Dental implants placed in a patient with multiple myeloma: report of case.

A 69-year-old male, under active treatment for multiple myeloma, fractured his left maxillary second premolar palatal abutment tooth and requested treatment because his upper partial denture no longer fit. Several other teeth were deemed hopeless, and construction of a maxillary complete denture presented problems. There are general contraindications regarding dental implant surgery in the terminally ill patient; however, in this case, the patient wished to maximize his quality of life and opted to undergo implant surgery. With close management, the treatment was successful.

Assessment of the Oropharyngeal Space Using CBCT and Its Impact on Volume of Airway Prior and after Denture Placement: In Vivo Study.

AIM: The current study aimed to assess the oropharyngeal space using cone-beam computed tomography (CBCT) and its effect on airway volume both before and after denture placement. MATERIALS AND METHODS: For this investigation, a total of 15 individuals with fully edentulous upper and lower ridges, ranging in age from 40 to 70, were taken into consideration. A recording of the pulmonary function test was made both prior to and following full denture recovery. Prior to finishing denture therapy, the patient underwent a CBCT scan while standing erect and wearing a cephalostat. For one patient, tw

Quantitative 3D Evaluation of Facial Soft Tissue Modifications Following Complete Denture Treatment in Edentulous Patients: A Prospective Before-After Study.

Background: Three-dimensional (3D) facial scanning is an objective, non-invasive method for quantifying facial soft-tissue changes following complete denture (CD) rehabilitation. Reliable quantification of these changes in completely edentulous patients can support more predictable aesthetic and functional outcomes. Methods: This prospective before-after observational study included 30 completely edentulous patients (12 men, 18 women; age 48-87 years; mean ± SD: 67.8 ± 9.2 years) who received new maxillary and mandibular CDs. Structured-light 3D facial scans were obtained at baseline (edentulo

Effects of wearing and removing dentures on oropharyngeal motility during swallowing.

Wearing dentures and dysphagia are common in older individuals; however, it is still unknown how dentures affect oral and pharyngeal swallowing. The purpose of this study was to reveal the effects of wearing and removing dentures on oropharyngeal movements during pharyngeal swallowing in the feeding sequence of solid food. Participants were 25 edentulous volunteers (nine men, 16 women; mean age 76·2 years) who wore complete dentures. The test food was minced agar jelly containing barium sulphate. Subjects were instructed to feed and swallow the test food with or without dentures during observa

Clinical and microbiologic effects of chemical versus mechanical cleansing in professional supportive implant therapy.

The aim of the present study was to compare the cleansing properties of mechanical supportive care for dental implants with the use of an etching gel. Sixteen patients underwent a 5-month clinical trial with monthly recalls. These patients, wearing maxillary complete dentures and mandibular overdentures supported by a bar device on 4 implants, were treated in a split-mouth study design. Test and control therapy were randomly assigned to left and right sides of the mandible. At the test side, 35% phosphoric etching gel (pH 1) was applied in the peri-implant sulcus. After 1 minute, the sulcus wa

Ridge augmentation with dense hydroxylapatite/resorbable suture matrix.

The atrophic edentulous ridge represents one of the greatest challenges faced in general practice. An estimated 20 million adults in the U.S. are edentulous in at least one arch and a significant number of these patients experience problems with their prostheses from alveolar bone loss. Many of these patients, for various reasons, are not candidates for endosseous dental implants. Alveolar ridge augmentation is a viable option in many of these cases; however, this procedure has been unpredictable because of particle migration. Presented here is a method of ridge augmentation with dense hydroxy

Stabilization splint treatment on complete denture--two reported cases.

The pre-prosthetic treatments of two female patients with osteoarthritis of the temporomandibular joints were presented in this paper. In the first case report, an 83-year-old patient wore complete dentures (CDs) for 30 years. The second patient (71 years of age) wore a three-year-old upper CD and a lower removable partial denture with a two-unit fixed dental prosthesis. Pain was successfully eliminated by the stabilization splint on the upper CD. Magnetic resonance imaging was used to confirm the diagnosis and clinical follow-up of the splint treatment.

Prosthodontic rehabilitation of dental implants with exposed threads: a clinical report.

An unconventional technique was used to address the challenge of exposed dental implant threads. Titanium dental implants were prepared with rotary instruments to eliminate the exposed threads and to receive a fixed cementable prosthesis. The technique required fabrication of a gold casting of 2 copings connected by a round bar to fit the prepared implants and an implant-retained overdenture. This technique may be applied to other situations when multiple implants present with thread exposure and bone resorption. These include single crowns, partial fixed dental prostheses, and attachments for

Management of patients with excessive gingival display for maxillary complete arch fixed implant-supported prostheses.

Maxillary complete arch fixed implant-supported prostheses are a popular treatment option for edentulous patients. Excessive gingival display or gummy smile in edentulous patients is relatively uncommon. However, many partially edentulous patients or completely dentate patients with compromised dentition and excessive gingival display may seek a fixed implant-supported prosthesis. Some of these patients may be candidates for immediate implant placement and insertion of the prosthesis, while others may carry over their preexisting excessive gingival display to the edentulous state for a variabl

Reconstruction of severely resorbed atrophic maxillae and management with transitional implants.

The reconstruction of the severely resorbed maxilla requires complex surgical treatment sequencing. Often, multiple grafting procedures are required either before or in conjunction with implant placement. Regardless of the surgical modality, the grafting procedures and the placement of implants in poor quality bone require undisturbed healing during which no pressure is placed on the grafted implant ridge. The use of transitional implants allows the surgeon to provide stable temporary prostheses throughout the healing phase, while preventing pressure from being placed on the grafted or implant

Studies of changes in occlusion after the insertion of complete dentures. Part I.

The present study aims to investigate changes in the occlusion of complete dentures after their insertion. A total of 85 edentulous patients were provided with new complete dentures. Their individual hinge axes were determined using mechanical axiography and the upper finished dentures were transferred by facebows to Dentatus articulators. The lower dentures were mounted according to an intraoral central bearing point (CBP) registration and equilibrated in terminal hinge position. An articulator specially modified for measurements in the condylar area was used. The differences between the posi

Complete Denture in a Microstomia Patient.

Microstomia is the term used to describe a condition of reduction in the size of oral aperture which can be either acquired or congenital and affects the quality of life. Dentists occasionally come across patients with constricted oral openings. Limited oral opening makes access to the oral cavity for any dental procedure difficult. It's the duty of a dentist to provide every possible care to the patient with microstomia. This paper describes the treatment of a microstomia patient requiring complete dentures using sectional dentures. The cause of microstomia in the indexed patient was developm

Restoration of blinking reflex and facial symmetry in a Bell's palsy patient.

Patients afflicted with Bell's palsy are faced with both functional and esthetic impairment. Prominent among these are the inability to close the eyelids and abnormal facial appearance, with concomitant difficulty in eating, drinking and speaking. Rehabilitation of such patients can be achieved by a multispecialty approach, with the prosthodontist functioning as an integral part of the treatment team. This article describes a simple and effective approach to restore the blinking reflex of the upper eyelid with custom gold implant and facial esthetics with cheek support prosthesis.

A simple method for prosthodontic rehabilitation of edentulous patient with epidermolysis bullosa: a clinical case report.

An abnormally small oral orifice is defined as microstomia. Microstomia may result from epidermolysis bullosa (EB), which consists of a group of disorders characterized by the presence of mechanical fragility of the skin with recurrent de-velopment of blisters and vesicles, resulting from minor mechanical friction or trauma. Since such patients have a small oral aperture, it may be impossible to take impression and fabricate dentures using conventional methods. In this article, a simple method for taking preliminary impressions from upper and lower edentulous ridges in one patient with limited

The etiology of inflammatory papillary hyperplasia.

An investigation was carried out to evaluate some of the etiologic factors of inflammatory hyperplasia. The lesion was found in 97 out of 700 patients (13.9 per cent). It occurred in the palate and was always associated with the wearing of dentures, especially those which had relief areas in the palate. It was possible to associate the lesion with constant wearing of the dentures. The results indicate that the cause of inflammatory papillary hyperplasia is complex, and it seems to occur only in susceptible subjects when the tissues are irritated by local factors. The lesion is treated by reduc

Comparison of obturator design for acquired maxillary defect in completely edentulous patients.

The most challenging and appreciated area in the field of Prosthodontics is the rehabilitation of maxillary defects. Tumors of the head and neck are the common cause for acquired maxillofacial defects. Surgical consequences predispose the patient to hypernasal speech, fluid leakage into nasal cavity, impaired masticatory function, and cosmetic deformity. The Prosthodontists play a significant role in the intervention and improve the quality of life of such patients. The current article describes two clinical case reports of completely edentulous patients with acquired maxillary defects.
