strains are emerging as a cause of invasive disease, including highly virulent endocarditis, and have been associated with aggressive valvular destruction and embolic complications in reported cases without classical risk factors. A 26-year-old man with a history of former intravenous drug use and recent inhalational drug use presented with symptoms of systemic infection. He did not have any known pre-existing cardiac disease. Blood cultures grew C. diphtheriae. A transthoracic echocardiogram revealed large (>1 cm) aortic and mitral vegetations associa

Fulminant infective endocarditis with toxin-negative Corynebacterium diphtheriae in people with substance use experiencing homelessness, England, 2024 to 2025.

Between July 2024 and January 2025, five male patients in their early 20s to early 50s were confirmed with infective endocarditis associated with non-toxigenic Corynebacterium diphtheriae in England. Three were known to have experienced homelessness. All five used non-intravenous recreational drugs. Disease progression was rapid, four patients required surgical intervention, one died. Whole-genome sequencing and multilocus sequence type (MLST) analysis identified four individuals as ST559. Clinicians and substance use services have been alerted and enhanced surveillance implemented. A prevalen

Longitudinal study of wound healing status and bacterial colonisation of Staphylococcus aureus and Corynebacterium diphtheriae in epidermolysis bullosa patients.

Epidermolysis bullosa (EB) is an inherited disorder characterised by skin fragility and the appearance of blisters and wounds. Patient wounds are often colonised or infected with bacteria, leading to impaired healing, pain and high risk of death by sepsis. Little is known about the impact of bacterial composition and susceptibility in wound resolution, and there is a need for longitudinal studies to understand healing outcomes with different types of bacterial colonisation. A prospective longitudinal study of 70 wounds from 15 severe EB patients (Junctional and Recessive Dystrophic EB) from Ch

A Rare Case Report of Non-toxigenic Corynebacterium diphtheriae Bloodstream Infection in an Uncontrolled Diabetic With Peripheral Vascular Disease.

Corynebacterium diphtheriae usually causes respiratory diphtheria, which is considered as a disease of toxemia but never bacteremia. Over the last few decades, cutaneous diphtheria has been increasingly reported owing to the emergence of the non-toxigenic strain, which causes locally necrotic and ulcerative lesions. Bacteremia is very rare, but the existing evidence in the literature suggests that the organism can rarely cause invasive infections such as septicemia, endocarditis, and osteoarthritis. Here, we present a rare case of C. diphtheriae causing bloodstream infections in an elderly dia

Corynebacterium rouxii, a recently described member of the C. diphtheriae group isolated from three dogs with ulcerative skin lesions.

Corynebacterium (C.) diphtheriae is one of the two etiological pathogens for human diphtheria with significant morbidity and mortality. Recently, members of its biovar Belfanti have been described as two novel species, C. belfantii and C. rouxii. The most important virulence factor and also the premise to cause diphtheria is the isolate's capacity to encode and express the diphtheria toxin (DT). In contrast to C. ulcerans, which represents a potentially zoonotic pathogen, C. diphtheriae (incl. the novel deduced species) has almost exclusively been found to comprise a human pathogen. We here re

[Non-toxic Corynebacterium diphtheriae septicemia with endocarditis in an earlier healthy adult. First case and review of the literature].

Corynebacterium diphtheriae septicaemia is rarely encountered, usually in very particular situations: children with severe congenital heart disease or after heart surgery. Rare cases have been reported in immunodepressed adults or drug addicts. We observed a case in a formerly healthy 41-year-old woman who was hospitalized for fever unresponsive to bacampicillin. In this patient, no portal of entry could be identified; there was no history of past surgery nor drug abuse. The patient was not immunodepressed and HIV serology was negative. The last anti-diphtheria vaccination had been given at th

Two diphtheria sub-clusters with autochthonous cases in Germany and Poland within a Corynebacterium diphtheriae ST-574 outbreak, 2022 to July 2025.

Within a Corynebacterium diphtheriae ST-574 outbreak comprising mostly migration-associated cases since 2022, Germany detected two sub-clusters in 2025. Among sub-cluster cases with travel information (24/26), 22 were acquired autochthonously. One of two imported cases came from a Polish voivodeship having an additional case. Sub-cluster 1 included 16 cutaneous cases, sub-cluster 2 included six cutaneous and five respiratory cases. One unvaccinated child and two older adults died. Of 27 cases, 12 experienced homelessness. Detection and diphtheria toxin status of Corynebacterium ulcerans in canine and feline clinical specimens in Japan.

Corynebacterium ulcerans (C. ulcerans) is capable of producing diphtheria toxin and causing zoonotic diphtheria, with companion animals recognized as an important infection source. We evaluated the occurrence and diphtheria toxin status of C. ulcerans in canine and feline clinical specimens in Japan. Out of 244 Corynebacterium isolates, 18 were identified as C. ulcerans using the matrix-assisted laser desorption ionization time-of-flight mass spectrometry (MALDI-TOF MS) and C. ulcerans-specific PCR, including 1 from a dog and 17 from cats. The nasal cavity was the most common isolation site (1

Household transmission of non-toxigenic diphtheria toxin gene-bearing Corynebacterium diphtheriae following a cluster of cutaneous cases in a specialist outpatient setting.

Introduction. Combination of PCR and Elek testing to identify toxigenic corynebacteria has revealed organisms described as non-toxigenic toxin-gene bearing (NTTB) Corynebacterium diphtheriae or C. ulcerans (i.e. PCR tox positive; Elek negative). These organisms carry part or all of tox, but are unable to express diphtheria toxin (DT) and present a challenge to clinical and public health case management.Gap analysis/Hypothesis. There are few data on the theoretical risk of NTTB reversion to toxigenicity. This unique cluster and subsequent epidemiologically linked isolates allowed the opportunit
