Decision theoretic analysis of improving epidemic detection.

The potentially catastrophic impact of an epidemic specially these due to bioterrorist attack, makes developing effective detection methods essential for public health. Current detection methods trade off reliability of alarms for early detection of outbreaks. The performance of these methods can be improved by disease-specific modeling techniques that take into account the potential costs and effects of an attack to provide optimal warnings and the cost and effectiveness of interventions. We study this optimization problem in the framework of sequential decision making under uncertainty. Our

On the role of anxiety in decisions under possiblistic uncertainty.

We focus on situations in which we must decide on what time to take an action. The action is not in question it is the time of action. We call these "time for action decisions," a prototypical example being deciding when to leave on a journey. We point out that during this type of decision process, the decision-maker recognizes two groups of forces acting on him: one that pushes him to act now and other that pushes him act later. We note that the strength of these forces depends on the information available about various uncertainties associated with the situation. It also strongly depends upo

Effects of Decision Aids on Decision Knowledge, Conflict, and Satisfaction Among Patients With Cancer: A Systematic Review and Meta-Analysis.

AIM: To examine the effects of decision aids (DAs) on decision knowledge, conflict, and satisfaction among patients with cancer. BACKGROUND: DAs are frequently used to improve the quality of health decision-making across various cancer types (e.g., breast, prostate, and colorectal cancer). However, the impact of DAs on decision knowledge, conflict, and satisfaction remains unclear, and the various delivery methods (e.g., telephone, brochure, mixed delivery methods, and web-based delivery methods) exhibit inconsistent effects. METHODS: We systematically searched eight databases and two search

A formalized shared decision-making process with individualized decision aids for older patients referred for cardiac surgery.

BACKGROUND: Comprehension of risks, benefits and alternative treatment options is poor among patients referred for cardiac surgery interventions. We sought to explore the impact of a formalized shared decision-making (SDM) process on patient comprehension and decisional quality among older patients referred for cardiac surgery. METHODS: We developed and evaluated a paper-based decision aid for cardiac surgery within the context of a prospective SDM design. Surgeons were trained in SDM through a Web-based program. We acted as decisional coaches, going through the decision aids with the patient

Lessons Learned From Shared Decision-Making With Oral Anticoagulants: Viewpoint on Suggestions for the Development of Oral Chemotherapy Decision Aids.

Oral chemotherapy is commonly prescribed, and by using decision aids (DAs), clinicians can facilitate shared decision-making (SDM) to align treatment choices with patient goals and values. Although products exist commercially, little evidence informs the development of DAs targeting the unique challenges of oral chemotherapy. To address this gap in the literature, our objective was to review DAs developed for oral anticoagulation, DA use in oncology, and patient preference surveys to guide the development of DAs for oral chemotherapy. We focused on reviewing SDM, patient preferences, and speci

Effectiveness of a patient decision aid for patients with superficial basal cell carcinoma: A pre- and post-implementation study.

OBJECTIVE: This study evaluates whether using a patient decision aid (PDA) for patients with superficial basal-cell carcinoma (sBCC) results in a decreased decisional conflict level and increased knowledge. METHODS: In a prospective multicentre study, patient groups were included before and after implementation of a PDA. Decisional conflict levels were compared directly after making the treatment decision, measured once as the mean score on the decisional conflict scale (DCS). Higher scores correspond with higher conflict levels (0-100). Secondary outcomes were knowledge on treatment options,

Encounter decision aids to facilitate shared decision-making with women experiencing heavy menstrual bleeding or symptomatic uterine fibroids: A before-after study.

OBJECTIVE: Is the level of shared decision-making (SDM) higher after introduction of a SDM package (including encounter decision aids on treatment options for heavy menstrual bleeding and training for clinicians) than before?. METHODS: This before-after study, performed in OB-GYN practice, compared consultations before and after introduction of a SDM package. The target sample size was 25 patients per group. Women seeking treatment for heavy menstrual bleeding were eligible. After their appointments, patients filled out a three-item patient-reported SDM measure. Treatment discussions were aud

Assessing internet-based information used to aid patient decision-making about surgery for perianal Crohn's fistula.

BACKGROUND: Decision-making in perianal Crohn's fistula (pCD) is preference sensitive. Patients use the internet to access healthcare information. The aim of this study was to assess the online information and patient decision aids relating to surgery for pCD. METHODS: A search of Google™ and the Decision Aids Library Inventory (DALI) was performed using a predefined search strategy. Patient-focussed sources providing information about pCD surgery were included in the analysis. Written health information was assessed using the International Patient Decision Aids Standards (IPDAS) and DISCERN

Risk Analysis of a Fuel Storage Terminal Using HAZOP and FTA.

The size and complexity of industrial chemical plants, together with the nature of the products handled, means that an analysis and control of the risks involved is required. This paper presents a methodology for risk analysis in chemical and allied industries that is based on a combination of HAZard and OPerability analysis (HAZOP) and a quantitative analysis of the most relevant risks through the development of fault trees, fault tree analysis (FTA). Results from FTA allow prioritizing the preventive and corrective measures to minimize the probability of failure. An analysis of a case study

Decision analysis in clinical gastroenterology.

OBJECTIVES: Although the tools of medical decision analysis have been mostly used to address complex issues of healthcare and management policies, an important role remains for decision analysis to play in solving mundane medical problems of daily practice. The present article aims to restate the origins of decision analysis and to demonstrate the utility of decision models in clinical gastroenterology. METHODS: Three clinical scenarios are presented to illustrate the applicability of decision analysis in clinical gastroenterology. The examples are modeled as simple decision trees and phrased

A clinical decision analysis of cryotherapy compared with expectant management for cervical dysplasia.

BACKGROUND: Screening for precursors of cervical cancer with colposcopic examination for women with abnormal Papanicolaou (Pap) smears identifies those with cervical dysplasia. Though the majority of mild dysplasias (CIN I) will regress, many are treated with cryotherapy. METHODS: We used decision analysis to compare immediate cryotherapy with expectant management (following with another Pap smear or colposcopy, with treatment reserved for progression or a duration of 2 years). The decision tree included the possibility of more invasive surgical procedures if the cryotherapy was ineffective o

Ultra-efficient multiple cross displacement amplification-lateral flow biosensor (MCDA-LFB) for serogroup identification of prevalent Neisseria meningitidis.

Meningococcal disease caused by Neisseria meningitidis remains a major global public health concern. Serogroup A, B, C and W135 were the major disease-causing serogroups. It is vital to timely and efficiently detect and differentiate these four serogroups. Herein, we developed multiple cross displacement amplification-lateral flow biosensor (MCDA-LFB) assays targeting ctrA, sacB, siaD, siaD and synG gene respectively for detection and subtyping of four N. meningitidis serogroups. This assay utilizes LFB to detect FITC and biotin-labeled target amplicons produced by MCDA through double antibody

Discriminating neoplastic and normal brain tissues in vitro through Raman spectroscopy: a principal components analysis classification model.

BACKGROUND AND OBJECTIVE: Because of their aggressiveness, brain tumors can lead to death within a short time after diagnosis. Optical techniques such as Raman spectroscopy may be a technique of choice for in situ tumor diagnosis, with potential use in determining tumor margins during surgery because of its ability to identify biochemical changes between normal and tumor brain tissues quickly and without tissue destruction. METHODS: In this work, fragments of brain tumor (glioblastoma, medulloblastoma, and meningioma) and normal tissues (cerebellum and meninges) were obtained from excisional

The use of cognitive aids during emergencies in anesthesia: a review of the literature.

Cognitive aids are prompts designed to help users complete a task or series of tasks. They may take the form of posters, flowcharts, checklists, or even mnemonics. It has been suggested that the use of cognitive aids improves performance and patient outcomes during anesthetic emergencies; however, a systematic assessment of the evidence is lacking. The aim of this literature review was to determine (1) whether cognitive aids improve performance of individuals and teams and (2) whether recommendations can be made for future cognitive aid design, testing, and implementation. Medical, nursing, an

A game theoretical approach to optimal control of dual drug delivery for HIV infection treatment.

For human immunodeficiency virus (HIV) infection treatment, the host immune system and antiviral drugs form a coalition to fight against HIV. Evidence suggests that drug therapy with Highly Active Antiretroviral Therapy can effectively prolong the life of the patient because it can reduce the replication of HIV while, at the same time, protecting the CD4+ T cells. The dosages of different drugs, however, are important in determining the success of the treatment. In addition, drug therapy for HIV is complicated by the fact that viruses can mutate, thereby effectively escaping from the attack of

[Preventing ineffective laparoscopic cholecystectomies].

The number of laparoscopic cholecystectomies in the Netherlands has increased significantly in recent years. However, there is a large variation in practice. This is a sign of inefficient use of cholecystectomy. Cholecystectomy is the treatment of choice in patients with uncomplicated symptomatic gallstones, but 30% of the patients has persistent pain after surgery. In a large prospective multicentre randomised trial, a restrictive strategy was compared to usual care in patients with symptomatic gallstones. The results show suboptimal pain reduction in both groups. There were fewer cholecystec

A Primer on Bayesian Decision Analysis With an Application to a Kidney Transplant Decision.

A clinical decision support system (CDSS) is a computer program, which is designed to assist health care professionals with decision making tasks. A well-developed CDSS weighs the benefits of therapy versus the cost in terms of loss of quality of life and financial loss and recommends the decision that can be expected to provide maximum overall benefit. This article provides an introduction to developing CDSSs using Bayesian networks, such CDSS can help with the often complex decisions involving transplants. First, we review Bayes theorem in the context of medical decision making. Then, we int

User-centered development of a hypospadias decision aid prototype.

INTRODUCTION: Many parents who choose hypospadias repair for their sons experience decisional conflict (DC) and decisional regret (DR). We previously found that parental decision-making about hypospadias surgery is a complex process characterized by cyclic information-seeking to alleviate anxiety and confusion. OBJECTIVE: The objective of this study was to engage parents of hypospadias patients and pediatric providers in the co-design of a decision aid (DA) prototype to facilitate shared decision-making about hypospadias surgery and address DC and DR. METHODS: From August 2018 to January 201

Creation of a Decision Support Tool for Expectant Parents Facing Threatened Periviable Delivery: Application of a User-Centered Design Approach.

BACKGROUND: Shared decision-making (SDM) is optimal in the context of periviable delivery, where the decision to pursue life-support measures or palliation is both preference sensitive and value laden. We sought to develop a decision support tool (DST) prototype to facilitate SDM by utilizing a user-centered design research approach. METHODS: We convened four patient and provider advisory boards with women and their partners who had experienced a surviving or non-surviving periviable delivery, pregnant women who had not experienced a prior preterm birth, and obstetric providers.
