The use of non-destructive approaches for digital acquisition (e.g. computerised tomography-CT) allows detailed qualitative and quantitative study of internal structures of skeletal material. Here, we present a new R-based software tool, Icex, applicable to the study of the sizes and shapes of skeletal cavities and fossae in 3D digital images. Traditional methods of volume extraction involve the manual labelling (i.e. segmentation) of the areas of interest on each section of the image stack. This is time-consuming, error-prone and challenging to apply to complex cavities. Icex facilitates rapi

A small percentage of patients with acute frontal or ethmoid sinusitis develop orbital, cranial or CNS complications. At selected University of Tennessee affiliated hospitals, from 1974 to 1978, there were 14 such cases which required major surgical intervention in addition to intensive medical therapy. The most common complication in this series was subperiosteal orbital abscess. The most common bacterial isolates were streptococcus and staphylococcus. A discussion of complications associated with frontal and ethmoid sinusitis is included as well as recommendations for medical and surgical ma

The most common cause of cerebrospinal fluid (CSF) rhinorrhoea is damage to the skull base with a dura mater's rupture due to an accident or an iatrogenic injury. This applies to over 96% of cases. Other possibilities that can lead to CSF leakage are neoplasms of the nasal cavity, paranasal sinuses, and nasopharynx. Although prostate cancer spreads to bones, cranial metastases to paranasal sinuses are extremely rare. We present a case of an 83-year-old patient with CSF leakage due to infiltrating metastatic prostate cancer. Cerebrospinal fluid rhinorrhea turned out to be the first symptom of p

Central nervous system (CNS) infections due to filamentous basidiomycetes are extremely rare. We encountered a case of epidural abscess due to Schizophyllum commune that extended from sinusitis. A 53-year-old Japanese man presented at our hospital with a headache. Computed tomography (CT) of the cranium and sinuses showed ethmoid and sphenoid sinusitis with no intracranial abnormalities. The patient was diagnosed with acute sinusitis and underwent antibiotic treatment. However, the symptoms deteriorated, and the patient came to our hospital again with consciousness disturbance. CT scan of the

Venous anomalies are the most commonly identified abnormality by imaging in the work-up for pulse synchronous tinnitus. Potential diagnoses include idiopathic intracranial hypertension, sigmoid sinus wall anomalies, transverse and sigmoid sinus stenosis, jugular bulb anomalies, and prominent posterior fossa emissary veins. These causes are discussed in detail along with the association between sigmoid sinus wall anomalies and idiopathic intracranial hypertension.

The consequences of intracranial spread of sinus infection can be dismal. The subtle presentation of sphenoid sinusitis often leads to a delay in diagnosis. The disease may go unrecognized until complications are severe enough to cause more localizing symptoms. Often infections in the head and neck spread into the cranial cavity, leading to a localized effect. For example, otogenic infections can spread to the mastoid or lateral venous sinus. We report a case of sphenoid sinusitis complicated by lateral and sigmoid venous sinus thrombosis.

Aspergillus sp. sinusitis is not uncommon in immunocompromised patients but is unusual in patients who are not immunocompromised. The disease may occur as a saprophytic condition, as an allergic sinusitis or as a potentially lethal invasive disease. The differentiation between non-invasive and invasive Aspergillus sp. sinusitis is crucial and this distinction is fully discussed. The treatment options are also considered. Invasive disease requires aggressive treatment with long-term antifungal agents in sufficient doses combined with wide surgical excision. We present a patient who presented wi

This is a case description of a male patient found to have orbital and intracranial emphysema, specifically with air in his cavernous sinuses bilaterally following penetrating trauma to the medial orbit from a goat's horn. There were no orbital or skull base fractures. Although the presence of traumatic intracranial emphysema is not uncommon, it is typically the result of direct communication of the cranial vault with the paranasal sinuses in the setting of associated fracture or, alternatively, from direct penetration and inoculation. We present a rare case of orbital emphysema with traumatic

The cranial nerves, due to its location, are not infrequently involved in the nasopharyngeal neoplasm evolutionary course. In this paper we describe two cases in which the neoplastic infiltration in both cavernous sinuses brought the appearance of an acute bilateral ophthalmolegia on. This symptomatology meant, in the first case, the onset of disease. In the second one, it was the reason of consultation. Later we discuss the topographic location of the damages in both cavernous sinuses considering the observed neurological shortcomings. Finally we approach a short revision of the causes of bil

Isolated sphenoid sinusitis is a rare disorder and may present with complications due to its anatomical location and proximity to the intracranial and orbital contents. It is frequently misdiagnosed, because the sphenoid sinus is not visualised adequately with routine sinus radiographs and is not accessible to direct clinical examination. We report a case who presented with hemicranial headache and ipsilateral abducens nerve palsy as the presenting feature of sphenoid sinusitis. The symptoms disappeared within a week of conservative treatment. Sphenoid sinusitis should be kept in the different

The association of large mastoid emissary veins and pulsatile tinnitus has been reported. However, therapeutic options for this condition remain limited. We report a case of endovascular coiling of a large mastoid emissary vein in a patient with disabling pulsatile tinnitus with significant improvement of symptoms. To our knowledge, endovascular coiling of large mastoid emissary vein causing pulsatile tinnitus has not been reported.

BACKGROUND: The traditional retrosigmoid (RS) approach provides limited exposure of the inferior compartment of the CPA, while radical skull base approaches are demanding and associated with significant morbidity.
METHODS: This study outlines the relevant surgical anatomy and the different surgical steps of a modified retrosigmoid (MRS) approach.
RESULTS: The MRS provides enhanced exposure of the CPA and deep vascular structures resulting from a modified RS craniotomy and limited exposure of the sigmoid sinus.
CONCLUSION: In selected posterior fossa lesions, this cisternal approach is a str

Variations of the intracranial venous sinuses are important to the surgeon during intraoperative procedures and to the clinician during imaging interpretation. We report a male cadaver found to have a rare venous sinus variation. In all likelihood, this sinus corresponded to the rarely reported accessory venous sinus of Hyrtl. The sinus was approximately 5 mm in width and traveled from the sphenoparietal sinus anteriorly to the veins, draining into the foramen spinosum (i.e. the middle meningeal veins) posteriorly. No other variations or obvious pathology were identified intracranially or extr

OBJECTIVE: Certain neurosurgical procedures require sectioning of the tentorium cerebelli. The presence of venous sinuses within the tentorium makes these procedures difficult. The aim of this study was to investigate the incidence, size, location, configuration, and pattern of venous drainage of these sinuses.
METHODS: The tentorium cerebelli was studied in 80 fresh cadavers. After the skull cap and the supratentorial portion of the brain were removed, the tentorium was inspected for the presence of venous sinuses. Their location, size, configuration, and pattern of venous drainage were note

Sphenoid sinusitis is a rare form of sinusitis characterized by inflammation of the sphenoid sinuses, located deep within the skull. Despite its rarity, sphenoid sinusitis can lead to significant morbidity due to its proximity to critical anatomical structures, including the optic nerves and the cavernous sinuses. Clinical presentations may range from non-specific symptoms such as headaches and facial pain to more severe manifestations like visual disturbances or cranial nerve palsies. Diagnosis typically involves a combination of clinical evaluation, imaging studies (such as computed tomograp

Of 60 patients with acute bilateral ophthalmoparesis, the cause was within the brainstem in 18 (pretectal infarct, phenytoin toxicity, and Wernicke's syndrome), in cranial nerves in 26 (Guillain-Barré or Fisher syndrome, tuberculous meningitis), within the cavernous sinuses in 8 (tumors or infection), and at the myoneural junction in 8 (myasthenia or botulism).

The cavernous sinuses of 50 adult cadavers were examined to investigate the relationships of the blood vessels and cranial nerves, important structures during surgery in this sinus. The first and second divisions of the fifth cranial nerve were embedded in the deep dural layer of the cavernous sinus and were supplied by the two main branches of the intracavernous carotid artery. The meningohypophyseal artery supplied the sixth cranial nerve in Dorello's canal and the third and fourth cranial nerves where they entered the dura. The inferolateral trunk supplied the third, fourth, fifth, and sixt

The authors review their 2-year experience with a rhinoseptal transsphenoidal approach to skull-base tumors of various pathologies involving both the sphenoid and cavernous sinuses. Eight patients with cranial nerve palsies attributable to compression of the contents of the cavernous sinus and/or optic canal are included in this report. Among these patients, a total of 17 cranial nerves were affected. Postoperative normalization was achieved in eight nerves, significant improvement in seven nerves, and no improvement in two nerves. There were no operative complications of aggravation of crania
