The prevalence of canine patellar luxation in three centres. Clinical features and radiographic evidence of limb deviation.

The medical records of 155 dogs with patellar luxation (PL) from three different centres were analysed. Each case was classified according to the nature of its luxation and any concurrent orthopaedic conditions plus the age at diagnosis were also noted. Measurements relating to angle of inclination (AOI) of the femoral neck and medio-lateral bowing of the femur and tibia at the stifle were also recorded. The femoral and tibial data were compared to dogs with another orthopaedic condition in a case-control assessment. Labradors were most commonly affected (21%). Most luxations were medial (92%)

Surgical treatment of developmental dysplasia of the hip in adults: I. Nonarthroplasty options.

Hip dysplasia is a developmental disorder that results in anatomic abnormalities leading to increased contact pressure in the joint and, eventually, coxarthrosis. However, many patients with hip dysplasia become symptomatic before the development of severe degenerative changes because of abnormal hip biomechanics, mild hip instability, impingement, or associated labral pathology. Several nonarthroplasty treatment options are available. Because the primary deformity is mostly acetabular, for many patients, a reconstructive osteotomy that restores more nearly normal pelvic anatomy is preferable.

Morphometric analysis of the femur in cerebral palsy: 3-dimensional CT study.

BACKGROUND: The cause of hip disorder in cerebral palsy (CP) has been thought to involve muscle imbalance, flexion, and adduction contracture of the hip joint, acetabular dysplasia, and femoral growth abnormalities. The aim of this study was to quantitatively evaluate the 3-dimensional femoral geometry and subluxation/dislocation of the hip in spastic CP using 3D-CT reconstructed images of the pelvis and the femur, focusing on the femoral growth abnormalities in CP. METHODS: Between June 2006 and September 2009, 186 hips in 93 bilateral spastic CP patients, including spastic diplegia (SD) in

Pertrochanteric Femur Fracture in a Post-Polio Patient treated with a Pediatric Hip Plate: A Case Report.

INTRODUCTION: Fracture fixation in post-polio residual paralysis is technically demanding due to dysplastic proximal femoral anatomy, osteopenia, and distorted trochanteric morphology. Standard cephalomedullary nailing may be impossible when the medullary canal is narrow, or the entry point is malpositioned. CASE REPORT: A 45-year-old man with childhood poliomyelitis and complete paralysis of the right leg sustained an AO 31-A1.2 pertrochanteric fracture. Radiographs revealed coxa valga and a markedly narrow canal, precluding safe cephalomedullary nailing. Closed reduction on a traction table

[Development in the treatment of congenital dislocation of hip (author's transl)].

Authors review the development in the treatment of congenital dislocation and dysplasy of hip and demonstrate the results with various methods. Results with treatment according to Lorenz (1040 cases) were unsatisfactory because of frequent occurance (66.3 percent) of osteochondritis. The treatment with Frejka cushion (360 cases) did not improve basically the results. Pavlik's strapping, especially when started under 1 month of age gave excellent results (3156 cases). In operative treated the results of 332 Colonna operations were unsatisfactory because of consecutive deformation of the hip and

Overgrowth syndrome associated with a gain-of-function mutation of the natriuretic peptide receptor 2 (NPR2) gene.

The signal pathway of the C-type natriuretic (CNP) and its receptor, natriuretic peptide receptor 2 (NPR2) is involved in the longitudinal growth of long bones. Loss of function mutations at NPR2 cause acromesomelic dysplasia, type Maroteaux, while overproduction of CNP by chromosomal translocation and a gain-of-function mutation at NPR2 have been reported to be responsible for an overgrowth syndrome in three cases and one family, respectively. We identified a four-generation family with an overgrowth syndrome characterized by tall stature, macrodactyly of the great toes, scoliosis, coxa valga

Editorial Commentary: Evaluate for the Beighton Score and Additional Radiographic Signs of Instability Prior to Proceeding With Hip Arthroscopy in Patients With Combined Borderline Hip Dysplasia and Excessive Femoral Anteversion.

Femoral version abnormalities have been increasingly recognized as a key factor in the pathogenesis of nonarthritic hip pain. Excessive femoral anteversion (EFA), defined as femoral anteversion greater than 20°, has been postulated to create unstable alignment of the hip, which is exacerbated in patients with concomitant borderline hip dysplasia (BHD). The optimal treatment algorithm for hip pain in EFA-BHD patients remains debated, with some surgeons advocating against arthroscopic procedures in isolation owing to the combined instability due to the femoral and acetabular abnormalities. When

A new family with epiphyseal chondrodysplasia type Miura.

Epiphyseal chondrodysplasia, Miura type (ECDM) is a skeletal dysplasia with tall stature and distinctive skeletal features caused by heterozygous NPR2 pathogenic variants. Only four families have been reported. We present a family with five affected individuals (mother, three sons, and daughter). The mother's phenotype was relatively mild: borderline tall stature and elongated halluces operated during childhood. The children were remarkably more severely affected with tall stature, scoliosis, and elongated toes and fingers leading to suspicion of Marfan syndrome. Progressive valgus deformities

Spondyloepiphyseal dysplasia with accumulation of glycoprotein in the chondrocytes: spondyloepiphyseal dysplasia, Stanescu type.

OBJECTIVE: To clarify the phenotype in a bone dysplasia termed "spondyloepiphyseal dysplasia with accumulation of glycoprotein in the chondrocytes" by Stanescu et al. DESIGN AND PATIENTS: Subjects comprised two definitive cases of one family and one probable case of another family. Histologic examination in one patient warranted the diagnosis of the first family, whereas the diagnosis of the second family was based solely on clinical and radiologic grounds. RESULTS: Pedigrees revealed an autosomal dominant mode of transmission. All three patients shared painful large joints with joint restri

Adverse sequelae due to plate retention following submuscular plating for pediatric femur fractures.

OBJECTIVE: Submuscular plating has become a common operative treatment of length-unstable pediatric femur fractures. There has been no consensus regarding the need for plate removal after fracture union. The purpose of this report was to describe the potential constellation of findings that may occur with growth after plate retention. DESIGN: Retrospective case series. METHODS: A retrospective study was done on all patients who underwent a submuscular plate application for a pediatric femur fracture at our institution between 2003 and 2010 by the senior author. We reviewed the clinical and r

Valgus slipped capital femoral epiphysis: report of two cases and a comprehensive review of literature.

Valgus slip of the capital femoral epiphysis is a known but extremely rare form of a slipped capital femoral epiphysis. After the first report in 1926 up to the present, approximately 44 cases have been reported. We report two cases of valgus slip of the capital femoral epiphysis with a comprehensive review of the existing literature and the precautions that are needed while planning surgical management of these patients. Awareness of this condition is essential to make a prompt diagnosis and plan the appropriate management.

[Triple osteotomy of the pelvis in dysplastic hip joints in children and adults].

We report on a post operative radiological and clinical follow-up (3-6.4 years) of 80 triple-osteotomies (29 operations according to the technique of Carlioz and 51 according to Tönnis). This study includes 32 osteotomies in children and 48 in adults. In 82% of the cases of low dysplastic hip joints and in 88% of the cases in severe dysplastic hip joints, the results came up to the expectations of the patients. The acetabular inclination angle and the acetabular angle improved about 10 degrees and the center edge angle about 25 degrees. In cases of neurological diseases, the radiological resul

Description of the potential of an arthrometer for standard and reduced radiographs suitable to measurement of angles and segments of hip, knee, foot and joint space widths.

BACKGROUND: Primary osteoarthritis is usually selected in either epidemiological or therapeutic studies. This implies exclusions. Among cases of secondary osteoarthritis considered for either stratification or exclusion--or for prognosis and treatment in daily practice--are those due to architectural defects. Parameters of the latter should be measured to ascertain diagnosis. At present, measurements have to be performed either on digitized reduced films or standard radiographs. OBJECTIVE: To finalize an instrument capable of measuring the main angles and segments characteristic of the main d

A new case of Melnick-Needles syndrome with skeletal manifestations: A case report.

INTRODUCTION AND IMPORTANCE: Melnick-Needles syndrome (MNS) is a rare skeletal dysplasia that affects skeletal and connective tissue. Less than 70 cases of MNS reported in the literature. MNS had various clinical manifestations such as skeletal deformity, cortical bony sclerosis, facial abnormality, and urogenital symptoms. CASE PRESENTATION: We presented a 5-year-old girl who referred to our orthopedic clinic with knee valgus deformity, spinal kyphoscoliosis, bilateral coxa valga, and humerus cortical irregularity. Based on some facial and skeletal feature, MNS was confirmed with genetic eva

[Challenges of primary hip arthroplasty with high hip dislocation].

BACKGROUND: Endoprosthetic care of high hip dislocation is a surgical challenge. The hip anatomy is greatly altered in these patients, including a rather flat and small acetabulum with impaired bone quality and a relevant chance of a bony defect of the acetabular roof. Additionally, the front coverage and in some cases even the dorsal coverage of the hip are missing. The proximal femur is characterized with an increased antetorsion, a coxa valga position and an enlarged greater trochanter. The medullary cavity is narrowed, the offset is reduced, and the absolut leg length can be enlarged. Furt

Morquio A syndrome: diagnosis and current and future therapies.

Morquio A syndrome is an autosomal recessive disorder, one of 50 lysosomal storage diseases (LSDs), and is caused by the deficiency of N-acetylgalactosamine-6-sulfate sulfatase (GALNS). Deficiency of this enzyme causes specific glycosaminoglycan (GAG) accumulation: keratan sulfate (KS) and chondroitin-6-sulfate (C6S). The majority of KS is produced in the cartilage, therefore, the undegraded substrates accumulate mainly in cartilage and in its extracelluar matrix (ECM), causing direct leads to direct impact on cartilage and bone development and leading to the resultant systemic skeletal spondy

Outcome of extensive varus and valgus stem alignment in short-stem THA: clinical and radiological analysis using EBRA-FCA.

INTRODUCTION: The principle of implanting a calcar-guided short stem consists of an individual alignment alongside the medial calcar providing the ability of reconstructing varus and valgus anatomy in a great variety. However, still, there are broad concerns about the safety of extensive varus and valgus positioning in regard to stability, bony alterations, and periprosthetic fractures. MATERIALS AND METHODS: 216 total hip arthroplasties using a calcar-guided short stem (optimys, Mathys Ltd.) in 162 patients were included.
