Feasibility of implementing a decision aid for advanced cancer patients in a comprehensive cancer centers' outpatient setting: Valuable lessons learned.

BACKGROUND: Decision aids (DAs) have been proposed as tools to empower patients in decision-making. However, implementing DA for decisions in advanced cancer is challenging. This study focuses on perspectives of oncologists and other healthcare providers on the hindering and facilitating factors for implementing a customizable DA. This DA is designed to support discussions about whether patients with advanced, incurable cancers should pursue or forgo further anticancer treatment. METHODS: This qualitative study utilized content analysis following Sandelowski's approach on two different datase

Assessment of data demand for informed-decisions among health facility and department heads in public health facilities of Amhara Region, northwest Ethiopia.

BACKGROUND: Evidence-based decision-making is a foundation of health information systems; however, routine health information is not mostly utilized by decision makers in the Amhara region. Therefore, this study aimed to explore the facility and department heads' perceptions towards the demand for and use of routine health information for decision making. METHODS: A phenomenological qualitative study was done in eight districts of the Amhara region from June 10/2019 to July 30/2019. We obtained written informed consent and recruited 22 key informants purposively. The research team prepared a

Barriers to evidence-based decision making in public health: a national survey of chronic disease practitioners.

OBJECTIVE: Existing knowledge of evidence-based chronic disease prevention is not systematically disseminated or applied. This study investigated state and territorial chronic disease practitioners' self-reported barriers to evidence-based decision making (EBDM). METHODS: In a nationwide survey, participants indicated the extent to which they agreed with statements reflecting four personal and five organizational barriers to EBDM. Responses were measured on a Likert scale from 0 to 10, with higher scores indicating a larger barrier to EBDM. We analyzed mean levels of barriers and calculated a

[Quality end of life in uremic patients: theory and practice].

The rate of fragile elderly patients affected by chronic kidney disease stage 5-5D is rapidly increasing. The decision making process regarding the start and the withdrawal of dialysis is often difficult for all those involved: patients, relatives, nephrologists and renal nurses. Therefore nephrologists and renal nurses are called to rapidly improve their theoretical and practical competence about the end-of-life care. The quality of clinical intervention and management requires a sound expertise in the ethical, legal, organizational and therapeutic aspects, not trivial nor even deductible fro

Speech and language therapists' approaches to communication intervention with children and adults with profound and multiple learning disability.

BACKGROUND: People with profound intellectual and multiple disabilities (PMLD) have communication impairments as one defining characteristic. AIMS: To explore speech and language therapists' (SLTs) decision making in communication interventions for people with PMLD, in terms of the intervention approaches used, the factors informing the decisions to use specific interventions and the extent to which the rationales underpinning these decisions related to the components of evidence based practice (EBP), namely empirical evidence, clinical experience and client/carer views and values. METHODS &

Complex contexts and relationships affect clinical decisions in group therapy.

Clinical errors tend to be underreported even though examining them can provide important training and professional development opportunities. The group therapy context may be prone to clinician errors because of the added complexity within which therapists work and patients receive treatment. We discuss clinical errors that occurred within a group therapy in which a patient for whom group was not appropriate was admitted to the treatment and then was not removed by the clinicians. This was countertherapeutic for both patient and group. Two clinicians were involved: a clinical supervisor who i

Balancing innovation and efficiency in a private-equity-changed world of IVF: the case for sustainable scale.

The scale of IVF centres currently ranges from small, stand-alone units to large networked organizations operating across multiple regions or countries. This diversity in organizational models may influence clinical outcomes, patient experience, professional practice, innovation and long-term sustainability. In parallel with broader healthcare trends, the fertility sector has undergone rapid consolidation over the last decade, prompting renewed debate regarding the relationship between centre size, governance structures, quality of care and efficiency. This opinion paper critically examines th

The paradox of efficiency: institutional interfaces, residuals, and the erosion of innovation drivers in AI-augmented organizations.

As artificial intelligence (AI) becomes deeply embedded in organizational cognition and decision-making, a profound paradox emerges: AI significantly enhances operational efficiency while systematically eroding the generation and retention of innovation drivers. This study introduces the institutional interface as a meso-level analytical lens for examining how specific configurations of AI design choices, human-AI interaction protocols, and organizational norms produce systematic biases. Under the design conditions theorized in this article, these configurations tend to erode innovation driver

Positively deviant networks: what are they and why do we need them?

PURPOSE: This paper aims to report "positively deviant" experiences of three public sector networks seeking to enhance organizational and system level capacities. It is the authors' thesis that the knowledge base concerning the true benefits and pitfalls of networks can be captured and interpreted only through intense, ongoing learning effort embedded in practice on the ground, combined with sustained in-depth observation and collaborative research. DESIGN/METHODOLOGY/APPROACH: The paper describes through case examples why and how different kinds of networks within different jurisdictional co

Decentralized governance in tiered healthcare systems: Supporting equity with blockchain.

Tiered healthcare systems seek to support equity by coordinating care across primary, secondary, and tertiary healthcare delivery organizations, yet fragmented cross-tier coordination and persistent differences in organizational capacity and authority can create avoidable disadvantages. Blockchain can distribute selected operational functions across organizations, but technical decentralization alone does not determine how such coordination is governed or how existing power asymmetries are affected. Drawing on 45 semi-structured interviews with key actor groups in China's tiered healthcare sys

Changes in shock management and vasoactive therapy after transition to a closed intensivist model in a pediatric intensive care unit.

BACKGROUND: The organizational structure of pediatric intensive care units (PICUs) varies widely, and the degree of intensivist involvement may influence decision-making in shock management. However, the impact of transitioning from an open to a closed intensivist-led ICU model on shock management and vasoactive therapy patterns has not been thoroughly evaluated. OBJECTIVES: To evaluate how stepwise increases in intensivist decision-making authority influence hemodynamic management strategies and vasoactive therapy patterns in critically ill children METHODS: We conducted a retrospective stud

Using a stated preference discrete choice experiment to assess societal value from the perspective of decision-makers in Europe. Does it work for rare diseases?

OBJECTIVES: To pilot the feasibility of using a discrete choice experiment (DCE) design to investigate individual preferences from the decision-maker perspective regarding the use of public funding for orphan drugs and generate prior information for future experimental designs. METHODS: A DCE was used on a convenience sample of participants from five European countries (England, France, Germany, Italy and Spain), exploring their preferences in distinct healthcare scenarios involving orphan drugs. A preliminary review of the empirical literature on distributive preferences informed the selecti

Equipping an urban hospital police and security team with Narcan.

Narcan, the nasal-spray form of naloxone, has been approved by the FDA as an easy-to-use version of a drug for saving lives of people who have overdosed on opioids--heroin or prescription painkillers. Communities across the US have been equipping first responders and police with the spray. Now, as the increase in overdose deaths has spread to hospitals, those facilities will have to decide whether their police/security officers should be equipped with. naloxone and trained in its use. In this article, the authors relate their health system's decision, how it was reached, and how it has been im

Accountability in urinary catheter management.

Catheterisation is traditionally considered an area of nursing expertise. Patient involvement in decision-making is important. Although considered autonomous, community nurses appear to encourage a large degree of medical involvement in decision-making related to catheterisation.

Alcohol and managerial performance.

Effects of alcohol intoxication at .05 and .10 breath alcohol concentration upon a number of validated indicators of managerial performance were investigated in a double-blind crossover placebo-controlled simulation design. Managers (N = 48) spent 2 days handling quasi-experimental (partially event-controlled) tasks that permitted realistic managerial decision making under both "normal" and "emergency" conditions. The data showed that speed and frequency of managerial action were affected by alcohol at the .10 but not the .05 level. Strategy and planning deteriorated at both levels. Limited im

Corporate working in health visiting: a concept analysis.

AIM OF THE PAPER: The aim of this paper is to examine individualized health visiting care and compare it to corporate working within a consensual management style. Corporate working has been discussed and used in many different ways since the idea first came to light at the end of the 1980s. Resource management makes it an appealing model, however, analysing how corporate working functions in the practice setting reveals the complexity of this method of service provision. BACKGROUND: This paper is based on a method of practice developed by health visitors in Haywards Heath, West Sussex, who i

Exploring Health Care Professionals' Perceptions Regarding Shared Clinical Decision-Making in Both Acute and Palliative Cancer Care.

UNLABELLED: Developments in cancer care have resulted in improved survival and quality of life. Integration of acute and palliative cancer care is desirable, but not always achieved. Fragmented care is associated with sub-optimal communication and collaboration, resulting in unnecessary care transitions. The aim of this study was to explore how health care professionals, from both acute and palliative care, perceive clinical decision-making when caring for patients undergoing active cancer treatment in parallel with specialized palliative care at home. METHODS: Qualitative explorative design,

The impact of decentralisation on sexual and reproductive health services in Ghana.

This paper analyses the impact of decentralisation on the political organisation, management and provision of sexual and reproductive health services in Ghana. It draws on qualitative research and interviews with key informants from the Ministry of Health, donors, NGOs, regional and district health management teams, local government and community leaders. Within a national reproductive health policy framework, previously disparate family planning, maternal and child health, STI and HIV/AIDS programmes have become more integrated, and donors have pooled or co-ordinated their funding.
