Incidence and clinical predictors of low defibrillation safety margin at time of implantable defibrillator implantation.

BACKGROUND: Determination of the defibrillation safety margin (DSM) is the most common method of testing device effectiveness at the time of implantation of implantable cardioverter defibrillator (ICD) or cardiac resynchronization therapy defibrillator (CRTD). Low DSM remains a problem in clinical practice. OBJECTIVE: The purpose of this study is to ascertain the incidence and clinical predictors of low DSM and the treatment strategies for low DSM in ICD or CRTD recipients. METHODS: Selected ICD or CRTD recipients from January 2006 to May 2008 who underwent DSM test at the time of implantati

Implantable cardioverter defibrillator implantation in children in The Netherlands.

UNLABELLED: To evaluate the indications, underlying cardiac disorders, efficacy and complications involved with implantable cardioverter-defibrillators (ICDs) in paediatric patients in The Netherlands, the records of all patients aged 18 years or younger who underwent ICD placement were reviewed retrospectively. Between January 1995 and September 2002, 23 patients (11 males, 12 females; median age 12 years, range 6 months to 16 years) underwent ICD implantation. The ICD was implanted for aborted sudden cardiac death ( n = 14), syncope ( n = 5) or for primary prevention of sudden cardiac death

Electrocardiographic parameters and outcomes in patients with cardiac amyloidosis and implantable cardioverter defibrillators.

BACKGROUND: Cardiac amyloidosis (CA) is an infiltrative cardiomyopathy that can lead to various arrhythmias. Electrocardiogram (ECG) abnormalities have been reported in CA, though there is limited data on the prognostic significance of these findings. METHODS: Between 1/1997 and 12/2019, we identified high arrhythmic-risk patients with CA who had clinically been treated with an implantable cardioverter defibrillator (ICD) to prevent sudden cardiac death from our multi-center practice. We studied baseline characteristics, comorbidities, echocardiograms, and ECG parameters. Regression models we

Long-term outcomes of subcutaneous implantable cardioverter defibrillators: the French nationwide HONEST study.

BACKGROUND AND AIMS: Although use of the subcutaneous implantable cardioverter defibrillator (S-ICD) is increasing, evidence from industry-independent and unselected populations remains limited. METHODS: HONEST is a ongoing nationwide academic observational study enrolling 98.2% of patients implanted with an S-ICD across France (2012-2019). Five-year clinical endpoints were centrally adjudicated. RESULTS: Overall, 4924 patients were enrolled (mean age 49.9 ± 15 years, 76.7% male, 63.0% for primary prevention). Implants used general anaesthesia (78.9%), and defibrillation testing (82.6%). Per

Quality of Life, Mood Disturbance, and Sexual Health in Implantable Cardioverter Defibrillators Recipients.

Implantable Cardioverter Defibrillators (ICDs) are essential for managing life-threatening arrhythmias but can impact patients' quality of life (QoL), mood, and sexual health. Although QoL may improve shortly after implantation, factors such as age, psychological state, and ICD shocks can influence long-term outcomes. Anxiety, depression, and fears around physical and sexual activity are common among ICD patients, yet the depth of these effects remains underexplored. This cross-sectional, correlational study examined associations between QoL, mood, and sexual health in 30 adult ICD patients (a

All Implantable Cardioverter Defibrillators Are Not Programmed Equal: A Case Report of Pseudomalfunction.

As the population ages, the need for anesthesiologists to manage complicated implantable cardioverter defibrillators (ICDs) has increased. We present a case of hemodynamically unstable ventricular tachycardia occurring during a transcatheter aortic valve replacement where magnet removal from the ICD did not result in the expected tachytherapies. When managing patients with an ICD, we emphasize preoperative placement of external defibrillator pads and prompt use of external shocks rather than relying on the ICD to manage hemodynamically unstable tachyarrhythmias.

Effects of hyperbaric exposure on mechanical and electronic parameters of implantable cardioverter-defibrillators.

AIMS: Hyperbaric oxygen therapy (HBOT) is the standard adjuvant treatment for life-threatening or disabling pathologies. Currently, mechanical and electronic variations of implantable cardioverter-defibrillators (ICD) in hyperbaric conditions have not been evaluated. As a result, many patients eligible for HBOT but ICD recipients cannot undergo this therapy, even in emergency situations. METHODS AND RESULTS: Twenty-two explanted ICD of various brands and models were randomized in two groups: single hyperbaric exposure at an absolute pressure of 4000 hPa and 30 iterative hyperbaric exposures a

Posttraumatic stress disorder in pediatric patients with implantable cardioverter-defibrillators and their parents.

BACKGROUND: An implantable cardioverter-defibrillator (ICD) in the pediatric patient (and the precipitating events that led to ICD placement) can be traumatic for patients and their families and may lead to posttraumatic stress disorder (PTSD). OBJECTIVES: This study aimed to estimate the prevalence of PTSD in pediatric patients with an ICD and their parents and identify the factors associated with PTSD incidence. METHODS: Pediatric participants with an ICD aged 8-21 years and parents of children aged 0-21 years completed surveys that included demographic characteristics and PTSD measures. P

Mortality Risk Increases With Clustered Ventricular Arrhythmias in Patients With Implantable Cardioverter-Defibrillators.

OBJECTIVES: This study sought to examine the adverse prognosis associated with ventricular arrhythmia clusters that falls outside the current electrical storm definition. BACKGROUND: Electrical storm is most frequently defined as a cluster of ≥3 episodes of ventricular arrhythmia (VA) in a 24-h period. This definition has been associated with adverse cardiovascular outcomes and mortality, but the effect of lesser and greater clustering of arrhythmias has not been described. METHODS: Among all patients in the Resynchronization in Ambulatory Heart Failure trial, 14,515 implantable cardioverter

[Prevention of sudden cardiac death and implantable cardioverter defibrillators: Understanding the competitive risk!].

Sudden cardiac death represents a major public health issue, with up to 50% of the cardiovascular mortality. Coronary artery disease and dilated cardiomyopathy both represent almost 90% of sudden cardiac death burden. Primary prevention using implantable cardioverter defibrillator relies, in this population, on the left ventricle ejection fraction simple measurement. In this paper, we aim to discuss in which extent a better understanding of competing risk situation may help for a better patient selection and eventually for optimizing primary prevention using implantable cardioverter defibrilla

Eligibility for subcutaneous implantable cardioverter defibrillators in the adult congenital heart disease population.

BACKGROUND: Patients with adult congenital heart disease (ACHD) have an increased risk of arrhythmic and sudden cardiac death. The subcutaneous implantable cardioverter defibrillator (S-ICD) provides a potentially safer alternative to transvenous ICDs in ACHD. Suitability for S-ICD depends on the surface electrocardiogram (ECG), which is often abnormal in ACHD patients. This study investigates the proportion of ACHD patients who meet the screening criteria for S-ICD implantation. METHODS: A standard screening ECG was performed in 102 patients with complex ACHD (Tetralogy of Fallot, Fontan cir
