Family therapy in the treatment of adolescents.

A family with an adolescent must transform itself from a predominantly nurturant unit to one that can tolerate and encourage the adolescent's need to separate. When an adolescent presents with symptoms that disrupt the developmental process, the clinician who is familiar with several models of family therapy is better able to select a therapeutic and cost-effective intervention. For family assessments, the authors recommend a biopsychosocial approach, which has replaced the outdated view that families cause psychiatric problems and which acknowledges the family as a potential source of healing

Post-Incident Responses (Debriefing) in Mental Health Services in England: A Policy Review.

INTRODUCTION: Post-incident responses, also described as debriefing, are structured processes that provide opportunities to address harm and identify learning to prevent future incidents. National guidance recommends post-incident responses after restrictive interventions but provides little indication of the explicit purpose. AIM: To examine how post-incident responses are defined and implemented according to English NHS mental health trust policies. METHOD: A Freedom of Information request was sent to all 52 English NHS mental health trusts to identify policies, guidelines, procedures and

Identifying (effective) elements in the psychological treatment of suicidality: A systematic review and meta-analysis.

INTRODUCTION: Various psychotherapeutic interventions have shown promise in reducing suicidality. However, it is unclear which components make interventions successful. Studying components individually rather than evaluating interventions as one (e.g. cognitive-behavioural therapy) could help us develop more effective interventions. The current study systematically recorded and evaluated individual components used in interventions aimed at reducing suicidality. METHODS: We systematically searched six databases for randomised controlled trials up to April 1, 2024. Included studies directly tar

Crisis Phone Services.

Pediatric mental health crises increasingly strain emergency departments (EDs), underscoring the need for coordinated alternatives to acute care. This review situates pediatric crisis phone services within the broader crisis care continuum and distinguishes between crisis hotlines, crisis triage lines, and phone-based follow-up services. Using Kids'Link of Rhode Island as an illustrative example, this review demonstrates how a statewide, integrated crisis triage model can support families, facilitate timely risk assessment and referral, and reduce unnecessary ED utilization. Findings highlight

The Effectiveness of Debriefing on the Mental Health of Rescue Teams: A Systematic Review.

Background: Rescue teams and emergency services face high levels of mental health problems due to their frequent exposure to traumatic situations. Critical incident stress debriefing (CISD) is widely used as a psychological intervention for emergency responders and military personnel exposed to traumatic events. However, its effectiveness remains controversial, with systematic reviews yielding mixed results and some evidence of negative and harmful outcomes. This systematic review, conducted according to PRISMA guidelines, evaluates the evidence on the efficacy of CISD in mitigating psychologi

Spaghettification and the Conceptual Black Hole of Borderline Personality Disorder: A Qualitative Discussion Around Competing Meanings Given to the Diagnosis and Their Potential Consequences.

This paper explores competing meanings which may arise through receiving a diagnosis of "borderline personality disorder," discussing how this may impact a person's treatment and their sense of self. This paper is informed by qualitative case study research, which utilized interviews to explore experiences of crisis and crisis intervention for people diagnosed with borderline personality disorder, their family and friends, and professionals who work with them. Utilizing this qualitative research data, alongside wider literature, the conceptualization of "borderline personality disorder" and th

Rapport Building in Written Crisis Services: Qualitative Content Analysis.

BACKGROUND: Building therapeutic relationships and social presence are challenging in digital services and maybe even more difficult in written services. Despite these difficulties, in-person care may not be feasible or accessible in all situations. OBJECTIVE: This study aims to categorize crisis counselors' efforts to build rapport in written conversations by using deidentified conversation transcripts from the text and chat arms of the National Child Abuse Hotline. Using these categories, we identify the common characteristics of successful conversations. We defined success as conversations

Patient and pharmacist perspectives on opioid misuse screening and brief interventions in community pharmacies.

BACKGROUND: Pharmacy-based screening and brief interventions (SBI) offer opportunities to identify opioid misuse and opioid safety risks and provide brief interventions that do not overly burden pharmacists. Currently, such interventions are being developed without patient input and in-depth contextual data and insufficient translation into practice. The purpose of this study is to qualitatively explore and compare patient and pharmacist perceptions and needs regarding a pharmacy-based opioid misuse SBI and to identify relevant SBI features and future implementation strategies. METHODS: Using

Promoting alcohol treatment engagement post-hospitalization with brief intervention, medications and CBT4CBT: protocol for a randomized clinical trial in a diverse patient population.

BACKGROUND: Alcohol use disorder (AUD) commonly causes hospitalization, particularly for individuals disproportionately impacted by structural racism and other forms of marginalization. The optimal approach for engaging hospitalized patients with AUD in treatment post-hospital discharge is unknown. We describe the rationale, aims, and protocol for Project ENHANCE (ENhancing Hospital-initiated Alcohol TreatmeNt to InCrease Engagement), a clinical trial testing increasingly intensive approaches using a hybrid type 1 effectiveness-implementation approach. METHODS: We are randomizing English and/

Alcohol brief intervention, specialty treatment and drinking outcomes at 12 months: Results from a systematic alcohol screening and brief intervention initiative in adult primary care.

BACKGROUND: Alcohol screening, brief intervention and referral to treatment (SBIRT) in adult primary care is an evidence-based, public health strategy to address unhealthy alcohol use, but evidence of effectiveness of alcohol brief intervention (ABI) in real-world implementation is lacking. METHODS: We fit marginal structural models with inverse probability weighting to estimate the causal effects of ABI on 12-month drinking outcomes using longitudinal electronic health records data for 312,056 adults with a positive screening result for unhealthy drinking between 2014 and 2017 in a large hea

The Cost-Effectiveness of Adapting and Implementing a Brief Intervention to Target Frequent Alcohol Use Among Persons with HIV in Vietnam.

Brief interventions to reduce frequent alcohol use among persons with HIV (PWH) are evidence-based, but resource-constrained settings must contend with competition for health resources. We evaluated the cost-effectiveness of two intervention arms compared to the standard of care (SOC) in a three-arm randomized control trial targeting frequent alcohol use in PWH through increasing the percent days abstinent from alcohol and viral suppression. We estimated incremental cost per quality-adjusted life year (QALY) gained from a modified societal perspective and a 1-year time horizon using a Markov m

A Within-Subject Pilot Feasibility Study of a Gambling Specific SBIRT Intervention Delivered in an Urban HIV/Primary Clinic.

BACKGROUND: Although there are few interventions available to provide screening and brief intervention targeted toward problematic gambling, Screening, Brief Intervention and Referral to Treatment (SBIRT) is an evidence-based intervention that has demonstrated effectiveness in reducing gambling behaviors. METHODS: The goal of this pilot study was to evaluate the feasibility, acceptability and preliminary outcomes of a gambling specific SBIRT intervention in a medical setting. Fifteen participants were recruited from an urban HIV/Primary Care clinic to receive the gambling specific SBIRT inter

The development and evaluation of a brief intervention for male partners of women hospitalized for perinatal distress.

BACKGROUND: Perinatal distress (PD) is a term used to describe mood and anxiety disorders experienced during pregnancy or in the postpartum period. In acute cases of PD, inpatient hospitalization may be indicated. Although hospitalization tends to result in improved immediate safety outcomes, many patients are discharged when they are still experiencing acute symptomology. Interpersonal and environmental factors, particularly those that exist within intimate relationships, can significantly help or hinder patient progress. Partners are also affected by the patient's symptoms and often lacks th

Evolution of child psychiatric teamwork in a somatic hospital.

This paper outlines the evolution of child psychiatric team work within the context of a general hospital. It focuses especially on the professional challenges and systemic problems arising during this 8 year long process. The question of what happens when two cultures meet in the same arena is discussed, and the process whereby a minority becomes a reality in a well-established majority system is described. Special attention is devoted to those principles appearing to play a major role in the evolutionary process. Prominent among these are: (1) operating within the already established; (2) tr

Are Mental Health Crisis Assessment and Treatment Teams Getting Busier and Managing Increased Complexity?

For several decades crisis assessment and treatment teams (CATTs) have been an integral part of community mental health treatment. Since their implementation, however, there has been a significant shift in demand and complexity for the general community requiring acute mental health response. This change coincides with a worldwide mental health workforce shortage and a need to utilise more novice clinicians. This study aimed to examine any change in workload and complexity for the population the CATTs support. A total of 100 files were audited over a 7-year period examining caseload and scenar

Is anti-ligature an automatic requirement for suicide prevention?: Assessing legal obligations in alternative mental health crisis services.

Around the world, peer-run, bed-based services that offer voluntary support to people experiencing mental health crisis are emerging as alternatives to hospital-based mental healthcare. In Victoria, Australia, where these 'alternative crisis services' are expanding, a key legal question has arisen: must non-coercive settings adopt the suicide prevention architectural design (or 'anti-ligature design') mandated in psychiatric wards under negligence and occupational health and safety laws? This article answers that question through a three-stage study: (1) a doctrinal analysis of Victorian case

The State of Mobile Crisis Response: a Scoping Review of Construct and Challenges.

UNLABELLED: There is a significant unmet need for mental health services in the United States. Many communities have responded to the unmet need by creating mobile crisis response programs. Enhanced understanding of the differences and common themes in mobile crisis response programs will guide efforts to construct frameworks for successful future program development. This review aims to characterize the current state of the industry among mobile crisis response teams. METHODS: We employed a scoping review methodology to explore the state of the work in mobile crisis response. RESULTS: Overa
