An Unusual Case of Delayed Midazolam Anaphylaxis and a Review of the Current Literature.

Midazolam is a commonly used, well-tolerated, anxiolytic, sedative, anesthesia induction agent, and an adjunct for procedural sedation that is used widely in the emergency department. The ability to administer midazolam via multiple routes, including intranasal, makes it a particularly common choice for use in children. Intranasal administration is safe, easy, and well tolerated and has been shown to be an effective method of obtaining anxiolysis and/or sedation. Adverse drug reactions, including allergic reactions, can occur with any medication. However, anaphylaxis is an uncommon phenomenon

Effectiveness of preoperative intranasal dexmedetomidine, compared with oral midazolam, for the prevention of emergence delirium in the pediatric patient undergoing general anesthesia: a systematic review.

BACKGROUND: Emergence delirium is defined as a cognitive disturbance during emergence from general anesthesia resulting in hallucinations, delusions and confusion manifested by agitation, restlessness, involuntary physical movement and extreme flailing in bed. Postoperative emergence delirium develops in 12% to 18% of all children undergoing general anesthesia for surgery. This post-anesthetic phenomenon changes cognitive and psychomotor behavior, and puts pediatric patients and health care personnel at risk of injury. A newer drug, dexmedetomidine, is a selective alpha-2 agonist, which works

Factors associated with prolonged anesthesia recovery following laparoscopic bariatric surgery: a retrospective analysis.

BACKGROUND: Phase I postanesthesia recovery is often prolonged after laparoscopic bariatric surgery. We hypothesized that postoperative respiratory depression is a major contributor to this delayed recovery. METHODS: Medical records of all patients who had a laparoscopic bariatric surgical operation from January 1, 2009, to December 31, 2012, were reviewed for clinical, anesthetic, and postanesthesia variables. Recoveries were defined as discharge from the recovery room in ≤90 min and in >90 min (prolonged postanesthesia recovery). We compared characteristics of patients without prolonged rec

Esophageal atresia.

Esophageal anomalies present a challenge to the pediatric surgeon and demand close attention and care from the nursing staff as well as the surgical staff. We have encountered a survival rate of 96 per cent for all infants undergoing operation for esophageal atresia. An emergency gastrostomy is recommended as an immediate procedure when the diagnosis is established. In many instances, it can be performed with local anesthesia if the infant is particularly small or has developed aspiration pneumonia. The definitive operation is delayed until the infant is in optimum condition to permit a genera

A Self-Inserted Foreign Body in the Urinary Bladder and Urethra.

Foreign objects in the lower genitourinary system are a rare urological emergency often associated with self-eroticism, drug intoxication, or psychiatric illness. In addition to clinical examination, multiple imaging modalities such as X-ray, ultrasound, computed tomography, and magnetic resonance imaging have been used for the diagnosis of foreign bodies. Surgical exploration and endoscopic extraction are the main approaches to the treatment. Here, we present the case of a 37-year-old male who presented to the emergency department with penile and urethral pain caused by an electrical wire ins

Near-Fatal Hemodynamic Collapse during Recovery from Staged Bladder Tumor Resection in a Patient with Giant Left Atrial Myxoma: A Case for Immediate Sequential Surgery.

INTRODUCTION: Surgical management of a large, obstructive left atrial (LA) myxoma coexisting with an active bleeding source presents a significant clinical dilemma. The systemic heparinization required for cardiopulmonary bypass (CPB) carries a high risk of exacerbating hemorrhage, whereas delaying cardiac surgery leaves the patient vulnerable to fatal myxoma-related complications. CASE PRESENTATION: A 77-year-old woman presented with gross hematuria from an 8-cm bladder tumor. Preoperative evaluation revealed a giant (74 × 40 mm) LA myxoma prolapsing through the mitral valve, causing functio

Fatal spontaneous bilateral extradural hematoma with swirl sign in a child with sickle cell disease: lessons from delayed surgical access at a tertiary center.

BACKGROUND: Spontaneous extradural hematoma (EDH) is a rare neurosurgical complication of sickle cell disease (SCD), with bilateral pediatric presentations rarely reported. The "swirl sign" on non-contrast CT, a marker of active bleeding and independent predictor of poor prognosis in traumatic EDH and in spontaneous intracerebral hemorrhage, has never been described in spontaneous SCD-related EDH. In low-resource settings, equitable and timely access to emergency neurosurgery remains a critical and modifiable determinant of outcome. CASE DESCRIPTION: A 13-year-old girl with homozygous SCD (Hb

From labor to surgery: expanding the role of programmed intermittent epidural bolus beyond obstetric.

Epidural analgesia has been effective for postoperative pain control, but its use has declined owing to adverse effects, such as motor blockade and delayed recovery. However, in obstetric anesthesia, it remains the gold standard of care, especially with the development of programmed intermittent epidural bolus (PIEB), which delivers boluses of local anesthetic at fixed intervals to improve drug spread and reduce motor blockade. Clinical studies on labor analgesia have demonstrated that PIEB offers superior analgesia, lower anesthetic consumption, and higher maternal satisfaction than continuou

Balloon control for ruptured AAAs: when and when not to use?

Emergency endovascular aneurysm repair (eEVAR) for ruptured abdominal aortic aneurysm (rAAA) must be carried out instantly. This requires firm logistics that include the ability to cross-clamp the aorta without delay. The present article focuses on the technique of balloon control of the aorta in eEVAR with aspects on indications as well as the organization of this type of vascular service. Transfemoral insertion of the occlusion balloon under local anesthesia is advocated and described. The use of dual balloons shortens the time of visceral ischemia without necessitating repeat declamping unt

Association of prehospital invasive blood pressure measurement and treatment times of intubated patients with suspected stroke - a retrospective study.

BACKGROUND: Invasive blood pressure measurement is commonly used in in-hospital patients with stroke requiring general anesthesia, but is much less established in the prehospital setting. While it allows for more precise blood pressure management, it might also lead to prehospital treatment delays. Therefore, this study aims to evaluate the potential impact of prehospital invasive blood pressure measurement on treatment times. METHODS: Adult patients (≥ 18 years) with suspected stroke (both ischemic and hemorrhagic) and prehospital induction of emergency anesthesia by physicians admitted to t

Ultrasound guiding the rapid diagnosis and treatment of perioperative pneumothorax: A case report.

BACKGROUND: Pneumothorax is one of the most common causes of acute dyspnea. In patients under general anesthesia, the symptoms may not be obvious, which may delay diagnosis and treatment. Computed tomography is the gold standard for the diagnosis of pneumothorax, but is not suitable for rapid diagnosis of this complication. In contrast, lung ultrasonography can provide rapid diagnosis and treatment of pneumothorax. CASE SUMMARY: The patient was a 53-year-old man admitted for rupture of the spleen caused by an accidental fall and emergency splenectomy was planned. Anesthesia was induced, and t

Reduction of Nonoperative Time Using the Induction Room, Parallel Processing, and Sugammadex: A Randomized Clinical Trial.

BACKGROUND: An important variable in the operating room is the nonoperative time (NOT), the time between skin closure on a previous case and skin incision on the following case. Mismanagement of NOT can result in significant financial losses and delays in the operating room (OR) schedule, which can negatively impact efficiency and patient, surgeon, and staff satisfaction. NOT includes general anesthesia induction time (IT), emergence time (ET), and turnover time (TOT), and can be calculated by adding the 3 components. OR efficiency can be increased by applying parallel processing for general a
