Major incident response: collecting ante-mortem data.

The Asian tsunami of 26 December 2004, which devastated coastal parts of more than 10 countries in and around the Indian Ocean caused over 200,000 casualties. People from more than 58 nationalities were amongst the victims and subsequently an international effort for disaster victim identification (DVI) was set up, coordinated by Interpol. DVI teams from more than 20 countries took part in the identification process which, because of the complexity of the situation, had to be conducted in an internationally agreed upon procedure. Standard operating protocols of post-mortem (PM) procedures were

Toxins of concern to animals and people.

Historically veterinarians have diagnosed accidental poisonings and identified possible terrorist events before they have come to the attention of public health authorities. There are many toxins that pose a threat to both humans and animals and the authors examine several of them here, namely, anthrax, tricothecenes, staphylococcal enterotoxin B, botulinum toxins, ricin, saxitoxin and dinoflagellate toxins. By discussing exposure routes, clinical signs and differential diagnoses the authors demonstrate how veterinarians are in a unique position to recognise zoonotic diseases, toxin exposure,

Submarine escape from depths of 30 and 60 feet: 41,183 training ascents without serious injury.

INTRODUCTION: In the case of a submerged, disabled submarine, survivors may be forced to escape by entering the water and ascending rapidly to the surface. The large pressure changes involved may produce pulmonary barotrauma, arterial gas embolism, or barotrauma. To assess the likelihood of such injuries, we retrospectively evaluated medical problems due to submarine escape training among military personnel. METHODS: We evaluated 41,183 controlled ascents performed over the past 21 yr in the escape training tank at Gölcük-Kocaeli, Turkey. Each trainee performed two free ascents from 30 ft and

Pakistan earthquake: experiences of a multidisciplinary surgical team.

INTRODUCTION: Four weeks after the earthquake in Kashmir, Pakistan, multidisciplinary surgical teams were organized within the United Kingdom to help treat disaster victims who had been transferred to Rawalpindi. The work of these teams between 05-17 November 2005 is reviewed, and experiences and lessons learned are presented. METHODS: Two self-sufficient teams consisting of orthopedic, plastic surgical, anesthetic, and theatre staff were deployed consecutively over a two-week period. A trauma unit was set up in a donated ward within a private ophthalmological hospital in Rawalpindi. RESULTS

Unexplained deaths in Connecticut, 2002-2003: failure to consider category a bioterrorism agents in differential diagnoses.

BACKGROUND: Recognition of bioterrorism-related infections by hospital and emergency department clinicians may be the first line of defense in a bioterrorist attack. METHODS: We identified unexplained infectious deaths consistent with the clinical presentation of anthrax, tularemia, smallpox, and botulism using Connecticut death certificates and hospital chart information. Minimum work-up criteria were established to assess the completeness of diagnostic testing. RESULTS: Of 4558 unexplained infectious deaths, 133 were consistent with anthrax (2.9%) and 6 (0.13%) with tularemia. None were co

Pro patria et spes gentis: military medicine, paediatric surgery and those who care for children.

Children and military medicine have many links. On humanitarian and disaster deployments, the surgery of war has increasingly seen children as the focus of clinical salvage. When Romans spoke of children, they used the phrase 'spes gentis'-'the hope of the race'. In modern times, there developed a synergy, in the context of defensive war, that its prosecution depended not only on the defence of territory but also on its hopes for continuation of people and culture, into the future. In the 19th century, in Australia, several regiments had the motto 'Pro Aris et Focis'-'For the Defence of Hearth

[Visceral surgery in mass casualty incidents due to disasters, crises and war].

BACKGROUND: Mass casualty incidents with a focus on visceral surgery pose major medical and organizational challenges for hospitals. They are characterized by a large number of simultaneously admitted patients, limited personnel and material resources as well as the necessity to make time-critical surgical decisions. From a visceral surgical perspective, abdominal injuries involving severe bleeding, contamination and multiorgan involvement are particularly demanding as they require rapid surgical intervention and close interdisciplinary cooperation. This injury pattern is typically encountered

Artificial intelligence in age and sex determination using maxillofacial radiographs: A systematic review.

In the past few years, there has been an enormous increase in the application of artificial intelligence and its adoption in multiple fields, including healthcare. Forensic medicine and forensic odontology have tremendous scope for development using AI. In cases of severe burns, complete loss of tissue, complete or partial loss of bony structure, decayed bodies, mass disaster victim identification, etc., there is a need for prompt identification of the bony remains. The mandible, is the strongest bone of the facial region, is highly resistant to undue mechanical, chemical or physical impacts a

Preparedness for the evaluation and management of mass casualty incidents involving anticholinesterase compounds: a survey of emergency department directors in the 12 largest cities in the United States.

OBJECTIVES: Anticholinesterases include carbamate and organophosphorus (OP) insecticides and nerve agents. Release of these compounds can flood emergency departments (EDs) with large numbers of poisoned victims and worried individuals. It was hypothesized that despite the focus of disaster preparedness on large metropolitan areas, EDs in these cities would still report self-perceptions of deficiencies in preparedness for mass casualty incidents (MCIs) involving these chemicals. DESIGN AND SETTING: A secure and anonymous online survey was prepared and piloted, and E-mail invitations were sent

[Expert consensus on the clinical application of continuous glucose monitoring in critically ill patients (2026 edition)].

Glycemic fluctuations are highly prevalent among critically ill patients. Glycemic abnormalities constitute an independent risk factor for adverse outcomes, and their effective identification and management are essential to improving clinical prognosis in this population. Continuous glucose monitoring (CGM) technology, which continuously and in real time captures interstitial fluid glucose concentrations, provides multidimensional glycemic metrics and offers a novel technical approach to overcoming the inherent limitations of conventional intermittent glucose monitoring, thereby enabling refin

[Bioterrorism].

Biological terrorism is intentionally to use infectious substances for developing diseases or death in animals or humans, leading to disaster and panic in our community. Bioterrorism-associated diseases are mostly rare or eradicated infectious diseases, and recently, we do not have experience to make a clinical and laboratory diagnosis. In particular, these infectious diseases have incubation periods from infection to development of the disease. The staff working at public health institutions, including legal medicine, must be involved in as a first responder when bioterrorism would happen. Th

Frontlines of Climate Change and Global Health Inequity: How Recurring Cyclones Undermine Health, Livelihoods, and Development in the Indian Sundarbans.

This study investigates the cascading impacts of recurrent cyclones on the physical and mental health, livelihoods, infrastructure, well-being, and long-term development of communities in the Indian Sundarbans, one of the world's most climate-vulnerable regions. Semi-structured interviews were conducted with community members, frontline public health workers government officials, NGO leaders, mental health counselors, and non-licensed village doctors. We aimed to identify the ecological, geographic, and socioeconomic conditions exacerbating the region's vulnerability to cyclones; examine the i

Creating and Maintaining a "Climate-Smart" Emergency Department: A Scoping Review of Current Progress and Future Potential.

INTRODUCTION: Climate change represents one of the most significant global health threats, with emergency departments (ED) serving as frontline responders to climate-related health emergencies. While EDs are major contributors to healthcare's environmental footprint and critical responders for climate disasters, no comprehensive review has examined sustainability and climate-resilience initiatives specifically implemented in ED settings. METHODS: We conducted a scoping review examining literature on sustainable and climate-resilient measures in EDs. Comprehensive searches of PubMed, Scopus, a

The Development of Swedish Military Healthcare System: Part II-Re-evaluating the Military and Civilian Healthcare Systems in Crises Through a Dialogue and Study Among Practitioners.

INTRODUCTION: Historical changes have transformed Sweden from being an offensive to a defensive and collaborative nation with national and international engagement, allowing it to finally achieve the ground for the civilian-military collaboration and the concept of a total defense healthcare. At the same time, with the decreasing number of international and interstate conflicts, and the military's involvement in national emergencies and humanitarian disaster relief, both the need and the role of the military healthcare system within the civilian society have been challenged. The recent impact

Infectious disease risks from dead bodies following natural disasters.

OBJECTIVE: To review existing literature to assess the risks of infection from dead bodies after a natural disaster occurs, including who is most at risk, what precautions should be taken, and how to safely dispose of the bodies. METHODS: Disease transmission requires the presence of an infectious agent, exposure to that agent, and a susceptible host. These elements were considered to characterize the infectious disease risk from dead bodies. Using the PubMed on-line databases of the National Library of Medicine of the United States of America, searching was done for relevant literature on th

Building community resilience to prevent and mitigate community impact of gun violence: conceptual framework and intervention design.

INTRODUCTION: The USA has the highest rate of community gun violence of any developed democracy. There is an urgent need to develop feasible, scalable and community-led interventions that mitigate incident gun violence and its associated health impacts. Our community-academic research team received National Institutes of Health funding to design a community-led intervention that mitigates the health impacts of living in communities with high rates of gun violence. METHODS AND ANALYSIS: We adapted 'Building Resilience to Disasters', a conceptual framework for natural disaster preparedness, to
