PURPOSE: To report a case of corneal edema associated with Descemet membrane (DM) damage likely due to shockwave trauma from a bullet passing by the eye.
OBSERVATIONS: A 27-year-old man presented to the emergency department with pain, redness, and severely decreased vision in the left eye immediately following a gun assault. Examination of the eye revealed a temporal conjunctival laceration, corneal edema, hyphema, and vitreous hemorrhage. Bullets were found in the patient's left chest and axilla, and a laceration was present on the bridge of his nose, but no foreign bodies were found in or a

We present the case of a boy with congenital bilateral folds in Descemet's membrane, causing high astigmatism and myopia. There are multiple causes of folds and tears in Descemet's membrane. In our case, the most likely origin is the mother's prolonged labor, although a severe car accident of the mother at the gestational age of 27 weeks as the cause of these folds cannot be entirely excluded.

Objective: To report a new technique for Descemet's membrane endothelial keratoplasty (DMEK) in aphakic and vitrectomized eyes. Case description: A 56-year-old man presented with corneal decompensation in the left eye after combined pars plana vitrectomy and lensectomy for the management of a traumatic eye injury. DMEK graft and recipient bed were prepared as regular. The posterior half of the donor stroma was dissected with a crescent knife to form a temporary stromal barrier tissue. The prepared donor stromal tissue was implanted into the anterior chamber (AC) to form a barrier over the iris

The eyes of a 25-year-old male were collected by the Utah Lions Eye Bank after his suicide by hanging. Following dissection of the corneoscleral buttons from intact globes, bilateral detached Descemet's membranes with subsequent scrolling in the periphery were observed. We believe these findings were caused by a large increase in intraocular pressure secondary to the hanging. Lens and anterior capsule fractures after hanging have been reported, but corneal damage has never been discussed. We invite transplant surgeons and eye bank recovery specialists to share their experience of similar corne

The purpose of this study was to characterize age-associated changes in the corneal endothelium and Descemet's membrane (DM) in C57BL/6 mice, an inbred strain commonly used as a genetic disease model. Corneas from mice aged 2 weeks to 24 months were studied. Light microscopy was used to assess central endothelial cell density, area, and morphology. Transmission electron microscopy was used to assess thickness and ultrastructural features of DM. Central corneal endothelial cell density showed a rapid decline from 5,232+/-892 cells/mm(2) (mean+/-S.D.) at 2 weeks to 2,532+/-112 cells/mm(2) at 16

PURPOSE/METHODS: We present two cases of Descemet's membrane detachment, the first one after extracapsular cataract extraction with scleral tunnel incision and the other after clear corneal incision used for phacoemulsification cataract extraction. These two cases were treated with intracameral injection of 20% sulfur hexafluoride gas (SF(6)).
RESULTS/CONCLUSIONS: Both cases responded satisfactorily to treatment. After four months of follow-up, the corneas remain clear with a visual acuity of 0.9 and 0.8, respectively. The relative facility of the 20% sulfur hexafluoride gas (SF(6)) technique

To determine whether Descemet's membrane (DM), which is altered by disease, interferes with endothelial cell growth, healthy human corneal endothelial cells were seeded onto DM from normal corneas and diseased corneal buttons from patients with Fuchs' endothelial dystrophy and pseudophakic bullous keratopathy (PBK). DM was first peeled off the corneal specimens and the endothelial cells removed by trypsinization. A suspension of first-passage corneal endothelial cells (2,000 cells/microliters; obtained from donor eye bank eyes and grown in Dulbecco's minimal essential medium with 10% fetal bov

A 45-year-old woman with healed syphilitic interstitial keratitis presented with decreased vision, foreign-body sensation of the right eye, and bilateral conjunctival injection. Examination disclosed a network of tissue on the posterior corneal surface, with extension into the anterior chamber. Bullous keratopathy developed after intracapsular cataract extraction, and a keratoplasty was performed. Pathologic examination of the corneal button disclosed corneal edema, posterior stromal vascularization, much thickening of Descemet's membrane, and concentrically laminated scrolls of Descemet's mem

The ultrastructure of Descemet's membrane was studied by transmission electron microscopy in nine corneal buttons from aphakic eyes with bullous keratopathy. All nine corneas had abnormal collagenous material in the posterior portion of Descemet's membrane. Five of the buttons had abnormalities in layers of Descemet's membrane secreted at an early age. These abnormalities are evidence of endothelial dysfunction long before cataract extraction. In the remaining four corneas, the Descemet's membrane secreted prior to cataract extraction remained relatively normal, despite corneal endothelial dec

The effect of age on the compositional analysis of human Descemet's membrane was examined. Membranes were obtained from human corneas ranging from 0.5 to 95 years and purified by sodium dodecyl sulfate (SDS) extraction. Substantial age-related changes were obtained in the amino acid compositions of the SDS-insoluble basement membranes. These results suggested that compositonal changes occur during the aging process, which appeared to reflect an alteration in the subunit make-up of the basement membrane.

PURPOSE: To report the visual and anatomical outcomes of Descemet membrane epiretinal graft for refractory full-thickness macular holes (MH).
METHODS: This interventional case series included patients with refractory MH who previously underwent standard pars plana vitrectomy with internal limiting membrane peeling for repair of MH with no success. Donor corneas unsuitable for keratoplasty were stripped at the Eye Bank. The corneo-scleral button was stained with blue dye, punched to obtain a 2-mm Descemet membrane graft which was positioned over the MH. At the end of surgery, 20% sulfur hexafl

PURPOSE: The postoperative occurrence of corneal guttae (CG) in patients after Descemet membrane endothelial keratoplasty (DMEK) can lead to a significant reduction in visual acuity (VA) with the subsequent need for repeat DMEK. Therefore, the aim of this study was to analyze the prevalence and clinical significance of CG in transplanted corneas after DMEK.
METHODS: The prevalence and progression of CG after DMEK of 1657 patients were examined using endothelial specular microscopy images. The severity grade of CG was classified into 3 grades (G1 <40%, G2 40-80%, G3 >80%). Central corneal thic
