Questionnaire assessment of usual practice in mood and cognitive assessment in Scottish stroke units.

PURPOSE: National and International guidelines recommend cognition and mood assessment for all stroke survivors. However, there is no consensus on preferred screening tool or method of assessment. We aimed to describe clinical practice in cognitive and mood assessment across Scottish stroke services. METHOD: We used a questionnaire based survey. After local piloting, we distributed the questionnaire using mixed methodologies (online and paper) across all Stroke Managed Clinical Networks in Scotland. We also distributed the questionnaire to specialist societies representing stroke physicians,

User Requirements for Comanaged Digital Health and Care: Review.

BACKGROUND: The sustainability of health and social care has led to an imperative to shift the balance of care to communities and support person-centered, integrated, preventive, comanaged, and sustainable care. The digital tool set can support this shift; however, it must extend beyond a clinical focus to include broader personal, social, and environmental needs, experiences, and outcomes. The existing digital health and care design and user requirements literature focuses mainly on specific digital products or design methods. There is little whole-system or whole-of-life consideration, which

Governing Generative Artificial Intelligence in Anatomy Education: A Proposed Risk-Tiered Framework for Donor-Derived Material, Anatomical Fidelity, and Assessment.

Generative artificial intelligence (GenAI) is now routinely used in anatomy education to draft explanations, generate and interpret images, produce assessment items, and support feedback. Broad responsible-AI principles are well established, but they do not resolve the questions anatomy educators actually face: whether donor-derived material may be entered into an external system, what counts as adequate verification of a generated anatomical structure, and which assessment decisions must remain human. Anatomy faculty themselves report unclear governance in these areas and ask for discipline-s

Intellectual disability content within tertiary medical curriculum: how is it taught and by whom?

BACKGROUND: Individuals with intellectual disability experience higher rates of physical and mental health conditions compared with the general population, yet have inequitable access to health care services. Improving the workplace capacity of medical professionals to meet the needs of this population is one way to reduce barriers to care and improve health outcomes. Using diverse pedagogy appropriate to learning outcomes to teach medical students about intellectual disability is a necessary step in improving future workplace capacity. However, there is a lack of research into how, and by who

Implementation phase of a multicentre prehospital telemedicine system to support paramedics: feasibility and possible limitations.

BACKGROUND: Legal regulations often limit the medical care that paramedics can provide. Telemedical solutions could overcome these limitations by remotely providing expert support. Therefore, a mobile telemedicine system to support paramedics was developed. During the implementation phase of this system in four German emergency medical services (EMS), the feasibility and possible limitations of this system were evaluated. METHODS: After obtaining ethical approval and providing a structured training program for all medical professionals, the system was implemented on three paramedic-staffed am

Developing a typology of proximity healthcare interventions and system-level characteristics in Denmark: an exploratory qualitative interview study.

OBJECTIVES: Proximity healthcare interventions aim to provide or support health services geographically close to, or in, citizens' own homes, involving the actors needed to deliver appropriate care across sectors. However, the field remains conceptually ambiguous, and systematic knowledge of how such interventions are organised is limited. This study therefore aims to develop an empirically grounded typology of proximity healthcare interventions in Denmark and explore system-level characteristics that characterise their development and organisation. DESIGN: Exploratory qualitative study based

Job satisfaction among medical assistants in German general practice: a qualitative study of social, structural and personal factors.

BACKGROUND: Medical assistants (MAs) are pivotal for access, coordination, and continuity in German primary care, yet workforce shortages and limited attention in research and policy persist. Prior studies quantify dissatisfaction and attrition drivers but offer little insight into how MAs who remain in practice experience job satisfaction. This study explores the social, personal and structural factors shaping MA job satisfaction in everyday work. METHODS: We conducted 14 semi-structured telephone interviews with MAs from German general practices (November-December 2023), recruited via the r

Developing a response to family violence in primary health care: the New Zealand experience.

BACKGROUND: Despite primary health care being recognised as an ideal setting to effectively respond to those experiencing family violence, responses are not widely integrated as part of routine health care. A lack of evidence testing models and approaches for health sector integration, alongside challenges of transferability and sustainability, means the best approach in responding to family violence is still unknown. The Primary Health Care Family Violence Responsiveness Evaluation Tool was developed as a guide to implement a formal systems-led response to family violence within New Zealand p

Tracheostomy in COVID-19 acute respiratory distress syndrome patients and follow-up: A parisian bicentric retrospective cohort.

BACKGROUND: Tracheostomy has been proposed as an option to help organize the healthcare system to face the unprecedented number of patients hospitalized for a COVID-19-related acute respiratory distress syndrome (ARDS) in intensive care units (ICU). It is, however, considered a particularly high-risk procedure for contamination. This paper aims to provide our experience in performing tracheostomies on COVID-19 critically ill patients during the pandemic and its long-term local complications. METHODS: We performed a retrospective analysis of prospectively collected data of patients tracheostom

Antibiotic resistance--action to promote new technologies: report of an EU Intergovernmental Conference held in Birmingham, UK, 12-13 December 2005.

The increase in microorganisms that have developed resistance to currently available antimicrobial agents has become a major cause for concern worldwide. These organisms are widespread in hospitals but also occur increasingly in the community. Some of these strains are multiresistant and the agents available to treat infections caused by them are few and dwindling. Over recent years there have been a number of responses by national, international and professional bodies to this situation, many aimed at curbing this unprecedented growth in resistance, but there is an increasing recognition that

Role of Palliative Care in Onco-Hematology Retrospective Observational Cohort Study in Deceased In-Hospital Patients with SACT at the End of Life: Experience with Real-World Data from a Cancer Monographic Institution.

BACKGROUND: The American Society of Clinical Oncology (ASCO) established recommendations for palliative care (PC), and they still remain the most trusted source overall. The standard published by C. Earle (defined in solid tumors) for referral to PC is > 55%. However, these rates remain unclear in general onco-hematology. Our referral rate reaches 60%; while it meets the standard, there are significant differences between ST and HM. Several authors have already pointed out these discrepancies. Arguing in some cases its possible relationship with the different behavior of professionals with dif

Exploitation and psychological strain in residency training: workload and shift violations in Turkish medical schools and research hospitals.

BACKGROUND: In Turkey, the path to specialisation is a highly demanding and costly process. After completing six years of intensive medical education, general practitioners must pass the highly competitive Medical Specialisation Examination (TUS) to enter residency training. To pass the examination for entry into specialist training, medical students in Turkey are compelled to attend highly expensive preparatory courses during the final two years of their six-year medical education. Additionally, graduates working as general practitioners often struggle to prepare for the specialist examinatio
