Medical records and follow-up data were reviewed in 297 genetically proven myotonic dystrophy type 2 (DM2) patients. Patients were selected by the criteria of cardiac sudden death before age 45. Sudden death occurred in four patients, three of whom were cardiological asymptomatic, and one with a history of heart failure. Cardiac histopathology showed dilated cardiomyopathy in all, and conduction system fibrosis in two patients. Pathogenetic CCUG ribonuclear inclusions were demonstrable in cardiomyocytes.

Sudden cardiac death is one of the most important causes of mortality in the modern industrialized world. Although it has been described for at least several centuries, we believe in fact that Hippocrates provided a concise, but historically compelling, description of sudden cardiac death in his Aphorisms II, 41: "Those who are subject to frequent and severe fainting attacks without obvious cause die suddenly." This would be the earliest description of sudden cardiac death known. The Aphorism was analyzed in the context of genuine Hippocratic writings as well as ancient Greek culture. The Apho

This report describes an adolescent who presented with ventricular tachycardia (VT) originating from the right ventricular apex, with no apparent underlying cause. Cardiac sarcoidosis was identified only at postmortem examination after sudden death. There must be a high index of suspicion for subtle forms of primary myocardial abnormalities in such cases. Investigation for VT should include specific investigations for cardiac sarcoidosis when no other pathology has been identified and there are persistent conduction abnormalities on resting ECG.

BACKGROUND: Despite routine use of angiotensin-converting enzyme (ACE) inhibitors, beta-blockers and spironolactone in patients with heart failure due to dilated cardiomyopathy (DCM), these patients still have a considerable annual mortality rate of 5-10%. Sudden unexpected death accounts for up to 50% of all deaths and is most often due to rapid ventricular tachycardia or ventricular fibrillation and less often due to bradyarrhythmias or asystole.
THERAPEUTIC OPTIONS: The use of beta-blockers in patients with heart failure has been shown to improve overall mortality considerably. This surviv

One of the most frustrating challenges faced by the forensic pathologist is the inability to determine the cause of death in a young person previously thought healthy. The four steps in the investigation of a sudden death include obtaining the history and scene information, performing a gross and microscopic autopsy, performing appropriate laboratory tests, and making the diagnosis. When examining the heart grossly it is important to preserve the anatomic landmarks, section the coronary arteries closely, and recognize lethal abnormalities such as anomalous origin of the coronary arteries. Only 30-40% of all victims of sudden cardiac death could so far be classified as risk patients during their lifetime. Risk factors for sudden death have little predictive value in an asymptomatic population: for example, the typical risk profile for the presence of coronary heart disease and changes in the surface-ECG at rest and especially in the surface-ECG under stress. Usually, the victims of sudden cardiac death among top performance athletes have been suffering from a heart disease of which they knew nothing beforehand: below 40 years of age, mostly from hypertrophic cardiomyopathy; beyo

Primary mitral valve prolapse is considered a rare cause of sudden cardiac death. Close to 100 cases are documented worldwide. The cause of death seems to be cardiac arrhythmia, so postmortal diagnosis is difficult and depends on severe changes of the mitral leaflets and--sometimes--on anamnesis. Three postmortem cases are presented, where a sudden cardiac death apparently was caused by mitral valve prolapse or its complications.

Sudden cardiac death is a frequent initial manifestation of underlying heart disease. We review the most frequent pathologic cardiovascular findings in victims of sudden cardiac death. We also discuss the role of exercise as a precipitant of sudden cardiac death.

The paper presents current concepts on possible mechanisms of sudden heart death and on systems of neurohormonal heart regulation changes in which may cause sudden heart death. The importance of the main humoral and hormonal changes in possible disorders of electrolyte cardiac balance determining the development of electric instability and ventricle fibrillation underlying most cases of sudden heart death is described. Some morphological changes both in the heart itself and neurohormonal systems regulating it are dealt with.

It is well known that thyroid dysfunction increases the risk of cardiovascular mortality and morbidity. The pleiotropic effect of thyroid hormones has a profound effect on the cardiovascular system, influencing both the formation of a normal cardiac rhythm and rhythm disturbance. A number of research studies have demonstrated correlations between TSH and FT4 levels and significant cardiovascular events. The pathophysiological mechanisms underlying these complex associations are, however, inadequately defined. A system-based examination of the relationship between thyroid homeostasis and cardio

Sudden cardiac death (SCD), the most devastating complication of hypertrophic cardiomyopathy (HCM), is primarily triggered by ventricular tachycardia or fibrillation. Despite advances in knowledge, the mechanisms driving ventricular arrhythmia in HCM remain incompletely understood, stemming from an interplay of multiple pro-arrhythmic factors. Myocyte disarray and myocardial fibrosis form a structural substrate favorable to re-entrant arrhythmias by altering myocardial electrophysiological properties, while cellular abnormalities predominate in patients without evident structural remodeling. Sudden cardiac death (SCD) is responsible for several millions of deaths every year and remains a major health problem. To reduce this burden, diagnosing and identification of high-risk individuals and disease-specific risk stratification are essential. Treatment strategies include treatment of the underlying disease with lifestyle advice and drugs and decisions to implant a primary prevention implantable cardioverter-defibrillator (ICD) and perform ablation of the ventricles and novel treatment modalities such as left cardiac sympathetic denervation in rare specific primary electric diseases

An increased cardiovascular morbidity and mortality, including the risk of sudden cardiac death (SCD), has been shown in patients with rheumatoid arthritis (RA). Abnormalities in autonomic markers such as heart rate variability and ventricular repolarization parameters, such as QTc interval and QT dispersion, have been associated with sudden death in patients with RA. The interplay between these parameters and inflammation that is known to exist with RA is of growing interest. In this article, we review the prevalence and predictors of SCD in patients with RA and describe the potential underly

Sarcoidosis is a systemic granulomatous disease of unknown aetiology, which is characterized by bilateral hilar lymphadenopathy and pulmonary disease. Clinically detected cardiac involvement occurs in 5% of sarcoid patients, although cardiac manifestations are discovered in 25% of the cases at autopsy. Sarcoid heart disease frequently causes atrioventricular block. The authors present the case of a 44-year-old man with bradycardia. On admission, second degree Mobitz II, then third degree atrioventricular block was diagnosed. Coronarography showed normal coronary arteries. 2.5 years following a

This literature review is devoted to the problem of studying sudden cardiac death (SCD) in hypertrophic cardiomyopathy (HCM). It covers prevalence of SCD and its structure among young individuals and trained athletes, contains discussion of relation of SCD to sex, age, and hypertrophic response of myocardium to physical exercise. Modern views on arrhythmogenic substrate underlying SCD in HCM as well as pathophysiologic cascade of life-threatening ventricular tachyarrhythmia are presented in detail. Approaches to identification of individuals at high risk of SCD are described and main and suppl

BACKGROUND: Mitral valve prolapse (MVP) may present with ventricular arrhythmias and sudden cardiac death (SCD) even in the absence of hemodynamic impairment. The structural basis of ventricular electric instability remains elusive.
METHODS AND RESULTS: The cardiac pathology registry of 650 young adults (≤40 years of age) with SCD was reviewed, and cases with MVP as the only cause of SCD were re-examined. Forty-three patients with MVP (26 females; age range, 19-40 years; median, 32 years) were identified (7% of all SCD, 13% of women). Among 12 cases with available ECG, 10 (83%) had inverted T
