Limitations and late complications of third-generation automatic cardioverter-defibrillators.

BACKGROUND: This study examines the limitations and complex management problems associated with the use of tiered-therapy, implantable cardioverter-defibrillators (ICDs). METHODS AND RESULTS: The study group comprises the first 154 patients undergoing implantation of tiered-therapy ICDs at our institution. Pulse generators from three different manufacturers were used. In 39 patients, a complete nonthoracotomy lead system was used. The perioperative mortality was 1.3%. Of these 154 patients, 37% experienced late postoperative problems. Twenty-one patients required system revision within 36.5 m

Single-incision implantation of cardioverter defibrillators using nonthoracotomy lead systems.

This study describes the placement of a newly designed implantable cardioverter defibrillator in a subpectoral device pocket using the incision for venous access in 16 patients undergoing implantation of an implantable cardioverter defibrillator with a nonthoracotomy lead system. The endocardial lead system consisted of a right atrial/superior vena cava defibrillation spring electrode and a right ventricular bipolar sensing/defibrillation electrode, inserted by cephalic venotomy or by puncturing of the subclavian vein. As a result of intraoperative testing using biphasic shocks the defibrillat

A follow-up study of patients with implantable cardioverter defibrillators.

This study involves 381 patients who received an implantable cardioverter defibrillator (ICD) for management of serious ventricular dysrhythmias and were followed for up to 9 years. The device was effective in preventing sudden cardiac death (SCD), with survival rates of 97% at 3 years and 94% at 5 years. Complication rates were low. Two hundred and twenty-five patients received at least one shock; 2.3 shocks per patient year were received. Shocks were categorized as appropriate, indeterminate, or inappropriate. Definitions of these categories and their occurrence are discussed and the charact

Patient and spouse acceptance and adaptation to implantable cardioverter defibrillators.

Although electrophysiologic devices have been available since 1932 for managing sudden cardiac death, it was not until 1980 that the predecessor to the automatic implantable cardioverter defibrillator was introduced. Subsequently, questions about psychosocial adaptation have prevented wide acceptance of these devices. To study this issue, 69 patients with treatment-resistant ventricular arrhythmias were sent a questionnaire following cardioverter implantation; spouses also received questionnaires. Of these, 42 patients and 38 spouses completed and returned questionnaires. The questionnaire was

A Review of External Defibrillation Strategies.

The most critical intervention for patients suffering cardiac arrest due to ventricular tachyarrhythmias is timely defibrillation. We present a comprehensive review on current external defibrillation strategies, including issues of access to and use of defibrillators, to maximize patient survival after cardiac arrest. Successful defibrillation depends on an electrical shock delivered with sufficient shock strength, vector, and waveform characteristics to a heart with a metabolic state capable of returning to an organized rhythm. Finally, access to and application of automated external defibril

Advances in cardiac pacing and defibrillation.

There have been many evolutionary milestones in cardiac implantable electronic device (CIED) therapy over the past few decades. These advancements have created new  challenges. Right ventricular pacing was the original (late 1950s) breakthrough, however the risk of pacing induced dyssynchrony has become evident. Biventricular pacing provided a viable way to mitigate dyssynchrony, but only benefits certain patients (primarily left bundle branch block and QRS duration ≥150 ms with depressed left ventricular (LV) function). Areas covered: Recent advances have made His Bundle pacing an option that

The Subcutaneous Implantable Cardioverter Defibrillator.

The subcutaneous implantable cardioverter defibrillator (S-ICD) is a subcutaneous alternative to conventional transvenous ICD (TV-ICD) systems, which have previously been shown to treat life-threatening ventricular tachyarrhythmias in cardiac disease patients. A review of the literature reveals that S-ICDs have similar shock efficacy rates for both induced and spontaneous ventricular tachyarrhythmias when compared with TV-ICDs. Furthermore, S-ICDs seem to have a higher specificity for withholding therapy when supraventricular tachycardia is present compared with TV-ICDs. The advantages of the

Implantable cardioverter defibrillator therapy.

Sudden cardiac death (SCD) is a major healthcare problem worldwide. The majority of SCD events occur in patients with clinically recognized heart disease and most episodes result from ventricular tachyarrhythmias. Implantable cardioverter defibrillator (ICD) therapy prevents SCD in specific patient populations. Significant progress in the design and technology has been made since the Food and Drug Administration first approved the ICD in 1985. First-generation ICDs were large, were implanted in the abdomen, required a thoracotomy for placing epicardial defibrillation patches, and were nonprogr

Removal of endocardial defibrillation leads.

Two patients, each with an endocardial defibrillation lead system (Endotak O62), required lead removal; one because of chronic lead infection and the second because of spurious shocks caused by lead insulation damage. Neither lead could be removed by simple traction. The defective lead was removed by a combination of catheterization techniques including a steerable ablation catheter and traction, both under general anesthesia. The lead with the insulation defect was rapidly removed with a locking stylet, suggesting that endocardial lead defibrillating leads can be removed similarly to pacemake

[New aspects of defibrillator therapy].

The implantable cardioverter defibrillator is currently a therapy of first choice in patients with malignant therapy refractory ventricular arrhythmias. The occurrence of malignant ventricular tachycardia cannot be suppressed by the defibrillator but is treated using antitachycardia pacing, cardioversion or defibrillation. During recent years, electrodes, defibrillation shockforms and device size were continuously optimized. The development of transvenous lead systems resulted in significant reduction of perioperative mortality and morbidity. With the availability of biphasic shockforms and si

[Atrial defibrillator: dream or reality?].

The experience acquired with the use of automatic ventricular cardiovertor-defibrillator led to the idea of developing an atrial device capable of automatically detecting and reducing atrial fibrillation. The large group of patients with paroxysmal atrial fibrillation not responding to pharmacological therapy makes this a particularly relevant question. The first problem is to define the need of this type of device. There is only one group of patients with resistant and/or poorly tolerated paroxysmal atrial fibrillation in whom non-pharmacological therapy is considered, for example ablation of

Is there an effect of amiodarone on the defibrillation threshold?

The interaction between amiodarone and the defibrillation threshold (DFT) is still controversial. We present a case with dilated cardiomyopathy and recurrent sustained monomorphic ventricular tachycardia who received an automatic implantable cardioverter defibrillator (AICD) while under long-term amiodarone treatment. AICD implantation was performed without thoracotomy. The transvenous lead was inserted via a left subclavian vein puncture and the patch was placed on the lateral chest wall, submuscularly. At the time of implantation a 35J shock was not successful in converting ventricular fibri

When is a wearable defibrillator indicated?

INTRODUCTION: Wearable cardioverter-defibrillator (WCD) is a novel tool that may be of interest in situations with atransient risk of sudden cardiac death. It offers a temporary and easy to remove protection against malignant ventricular arrhythmias. In this review, the authors describe evidence in literature and different international guidelines and consensus. AREAS COVERED: The authors searched PubMed, Cochrane Central Register of controlled trials, and Google Scholar for relevant studies and comments. EXPERT OPINION: If a WCD is indicated, the observance must be as perfect as possible. T

[Travelling with a pacemaker or implanted defibrillator].

There are no guidelines for patients travelling with implanted pacemakers or defibrillators. Only few publications deal with specific problems that this patient group might face. In this article different aspects of travelling with implanted electric devices are summarized. Patients with pacemakers and implanted defibrillators have nearly no limits when travelling. An exception to that rule is scuba diving, which mostly is limited because of the device. In general it is the underlying heart disease or arrhythmia that limits patients' travel activities. It is reasonable to travel after implanta

Imaging fibrillation/defibrillation in a dish.

BACKGROUND: Sheets of cultured cardiac cells constitute an emerging experimental model of cardiac tissue electrophysiology and arrhythmia. It has been used to study fundamental properties of reentrant (spiral) waves and electric field interactions with tissue structure and thus constitutes a valuable model for investigations of fibrillation and defibrillation. METHODS: Optical mapping can be used to visualize action potential propagation or calcium waves in confluent monolayers of neonatal rat ventricular cells. Spiral waves can be initiated by burst pacing and terminated by an electric field
