New Insights into the Fluid Management in Patients with Septic Shock.

The importance of fluid resuscitation therapy during the early stages of sepsis management is a well-established principle. Current Surviving Sepsis Campaign (SSC) guidelines recommend the early administration of intravenous crystalloid fluids for sepsis-related hypotension or hyperlactatemia due to tissue hypoperfusion, within the first 3 h of resuscitation and suggest using balanced solutions (BSs) instead of normal saline (NS) for the management of patients with sepsis or septic shock. Studies comparing BS versus NS administration in septic patients have demonstrated that BSs are associated

[Crystalloids and colloids: fluid homeostasis and toxicity].

Fluid balance is crucial in critically ill patients because mechanisms to counteract fluid dysbalances are limited. Therefore, an extremely important task of ICU staff is to assess and maintain fluid status. Fluid overload has been shown to have deleterious consequences for patients. Infusion fluids may be divided into colloid and crystalloid solutions. Within the crystalloids, balanced solutions have been shown to be superior over 0.9% saline with respect to acidosis, acute kidney injury, and mortality. The most widespread used colloid is hydroyethyl starch (HES). Compared to crystalloids, HE

Cold blood cardioplegia reduces the increase in cardiac enzyme levels compared with cold crystalloid cardioplegia in patients undergoing aortic valve replacement for isolated aortic stenosis.

OBJECTIVES: Cardiac arrest during cardiac surgery is most commonly induced by cold blood or cold crystalloid cardioplegia. The results from clinical studies are divergent regarding which of the 2 solutions provides better myocardial protection. This might be explained by several factors. Both heterogeneity in disease for the included patients and the fact that most studies are retrospective in design and that patients with coronary artery disease with different degrees of myocardial ischemia are included might explain these findings. To circumvent these potentially confounding factors, we incl

Balanced crystalloid versus saline for resuscitation in adult sepsis: a systematic review and meta-analysis.

BACKGROUND: Fluid resuscitation is the cornerstone of sepsis management, however, the choice between balanced crystalloid solutions and normal saline remains debated. This meta-analysis aimed to compare the efficacy of balanced solutions versus saline for fluid resuscitation in adult patients with sepsis. METHODS: A systematic literature search was performed in PubMed, Embase, Scopus, and other relevant sources. Retrieved studies were rigorously screened according to predefined inclusion and exclusion criteria. Data from the included studies were extracted for analysis. The primary outcome wa

[Thromboelastographic characteristics of different infusion therapy regimens in healthy bone marrow donors].

Infusion solutions are able to change the hemostatic system. Thromboelastography (TEG) is an integral technique to evaluate the hemostatic system. TEG was used to evaluate the effect of three infusion solutions (6% hydroxyexyethyl starch (HES) 200/0.5 - Hemohes; HES 130/0.4 - Voluven; modified gelatin solution - Gelofusin) on the hemostatic system in 36 bone marrow donors (healthy individuals). The solutions were used in combination with crystalloid solutions during a procedure to compensate for intraoperative blood loss. Hemostatic changes were noted by the end of an operation in all groups;

Parenteral fluid regimens for improving functional outcome in people with acute stroke.

BACKGROUND: Parenteral fluids are commonly used in people with acute stroke with poor oral fluid intake. However, the balance between benefit and harm for different fluid regimens is unclear. OBJECTIVES: To assess whether different parenteral fluid regimens lead to differences in death, or death or dependence, after stroke based on fluid type, fluid volume, duration of fluid administration, and mode of delivery. SEARCH METHODS: We searched the Cochrane Stroke Group Trials Register (May 2015), the Cochrane Central Register of Controlled Trials (CENTRAL), the Cochrane Database of Systematic Re

[Experience in using normofundin and sterofundin solutions for the correction of hypernatremia].

The efficacy of Sterofundin, Normofundin, and physiological solutions in correcting hypernatremia and impaired acid-base balance was analyzed in patients with sellar region tumors. Twenty-one patients were enrolled in the study; three groups were formed. Physiological solution added by 4% of potassium chloride and 25% magnesium sulfate solution was used in Group 1 (n = 7). Normofundin was employed in Group 2 (n = 7). Sterofundin was applied to Group 3 (n = 7). The volume of administered fluid corresponded to the sum of volumes of basic needs and those of fluid losses. The study revealed that t

I.V. fluid therapy. Part 2. I.V. fluid selection.

I.v. fluid selection depends on the estimated fluid loss, the primary fluid compartment involved, the patient's underlying problem and the physiological and haemodynamic impact of the i.v. solution. Clinically, the most important problem is intravascular fluid volume deficit, which is associated with hypotension, inadequate tissue oxygenation and hypoperfusion of essential organs. Intravascular volume resuscitation is therefore of primary importance. Crystalloid solutions have the disadvantage of only small amounts remaining in the IVS whereas colloids are known as plasma volume expanders due

Hypertonic saline in critical illness - A systematic review.

INTRODUCTION: The optimal approach to fluid management in critically ill patients is highly debated. Fluid resuscitation using hypertonic saline was used in the past for more than thirty years, but has recently disappeared from clinical practice. Here we provide an overview on the currently available literature on effects of hypertonic saline infusion for fluid resuscitation in the critically ill. METHODS: Systematic analysis of reports of clinical trials comparing effects of hypertonic saline as resuscitation fluid to other available crystalloid solutions. A literature search of MEDLINE and

Crystalloid versus colloid for fluid resuscitation of hypovolemic patients.

The choice of the initial asanguinous fluid--either crystalloid or colloid--used for the resuscitation of the hypovolemic patient remains controversial. Colloid supporters argue for the careful preservation of the plasma colloid osmotic pressure (PCOP) to protect the lung from pulmonary edema. A careful analysis of the Starling microvascular forces operative at the pulmonary capillary makes such an effect unlikely. In fact, the lung is relatively immune to hemodilution and any decrease in PCOP is roughly one fourth as important as increases in hydrostatic pressure in causing increased fluid ex

[Tactics of infusion therapy in the acute period of intracranial hemorrhages].

The paper deals with the determination of infusion therapy tactics in critically ill patients with intracranial hemorrhages on the basis of invasive measurements of systemic hemodynamics. The routine hemodynamic parameters (blood and central venous pressures, heart rate) are noted to fail to assess the volemic status of the patients in full. Unlike the use of colloidal solutions, infusion therapy with physiological sodium chloride is not shown to correct systemic hemodynamics. It has been ascertained that in acute intracranial hemorrhages, infusion therapy with crystalloidal solutions leads to

Effect of Intraoperative Goal-directed Balanced Crystalloid versus Colloid Administration on Major Postoperative Morbidity: A Randomized Trial.

BACKGROUND: Crystalloid solutions leave the circulation quickly, whereas colloids remain for hours, thus promoting hemodynamic stability. However, colloids are expensive and promote renal toxicity in critical care patients. This study tested the hypothesis that goal-directed colloid administration during elective abdominal surgery decreases 30-day major complications more than goal-directed crystalloid administration. METHODS: In this parallel-arm double-blinded multicenter randomized trial, adults having moderate- to high-risk open and laparoscopically assisted abdominal surgery with general

Superiority of perfluorocarbon cardioplegia over blood or crystalloid cardioplegia.

Oxygenated Fluosol-43 cardioplegia (CP), a perfluorocarbon with high oxygen solubility, was compared with crystalloid and oxygenated blood cardioplegia. Potassium in each CP was 25 mEq/l. Thirty perfused rabbit hearts in three groups of 10 hearts each underwent 100 minutes of global ischemia at 20 degrees C, followed by 45 minutes of reperfusion at 37 degrees C. During ischemia, CP was given every 20 minutes. With each CP injection, increases in myocardial oxygen tension were recorded using mass spectrometry and oxygen consumption (MVO2) was calculated. Left ventricular function was assessed b

Balanced crystalloids versus saline in critically ill adults with low plasma bicarbonate: A secondary analysis of a clinical trial.

PURPOSE: We aimed to determine if balanced crystalloids compared with saline improve outcomes in critically ill adults admitted with low plasma bicarbonate. MATERIALS AND METHODS: We performed a secondary analysis of the Isotonic Solutions and Major Adverse Renal Events Trial (SMART). We included patients who presented to the Emergency Department with a first measured plasma bicarbonate less than 20 mmol/L. Among these patients, we compared the effect of balanced crystalloid versus saline on the primary outcome of major adverse kidney events within 30 days (MAKE30), defined as a composite of
