"It's within your own power": shared decision-making to support transitions to buprenorphine.

INTRODUCTION: Buprenorphine is an effective first-line treatment for opioid use disorder (OUD) that substantially reduces morbidity and mortality. For patients using illicitly-manufactured fentanyl (IMF), however, transitioning to buprenorphine can be challenging. Evidence is lacking for how best to make this transition in the outpatient setting. A shared decision-making (SDM) approach has been found to benefit patients with OUD but has not been studied for buprenorphine initiation. We sought to explore participants' experiences with a SDM approach to buprenorphine initiation. METHODS: Partic

Exploring Key Elements of Surgical Shared Decision Making for Providers and Older Adults.

UNLABELLED: Background. Shared decision making (SDM) is an essential component of patient-centered care and especially important in high-risk populations, such as older adults. Although surgery has unique risks and the potential to cause permanent disability, which can affect the patient's quality of life, SDM is underused before surgery to explore these tradeoffs. This study's aim is to characterize the priorities, barriers, resources, and age-specific considerations for SDM in older adults undergoing surgery. Study Design. Five semi-structured focus groups were completed from February 2022 t

Shared Decision-Making With Otolaryngologists and Palliative Care Specialists in Oral Cavity Cancer.

IMPORTANCE: Advanced oral cavity cancer greatly affects vital functions, physical symptoms, psychological well-being, and social interactions. Early integration of palliative care is essential to address these burdens, yet access often remains limited, with many patients referred only near the end of life. OBJECTIVE: To evaluate whether the shared decision-making with otolaryngologists and palliative care specialists (SOP) model is associated with increased palliative care utilization among patients with advanced oral cavity cancer before death. DESIGN, SETTING, AND PARTICIPANTS: This prospe

Shared Decision-Making (SDM) for Female SUI: Current Practice in Three Western Countries.

INTRODUCTION: Different decision-making styles can be used to provide counselling for the multiple reasonable treatment options for patients with stress urinary incontinence (SUI). Shared decision-making (SDM) is currently advocated as the preferred style for preference sensitive decisions, as SDM takes patient preferences into account. This study aimed to map the current decision-making process for SUI in three Western countries. METHODS: We included 124 patients and 18 physicians in a multicentre, prospective study in five hospitals in Canada, the United Kingdom and the Netherlands. We used

[Shared Decision-Making in Patients with Multiple Sclerosis and Neuromyelitis Optica Spectrum Disorder].

In multiple sclerosis and neuromyelitis optica spectrum disorders, several new, highly effective drugs have been developed in recent years, and the prognosis for patients has improved. However, treatment has become more complex, and healthcare professionals should explain the efficacy and safety of drugs to patients and engage in shared decision-making (SDM) that considers the patient's preferences and values. Further studies are required to determine whether SDM is associated with patient outcomes.

Factors Involved in Shared Decision-making Regarding Treatment Selection by Patients With Cancer.

BACKGROUND/AIM: Patients diagnosed with cancer are expected to choose one or more treatment modalities after receiving corresponding explanations of the options. When making these choices, patients consider the effects of treatment and aspects related to their quality of life. These concerns can cause confusion and conflict owing to the complicated information provided by medical caregivers. The objective of the study was to identify perceptions of cancer treatment in patients with cancer and the decision-making factors affecting their treatment choices. PATIENTS AND METHODS: In this observat

Perspectives of residents on shared decision making in medication management: A qualitative study.

OBJECTIVES: Shared decision making is the process in which the person, their representative, and health care professional share information with each other, participate in the decision-making process, and agree on a course of action. At present, very little is known about shared decision making (SDM) in medication management from the perspective of long-term care facility residents. The objective of this study was to identify residents' beliefs, motivation, and aspects of the environment that facilitate or impede SDM. DESIGN: A qualitative study was conducted using face-to-face semi-structure

Shared Decision-Making for Patients Hospitalized with Acute Myocardial Infarction: A Randomized Trial.

OBJECTIVE: Adherence to guideline-recommended medications after acute myocardial infarction (AMI) is suboptimal. Patient fidelity to treatment regimens may be related to their knowledge of the risk of death following AMI, the pros and cons of medications, and to their involvement in treatment decisions. Shared decision-making may improve both patients' knowledge and involvement in treatment decisions. METHODS: In a pilot trial, patients hospitalized with AMI were randomized to the use of the AMI Choice conversation tool or to usual care. AMI Choice includes a pictogram of the patient's estima

Shared decision making for HIV pre-exposure prophylaxis (PrEP) with black transgender women.

Shared decision making is a collaborative process intended to develop a treatment plan that considers both the patient's preferences and the health provider's medical recommendations. It is one approach to reducing healthcare disparities by improving patient-provider communication and subsequent health outcomes. This study examines shared decision making about HIV pre-exposure prophylaxis (PrEP) with Black transgender women in Chicago, Illinois, USA, given high prevalence of HIV and disparities in PrEP use. Black transgender women were recruited online and in-person to participate in semi-stru

Disappointing evaluation of a shared decision-making intervention for residents and orthopaedic surgeons.

OBJECTIVE: To evaluate a shared decision-making (SDM) intervention in orthopaedic hip and knee osteoarthritis care. METHODS: Using a pre- post intervention design study, we tested an intervention, that included a decision aid for patients (ptDA) and a SDM training course for residents in training and orthopaedic surgeons. The theory of planned behaviour was used for intervention development. Primary outcomes included patient reported decisional conflict, SDM, and satisfaction. Secondary outcomes were physicians' attitude and knowledge, and uptake of the ptDA. RESULTS: 317 patients were inclu

Shared Decision Making in Common Chronic Conditions: Impact of a Resident Training Workshop.

PROBLEM: Physicians must be competent in several different kinds of communication skills in order to implement shared decision making; however, these skills are not part of routine medical student education, nor are they formally taught during residency training. INTERVENTION: We developed a 1- and 2-hour workshop curriculum for internal medicine residents to promote shared decision making in treatment decisions for four common chronic conditions: diabetes, depression, hypertension, and hyperlipidemia. The workshops included a written case exercise, a short didactic presentation on shared dec

Decision making in chronic glaucoma--optometrists vs ophthalmologists in a shared care service.

PURPOSE: Shared care schemes have become popular in glaucoma management in the UK over the past 20 years. Published audits of decision making processes by non-ophthalmologists working in such schemes are however rare. Our aim was to audit the appropriateness of optometrists' decision-making when working in an in-house shared care glaucoma scheme. METHODS: A single masked comparison study utilising retrospective consensus decisions by two ophthalmologists compared with 'live' optometrist decisions from three optometrists who reviewed patients with glaucoma, suspect glaucoma and ocular hyperten

Racial Disparities in Shared Decision Making Among Prostate Cancer Patients: A National Study.

Shared decision-making (SDM) plays an important role in patients with prostate cancer due to the complexities of screening and treatment. This study assessed the racial and ethnic disparities in SDM in patients diagnosed with prostate cancer in the United States. This cross-sectional analysis utilized 2014-2021 Medical Expenditure Panel Survey (MEPS) data. Male patients ≥ 18 years old with a diagnosis of prostate cancer were included. The Consumer Assessment of Healthcare Providers and Systems survey was used to operationalize the SDM, with scores categorized as poor (4-8), average (9-11), and
