Pituitary apoplexy (PA) is a rare but potentially life-threatening endocrine emergency caused by sudden hemorrhage or infarction usually within a pituitary tumor. Clinical presentation is highly variable, ranging from isolated headache to severe visual loss or altered consciousness. Prompt recognition, multidisciplinary evaluation, and timely management are essential to improve outcomes. Recent prospective and real-world studies have challenged the traditional view that urgent surgery is always required. Conservative management has been proposed to be a safe and effective option, although comp

Moyamoya disease is a rare chronic cerebrovascular disease seen both in children and adults. It has a progressive course, but may have a variable clinical presentation. The disease causes ischemic stroke, intracranial hemorrhage, headache, seizures, and transient ischemia attack in children and in adults. Although the pathogenesis of the disease remains unknown, research suggests a genetic predisposition. There are also undefined systemic processes involved in this vasculopathy. Better noninvasive diagnostic techniques for diagnosis of the Moyamoya disease have been developed, but medical trea

Advances in the management of traumatic brain injury, subarachnoid haemorrhage and intracranial tumours have led to improved survival rates and an increased focus on quality of life of survivors. Endocrine sequelae of the acute brain insult and subsequent neurosurgery, peri-operative fluid administration and/or cranial irradiation are now well described. Unrecognised acute hypopituitarism, particularly ACTH/cortisol deficiency and diabetes insipidus, can be life threatening. Although hypopituitarism may be transient, up to 30% of survivors of TBI have chronic hypopituitarism, which can diminis

Peripheral arterial disease (PAD) is a chronic atherosclerotic disease of the lower extremities associated with high cardiovascular morbidity and mortality, as well as destructive limb-associated complications such as intermittent claudication and ischemic rest pain. While revascularization with surgical and endovascular techniques is important to prevent limb loss in patients with critical limb ischemia, it fails to address the early symptoms of PAD or the systemic cardiovascular risks associated with PAD. The management of PAD incorporates aggressive lifestyle modification and comprehensive

Adult patients with sickle cell disease may present to the emergency department with a broad spectrum of acute clinical manifestations. These acute complications can be life-threatening and require a structured, time-sensitive emergency management approach. Although vaso-occlusive pain crises represent the most frequent cause of presentation, other severe complications-including cerebrovascular events, acute chest syndrome, priapism, and hepatobiliary disorders-may also require immediate medical evaluation. Prompt diagnostic assessment and initiation of appropriate therapy are essential to pre

BACKGROUND: Anxiety is a common, distressing, hard-to-diagnose and hard-to-treat symptom in Parkinson's disease. No formal guidelines exist to assist management.
OBJECTIVE: We provide a pragmatic guide to detecting and managing anxiety in Parkinson's disease.
METHODS: In this educational review, we describe the phenomenology, diagnostic challenges, hypothesized neurobiology, and the rationale for treatments for anxiety in Parkinson's disease. We review the major drug-classes and non-pharmacological treatment approaches in current use. We also present a meta-analysis of pharmacological treatm

Osgood-Schlatter's disease is a type of injury that occurs in children and adolescents due to overuse. The condition is characterized by pain, swelling, and inflammation in the region of the tibial tuberosity. Running and jumping intensively during periods of accelerated skeletal growth is associated with repeated and prolonged tensile forces that cause the disease. Initial inflammation turns into partial avulsion of cartilage from the tibial tuberosity, followed by the formation of ossicles in more severe cases. In most cases, OSD is managed conservatively by restricting strenuous exercise, p

Kienböck's disease, an uncommon avascular necrosis of the lunate, remains challenging to manage, particularly in young adults. We report a woman in her 20s with early-stage disease (Lichtman stage II) and positive ulnar variance, but without lunate collapse. She underwent a distal radial osteotomy with autologous cancellous bone grafting, stabilised with a volar locking plate, along with intraosseous lunate decompression. Postoperative recovery was uneventful, with complete union at the osteotomy site, restoration of wrist motion and return to unrestricted activity at 1 year. This case illustr

INTRODUCTION/BACKGROUND: Extracellular vesicles (EVs) are non-replicating lipid-bilayered bodies that are naturally released by a cell that aid in various biological functions including cell-to-cell communication. They resemble the cells that they originate from, mimicking their composition and contents. The shared Endosomal Sorting Complex Required for Transport (ESCRT) mechanism between virions and EVs allows EVs to aid in the dispersion and infection of viruses.
SCOPE/OBJECTIVES: The aim of this review is to encapsulate important studies that highlight the potential use of EVs in diagnosis

The treatment of peripheral arterial disease (PAD) has undergone many advancements and management continues to evolve. These changes include techniques, adjunctive therapies, medical management, and site of service. However, despite these advances, there still continues to be much debate about the best treatment. This Annals of Vascular Surgery special issue on PAD seeks to highlight current available evidence and expert views on treatment of PAD.

Perianal fistulizing Crohn's disease (CD) represents a severe phenotype of CD that is associated with significant morbidity and reduction in quality of life. Perianal fistulizing CD is caused by a complex interplay of genetic predisposition, immune dysregulation, gut dysbiosis, and various unknown physiological and mechanical factors. A multidisciplinary approach is hence required for optimal management . A detailed anatomical description and classification of perianal fistula, including comprehensive clinical, endoscopic, and radiological diagnostic workup, is an important prerequisite to tre

As a mechanism of cell death, ferroptosis has gained popularity since 2012. The process is distinguished by iron toxicity and phospholipid accumulation, in contrast to autophagy, apoptosis, and other cell death mechanisms. It is implicated in the advancement of multiple diseases across the body. Researchers currently know that osteosarcoma, osteoporosis, and other orthopedic disorders are caused by NRF2, GPX4, and other ferroptosis star proteins. The effective relief of osteoarthritis symptoms from deterioration has been confirmed by clinical treatment with multiple ferroptosis inhibitors. At

Colchicine-an anti-inflammatory alkaloid-has assumed an important role in the management of cardiovascular inflammation ≈3500 years after its first medicinal use in ancient Egypt. Primarily used in high doses for the treatment of acute gout flares during the 20th century, research in the early 21st century demonstrated that low-dose colchicine effectively treats acute gout attacks, lowers the risk of recurrent pericarditis, and can add to secondary prevention of major adverse cardiovascular events. As the first Food and Drug Administration-approved targeted anti-inflammatory cardiovascular the

PURPOSE OF REVIEW: Peyronie's disease characterizes a condition in which there is angular curvature of the penis. We know that the most patients with Peyronie's disease will not have spontaneous resolution of their penile curvature. As such, patients who desire treatment can elect for either surgical or nonsurgical therapy. Herein, we discuss the contemporary nonsurgical management options for Peyronie's disease.
RECENT FINDINGS: Nonsurgical management options for Peyronie's disease include oral therapy, intra-lesional injections, and penile traction therapy. At the time of this review, there

Buerger's disease (BD), also known as thromboangiitis obliterans, is a non-atherosclerotic inflammatory disorder of unknown aetiology that affects small-sized and medium-sized vessels of the extremities. It is usually observed in middle-aged adults, especially those who smoke or use tobacco products. This condition is more frequently observed in men, although recent findings indicate an increasing prevalence among women, potentially due to increased cigarette use. The association between pregnancy and BD is rare, with only a few published cases. Previous reports have indicated that BD may wors

CASE REPORT: Krill's disease or acute retinal pigment epithelitis (ARPE) is a self-limiting retinal disease with no specific treatment. Typical clinical and imaging features helped us to diagnose her with ARPE. Intravenous methylprednisolone (IVMP), which gives a rapid anti-inflammatory response, was advised. An SD-OCT scan post-injection showed a reduction in hyperreflectivity and height of lesion at day 3 and near total resolution by day 5.
CONCLUSION: This case suggests rapid resolution o

IMPORTANCE: The management of pilonidal disease continues to be a challenge due to high rates of recurrence and treatment-associated morbidity.
OBSERVATIONS: There is a heterogeneous repertoire of treatment modalities used in the management of pilonidal disease and wide practice variation among clinicians. Available treatment options vary considerably in their level of invasiveness, associated morbidity and disability, risks of complications, and effectiveness at preventing disease recurrence. Conservative nonoperative management strategies, including persistent improved hygiene, depilation,
