History of Morgellons disease: from delusion to definition.

Morgellons disease (MD) is a skin condition characterized by the presence of multicolored filaments that lie under, are embedded in, or project from skin. Although the condition may have a longer history, disease matching the above description was first reported in the US in 2002. Since that time, the condition that we know as MD has become a polemic topic. Because individuals afflicted with the disease may have crawling or stinging sensations and sometimes believe they have an insect or parasite infestation, most medical practitioners consider MD a purely delusional disorder. Pseudocyesis, delusional pregnancy, and psychosis: The birth of a delusion.

Both pseudocyesis and delusional pregnancy are said to be rare syndromes, but are reported frequently in developing countries. A distinction has been made between the two syndromes, but the line of demarcation is blurred. The aim of this paper is to review recent cases of pseudocyesis/delusional pregnancy in order to learn more about biopsychosocial antecedents. The recent world literature (2000-2014) on this subject (women only) was reviewed, making no distinction between pseudocyesis and delusional pregnancy. Eighty case histories were found, most of them originating in developing countries.

Aripiprazole as a viable alternative for treating delusions of parasitosis.

Delusions of parasitosis (DOP) is a psychiatric disorder characterized by the fixed false belief that one is infested with parasites or other organisms. Historically, pimozide, a first-generation antipsychotic, has been the treatment of choice for DOP, although there is risk for serious adverse effects including extrapyramidal symptoms, QTc prolongation and tardive dyskinesia. Recently, there have been several reports describing the effectiveness of second-generation antipsychotics (SGAs), but these agents have their own unique adverse effects, specifically metabolic changes with olanzapine, s

Eradication of Blastocystis carriage with antimicrobials: reality or delusion?

Metronidazole constitutes a mainstay in the antimicrobial therapy of intestinal protozoa, and is also traditionally considered first-line therapy in cases where there is a requirement to treat Blastocystis, a common protist of disputable clinical significance. Many compounds have been used in attempts to eradicate the parasite, and an accumulating body of data indicates that successful antimicrobial eradication of Blastocystis is far from straightforward. This review focuses on some issues that prevent us from reaching a clear understanding of how to eradicate Blastocystis based on chemotherap

Perceptions of hostility by persons with and without persecutory delusions.

INTRODUCTION: Current models of paranoia propose that ambiguous situations, in which cues regarding the intentions of others are lacking, may be perceived as hostile by persons with persecutory delusions (PD). Thus, a social-cognitive bias for the perception of hostility may be present. In this study, the Ambiguous Intentions Hostility Questionnaire (AIHQ) was used to present situations that are ambiguous regarding the intentions of others. It was predicted that on the AIHQ, persons with PD would show greater levels of perceived hostility, blame, and aggression than both psychiatric and nonpsy

[Post-partum delusion of pregnancy: an integrated view].

Case study and review of the literature We review the nosology and phenomenology of delusion of pregnancy. Two cases with post partum delusion of pregnancy which can be regarded as a delusion with bizarre content are described. The possible etiological factors particularly aspects of the novel cognitive theories of delusion formation and the hyperprolactinaemia caused by antipsychotic treatment are considered. While discussing the cases with post partum delusion of pregnancy we integrate the false sensory and cognitive processing of the puerperal somatic changes with the motivation theory of d

Somatoparaphrenia: a body delusion. A review of the neuropsychological literature.

A review of published brain-damaged patients showing delusional beliefs concerning the contralesional side of the body (somatoparaphrenia) is presented. Somatoparaphrenia has been reported, with a few exceptions, in right-brain-damaged patients, with motor and somatosensory deficits, and the syndrome of unilateral spatial neglect. Somatoparaphrenia, most often characterized by a delusion of disownership of left-sided body parts, may however occur without associated anosognosia for motor deficits, and personal neglect. Also somatosensory deficits may not be a core pathological mechanism of soma

Persistent auditory hallucinations and their relationship to delusions and mood.

Twelve patients who met DSM-III-R criteria for schizophrenia and had persistent auditory hallucinations completed a diary three times daily for 2 to 3 weeks. Using 5-point equal interval rating scales, nearly all patients were able to record consistently the nature of their hallucinations, the intensity of their delusional beliefs, and the mood and clarity of their thinking. For at least half the patients, there was a statistically significant relationship between the loudness and intrusiveness of hallucinations and the intensity of delusional beliefs; the more intrusive and distressing were t

Good diabetic control - a study in mass delusion.

The terms "diabetic control" and "diabetic management" are not synonymous. Diabetic control implies normal glucose metabolism, typically monitored by periodic determinations of plasma glucose and urine reducing sugar concentrations. A group of 220 diabetic children attending a camp complied 74% of the time with the request to collect and test their urine for URS. Fifty percent of random URS values determined by the children varied from those obtained on the same specimens of urine by laboratory technicians. Good diabetic control defined URS less than 25gm/24 hours was found in 18 of 54 childre

Functional brain networks underlying probabilistic reasoning and delusions in schizophrenia.

Delusions in schizophrenia are false beliefs that are assigned certainty and not afforded the scrutiny that normally gives rise to doubt, even under conditions of weak evidence. The goal of the current functional magnetic resonance imaging (fMRI) study is to identify the brain network(s) involved in gathering information under conditions of weak evidence, in people with schizophrenia experiencing delusions. fMRI activity during probabilistic reasoning in people with schizophrenia experiencing delusions (n = 29) compared to people with schizophrenia not experiencing delusions (n = 41) and healt

Functional Brain Networks Underlying Evidence Integration and Delusions in Schizophrenia.

Integrating evidence that contradicts a belief is a fundamental aspect of belief revision and is closely linked to delusions in schizophrenia. In a previous functional magnetic resonance imaging (fMRI) study on healthy individuals, we identified functional brain networks underlying evidence integration as visual attention network (VsAN; dorsal anterior cingulate cortex, insula, occipital regions), default-mode network (DMN), and cognitive evaluation network (CEN; orbitofrontal cortex, inferior frontal gyrus, parietal cortex). In the current clinical fMRI study, we compared network-based activi

Associations between ZnT3, tau pathology, agitation, and delusions in dementia.

OBJECTIVE: Neuropsychiatric symptoms such as agitation and delusions occur frequently in Lewy body dementia and Alzheimer's disease and represent significant burden and unmet treatment need. The underlying aetiology remains poorly understood. METHODS: We used a multidimensional linear model to look for associations between measurements of agitation, delusions, amyloid, tau and α-synuclein pathology, and synaptic proteins (ZnT3, PSD95, synaptophysin, and β-III-tubulin) across multiple brain regions in post-mortem tissue from a cohort of 130 Lewy body dementia and Alzheimer's disease patients a

Religious delusions in older adults: Diagnoses, combinations, and delusional characteristics.

OBJECTIVE: Religious delusions (RDs) tend to occur relatively often in patients with affective or non-affective psychosis. Few studies exist about RDs in later life. The current study explores (1) the distribution of RDs across diagnosis, (2) how RDs relate to other types of delusions, and (3) how RDs relate to several dimensions or characteristics of delusions. METHODS: Inpatients and outpatients in a Geriatric Psychiatry Department in the Netherlands (N = 155; mean age 76.5 years), and who were diagnosed with an affective or non-affective psychotic disorder, participated in semi structured

Delusional misidentifications and duplications: right brain lesions, left brain delusions.

When the delusional misidentification syndromes reduplicative paramnesia and Capgras syndromes result from neurologic disease, lesions are usually bifrontal and/or right hemispheric. The related disorders of confabulation and anosognosis share overlapping mechanisms and anatomic pathology. A dual mechanism is postulated for the delusional misidentification syndromes: negative effects from right hemisphere and frontal lobe dysfunction as well as positive effects from release (i.e., overactivity) of preserved left hemisphere areas. Negative effects of right hemisphere injury impair self-monitori

Right prefrontal hypometabolism predicts delusions in dementia with Lewy bodies.

Delusions (DEL) are frequent in dementia with Lewy bodies (DLB); however, the neural equivalent is poorly understood. The present study therefore aimed to identify the cerebral metabolic pattern of glucose of a DLB group suffering from DEL (DLB+DEL) as compared to a non-delusional group (DLB-DEL) and a control group (NL); and to determine the predictive value of the regional metabolic deficit for DEL symptomatology in comparison to other clinical variables significantly associated with DEL. Voxel-wise comparisons were conducted between the patient and control groups in SPM2. The most significa

[Dynamics of delusions during the late stages of paranoid schizophrenia].

Under examination there were 100 patients with paranoid schizophrenia who had reached an old age. In all the patients signs of delusion reduction were observed at the late stages of the disease. It was shown that most delusional disorders tended to transform into residual ones and to become peculiar manifestations of the derangement. Against this background it was actual delusional experiences that persisted most frequently. More and more place in their structure took psychopathological elements that reflected various aspects of the ageing of the patients' psychosis.

Vampiric Delusions in Treatment-Resistant Schizoaffective Disorder: A Case Report.

Clinical vampirism, or Renfield syndrome, is a rare psychiatric phenomenon characterized by the compulsion or desire to consume blood and has been described in association with psychotic and other psychiatric disorders. Reported presentations may include blood-drinking behaviors, urges to consume blood, and delusions or hallucinations involving blood. This report details the case of a 28-year-old man with a history of treatment-resistant schizoaffective disorder and adolescent-onset polysubstance use who presented with prominent vampiric delusions. The patient endorsed urges to consume blood i

MRI Study of the Brain Structural Characteristic in Persecutory Delusions.

The structural MRI of the cortex, subcortical structures, hippocampal subfields, and brainstem regions in patients with persecutory delusions in schizophrenia (ICD-10 code: F20) and delusional disorder (ICD-10 code: F22.0) were analyzed. Patients demonstrated multiple regions of decreased cortical gray matter thickness, reduced volumes of the left thalamus and some hippocampal sub-fields compared to mentally healthy subjects. No alterations in brainstem structures were found. The results were analyzed within the framework of the transnosological model of delusions.
