The Surgical Apgar Score is Associated with Delayed Graft Function after Deceased-Donor Kidney Transplantation: A Retrospective Cohort.

BACKGROUND: Delayed graft function (DGF) remains a frequent complication after deceased-donor kidney transplantation and is associated with adverse graft outcomes. The surgical Apgar score is a simple 10-point intraoperative composite score based on lowest heart rate, lowest mean arterial pressure and estimated blood loss, but its association with DGF remains unclear. METHODS: This monocentric retrospective study included all consecutive adult recipients of a deceased-donor kidney transplant performed between 2016 and 2021. A low surgical Apgar score was defined as ≤7 and DGF as the need for

Changes by Era in Risk Factors and Outcomes Among Deceased Donor Kidney Transplant Recipients With Delayed Graft Function.

INTRODUCTION: There are no effective therapeutic agents for preventing or treating delayed graft function (DGF) among deceased donor kidney transplant recipients (DDKTRs). Donor and recipient factors are important to predicting DGF and associated outcomes, which we hypothesize differed over time. METHODS: DDKTRs were stratified by transplant year into four eras-E1 (2000-2005), E2 (2006-2011), E3 (2012-2017), and E4 (2018-2021). We analyzed risk factors for DGF, along with one-year uncensored graft failure (UCGF), death-censored graft failure (DCGF), death with a functioning graft (DWFG), and

Lymphocyte depletion and risk of acute rejection in renal transplant recipients at increased risk for delayed graft function.

Delayed graft function (DGF) is a risk factor for acute rejection (AR) in renal transplant recipients, and KDIGO guidelines suggest use of lymphocyte-depletion induction when DGF is anticipated. We analyzed the United Network for Organ Sharing/Organ Procurement and Transplantation Network (UNOS/OPTN) database to assess the impact of induction immunosuppression on the risk of AR in deceased kidney recipients based on pretransplant risk of DGF using a validated model. Recipients were categorized into 4 groups based upon the induction immunosuppression: (1) Rabbit anti-thymocyte globulin (rATG);

The prognostic value of the furosemide stress test in predicting delayed graft function following deceased donor kidney transplantation.

OBJECTIVES AND METHODS: The Furosemide Stress Test (FST) is a novel dynamic assessment of tubular function that has been shown in preliminary studies to predict patients who will progress to advanced stage acute kidney injury, including those who receive renal replacement therapy (RRT). The aim of this study is to investigate if the urinary response to a single intraoperative dose of intravenous furosemide predicts delayed graft function (DGF) in patients undergoing deceased donor kidney transplant. RESULTS: On an adjusted multiple logistic regression, a single 100 mg dose of intraoperative f

Risk factors and outcomes of delayed graft function in renal transplant recipients receiving a steroid sparing immunosuppression protocol.

AIM: To analyse the risk factors and outcomes of delayed graft function (DGF) in patients receiving a steroid sparing protocol. METHODS: Four hundred and twenty-seven recipients of deceased donor kidney transplants were studied of which 135 (31.6%) experienced DGF. All patients received monoclonal antibody induction with a tacrolimus based, steroid sparing immunosuppression protocol. RESULTS: Five year patient survival was 87.2% and 94.9% in the DGF and primary graft function (PGF) group respectively, P = 0.047. Allograft survival was 77.9% and 90.2% in the DGF and PGF group respectively, P

The risk of allograft failure and the survival benefit of kidney transplantation are complicated by delayed graft function.

Concern about the long-term impact of delayed graft function (DGF) may limit the use of high-risk organs for kidney transplantation. To understand this better, we analyzed 29,598 mate kidney transplants from the same deceased donor where only 1 transplant developed DGF. The DGF associated risk of graft failure was greatest in the first posttransplant year, and in patients with concomitant acute rejection (hazard ratio: 8.22, 95% confidence interval: 4.76-14.21). In contrast, the DGF-associated risk of graft failure after the first posttransplant year in patients without acute rejection was far

Combined Thymoglobulin and Interleukin-2 Receptor Antagonist Induction Therapy in Kidney Transplantation Is Associated With Worse Delayed Graft Function, Acute Rejection, Graft Loss, and Mortality.

INTRODUCTION: Single-agent induction with either Thymoglobulin (ATG), alemtuzumab, or interleukin-2 receptor antibodies (IL2Ra) is a standard of immunosuppression during kidney transplantation (KTX). Dual-induction therapy (DIT) with ATG + IL2Ra is occasionally administered as part of a center-specific protocol or salvage following failed ATG or IL2Ra induction. We report the outcomes of single-agent regimens versus DIT. METHODS: Analysis of the UNOS database from January 1, 2000, to June 30, 2024, focused on KTX recipients induced with ATG, alemtuzumab, IL2Ra, or DIT. Recipient and donor cha

Integrated prediction of delayed graft function after kidney transplantation using the renal artery resistive index and pretransplant hematological parameters.

Delayed graft function (DGF) remains a frequent early complication after kidney transplantation and is associated with prolonged hospitalization and inferior graft outcomes. This retrospective cohort study evaluated whether the renal artery resistive index (RI) combined with pretransplant hematological parameters could improve DGF prediction. Adult recipients who underwent kidney transplantation between January 2022 and December 2024 at Shulan (Hangzhou) Hospital were screened; 280 patients were included and categorized into a DGF group (n = 88) or a non-DGF group (n = 192) according to the re

Development and validation of a multidimensional machine learning model for predicting delayed graft function in deceased donor kidney transplantation.

PURPOSE: Delayed graft function (DGF) remains a significant complication following deceased donor kidney transplantation. This study aimed to develop and validate a multidimensional machine learning model for predicting DGF by integrating clinical data, machine perfusion parameters, donor scores, and histopathological scores. METHODS: A retrospective analysis was conducted on 961 deceased donor kidney transplant recipients from January 2019 to December 2021. The dataset was stratified by the target variable and randomly divided into training (80%) and independent testing (20%) cohorts. Fiftee

Recipient-driven and donor-driven determinants of delayed graft function in paired kidney transplantation: evidence from a Chinese cohort.

BACKGROUND: Delayed graft function (DGF) is a frequent complication of kidney transplantations. When both kidneys from a single deceased donor are transplanted into two recipients, DGF outcomes may be discordant, affecting only one recipient, or concordant, affecting both recipients. The factors underlying these within-donor differences, particularly in the Chinese population, remain poorly understood. METHODS: We retrospectively analyzed 356 deceased-donor kidney transplants performed in our hospital between 2018 and 2022. We first examined recipient-specific factors associated with discorda

Exposure to renin-angiotensin system inhibitors before kidney transplantation is associated with a decreased risk of delayed graft function.

INTRODUCTION: Animal models suggest a protective role of angiotensin-converting enzyme inhibitors (ACEi) and angiotensin-II receptor blockers (ARBs) in reducing renal and cardiac ischemia-reperfusion injury. Our aim was to determine the association between pre-transplant ACEi/ARBs use and the occurrence of delayed graft function (DGF) in patients who received a kidney transplantation from a deceased donor. METHODS: Consecutive recipients between 2008 and 2021 in 2 Canadian university-affiliated centers were included in this retrospective cohort study. The main outcome was the occurrence of DG

Assessment of delayed graft function using susceptibility-weighted imaging in the early period after kidney transplantation: a feasibility study.

PURPOSE: This study aimed to explore the feasibility of susceptibility-weighted imaging (SWI) for evaluating delayed graft function (DGF) during the early posttransplantation period. METHODS: Sixty-nine recipients who accepted allograft renal transplantation underwent SWI during the second posttransplantation week. Renal allograft function was estimated via the glomerular filtration rate. Recipients with and without DGF were identified. For each transplanted kidney, the presence of abnormal signal intensity lesions (ASILs), excluding benign lesions, on SWI was assessed. Renal allograft functi

Early Focal Segmental Glomerulosclerosis as a Cause of Delayed Graft Function With Combined Acute Antibody Rejection: A Case Report.

Focal segmental glomerulosclerosis (FSGS) is the most common form of post-transplant glomerulonephritis. We describe a case where a biopsy proved that early recurrence of FSGS on postoperative day 1 was the cause of delayed graft function. A 39-year-old man, on hemodialysis for 15 years due to polycystic kidney disease, received a cadaveric renal transplantation. On postoperative day 1, his hourly urine output decreased from 700-800 mL to 50 mL. The graft biopsy showed a mild acute kidney injury confusing nephrotic syndrome. On postoperative day 45, his creatinine level increased to 3.02 mg/dL

Outpatient Management of Delayed Graft Function Is Associated With Reduced Length of Stay Without an Increase in Adverse Events.

Delayed graft function (DGF) is a common and costly complication of kidney transplantation. In July 2011, we established a multidisciplinary DGF clinic managed by nurse practitioners to facilitate early discharge and intensive management of DGF in the outpatient setting. We compared length of stay, 30-day readmission, acute rejection, and patient/graft survival in 697 consecutive deceased donor kidney transplantations performed between July 2009 and July 2014. Patients were divided into three groups: no DGF (n = 487), DGF before implementation of the DGF clinic (n = 118), and DGF clinic (n = 9

Pre-Transplant Expression of CCR-2 in Kidney Transplant Recipients Is Associated With the Development of Delayed Graft Function.

BACKGROUND: Delayed graft function (DGF) leads to a reduced graft survival. Donors' features have been always considered as key pathogenic factors in this setting. The aim of our study was to evaluate the recipients' characteristics in the development of DGF. METHODS: We enrolled 932 kidney graft recipients from 466 donors; 226 recipients experienced DGF. In 290 donors, both recipients presented with early graft function (EGF, group A), in 50 both recipients experienced DGF (group B), and in 126 one recipient presented with DGF and the other with EGF (group C). In group C, we selected 7 coupl

In kidney recipients from the same deceased donor, discordance in delayed graft function is associated with the worst outcomes.

INTRODUCTION: Delayed graft function (DGF) is a common complication among deceased donor kidney transplant recipients (DDKTs) and is associated with worse outcomes. The effect on outcomes of concordance versus discordance in DGF between two different recipients of kidneys from the same donor is largely unknown. METHODS: We reviewed all adult DDKTs for which both kidneys were transplanted to two different recipients at our center between 2014-2019. DDKTs were divided into four groups based on the DGF status: concordance no DGF (cc-no-DGF); discordance no DGF(dd-no-DGF); discordance DGF (dd-DGF

Predictive Values of Urinary Interleukin 18 and Neutrophil Gelatinase-Associated Lipocalin for Delayed Graft Function Diagnosis in Kidney Transplantation.

BACKGROUND: Delayed graft function is a main complication after deceased donor kidney transplantation that adversely affects graft outcome. Difficulties in prediction and early detection of delayed graft function have hindered the ability to perform proper therapeutic interventions. We investigated whether measuring urinary interleukin 18 and neutrophil gelatinase-associated lipocalin as markers of ischemia reperfusion injury could predict delayed graft function in deceased donor kidney transplant patients. METHODS: We studied 69 patients undergoing kidney transplantation from deceased donor
