Outbreake of Marburg virus disease in Johannesburg.

The first recognised outbreak of Marburg virus disease in Africa, and the first since the original epidemic in West Germany and Yugoslavia in 1967, occurred in South Africa in February 1975. The primary case was in a young Australian man , who was admitted to the Johannesburg Hospital after having toured Rhodesia. Two secondary cases occurred, one being in the first patient's travelling companion, and the other in a nurse. Features of the illness included high fever, myalgia, vomiting and diarrhoea, hepatitis, a characteristic maculopapular rash, leucopenia, thrombocytopenia, and a bleeding te

Leveraging limited testing data for early detection of emerging infectious disease outbreaks.

Early during emerging infectious disease outbreaks, case-based surveillance is constrained by limited test availability, diagnostic delays, and low clinical suspicion. Novel pathogens that mimic symptoms of established conditions may generate detectable outbreak signals in routine testing data, as infected individuals seek testing for known conditions and test negative. We developed analytic and simulation frameworks using Poisson and negative binomial models to evaluate whether total and negative testing volumes for a clinically similar ("mimicking") condition can provide timely outbreak warn

Rwanda's lessons for strengthening Africa's response to current Marburg virus disease outbreaks.

Marburg virus disease remains a major threat to global health security in Africa, where recurrent filovirus outbreaks intersect with fragile health systems. In 2024, Rwanda faced its first Marburg virus disease outbreak, yet achieved an unusually low case-fatality rate of 23%, in contrast to the high mortality reported in the Democratic Republic of the Congo (83%), Angola (90%), Uganda (60%), and Tanzania (100%). In this Health Policy paper, we summarise the key lessons from Rwanda that can inform the responses to current and future Marburg virus disease outbreaks in Africa. Using a narrative

An index-based system for early alerts of potential zoonotic disease outbreaks.

The Information System on Wildlife Health (SISS-Geo) is a free platform designed for the real-time collection of georeferenced data on wildlife health and environmental conditions via mobile devices. It serves as a collaborative tool, enabling health professionals, researchers, environmental managers, and the general public to report information about wildlife health occurrences directly within the system. Since its launch in 2014, SISS-Geo has been successfully applied in supporting decision-making during significant wildlife health events in Brazil. In this paper, we introduce a dynamic aler

Global trends of pandemic-prone and epidemic-prone disease outbreaks in 2024.

During 2024, the number of pandemic-prone and epidemic-prone disease outbreaks worldwide was estimated at 301. The data highlight a shift in disease outbreak patterns, with a decline in the number of countries reporting public health events of concern linked to COVID-19 and a rise in those reporting outbreaks of viral diseases transmitted by vectors.About 90% of the outbreaks in 2024 were associated with COVID-19, dengue, yellow fever, Oropouche virus disease and influenza (linked to identified zoonotic or pandemic influenza virus). Although disease outbreaks can affect any country anywhere, t

Paederus Dermatitis: Two Outbreaks of an Entomologic Disease in Austere Expeditionary Care.

Paederus dermatitis, also known as dermatitis linearis, is an acute cutaneous condition caused by contact with the potent vesicant toxin paederin, produced by endosymbiotic Pseudomonas-like bacteria within Paederus spp. beetles. Paederin is a protein synthesis inhibitor that halts cell division, leading to vesiculation and necrotic breakdown of the epidermis. The resultant damage facilitates secondary infection, especially in austere conditions. Between October 2022 and July 2023, several US special operations teams forward deployed to various countries in East Africa reported over 17 cases of

Use of Open-Source Epidemic Intelligence for Infectious Disease Outbreaks, Ukraine, 2022.

Formal infectious disease surveillance in Ukraine has been disrupted by Russia's 2022 invasion, leading to challenges with tracking and containing epidemics. To analyze the effects of the war on infectious disease epidemiology, we used open-source data from EPIWATCH, an artificial intelligence early-warning system. We analyzed patterns of infectious diseases and syndromes before (November 1, 2021-February 23, 2022) and during (February 24-July 31, 2022) the conflict. We compared case numbers for the most frequently reported diseases with numbers from formal sources and found increases in overa

Pathogenic bacteria associated with outbreaks of respiratory disease in Iranian broiler farms.

BACKGROUND: Multi-causal respiratory infections are more commonly observed than uncomplicated cases with single agents in the commercial poultry industry. Recently, increased mortality rates associated with respiratory clinical signs have been reported in Iranian broiler farms. OBJECTIVES: The present study aimed to determine the spectra of avian mycoplasmas (Mycoplasma gallisepticum, MG and Mycoplasma synoviae, MS) and Ornithobacterium rhinotracheale (ORT) in the broiler farms with the multi-causal respiratory disease (MCRD) from 2017 to 2020. METHODS: Trachea and lung tissue samples were c

Syndromic surveillance to detect disease outbreaks using time between emergency department presentations.

OBJECTIVE: Early warning of disease outbreaks is paramount for health jurisdictions. The objective of the present study was to develop syndromic surveillance monitoring plans from routinely collected ED data with application to detecting disease outbreaks. METHODS: The study involved secondary data analysis of ED presentations to major public hospitals in Queensland and South Australia spanning 2017-2020. Monitoring plans were developed for all major Queensland and South Australian public hospitals using an adaptation of Exponentially Weighted Moving Averages - a process control method used i

Migration and outbreaks of vaccine-preventable disease in Europe: a systematic review.

Migrant populations are one of several underimmunised groups in the EU or European Economic Area (EU/EEA), yet little is known about their involvement in outbreaks of vaccine-preventable diseases. This information is vital to develop targeted strategies to improve the health of diverse migrant communities. We did a systematic review (PROSPERO CRD42019157473; Jan 1, 2000, to May 22, 2020) adhering to PRISMA guidelines, to identify studies on vaccine-preventable disease outbreaks (measles, mumps, rubella, diphtheria, pertussis, polio, hepatitis A, varicella, Neisseria meningitidis, and Haemophil

The nexus between forest fragmentation in Africa and Ebola virus disease outbreaks.

Tropical forests are undergoing land use change in many regions of the world, including the African continent. Human populations living close to forest margins fragmented and disturbed by deforestation may be particularly exposed to zoonotic infections because of the higher likelihood for humans to be in contact with disease reservoirs. Quantitative analysis of the nexus between deforestation and the emergence of Ebola virus disease (EVD), however, is still missing. Here we use land cover change data in conjunction with EVD outbreak records to investigate the association between recent (2004-2

Recent loss of closed forests is associated with Ebola virus disease outbreaks.

Ebola virus disease (EVD) is a contagious, severe and often lethal form of hemorrhagic fever in humans. The association of EVD outbreaks with forest clearance has been suggested previously but many aspects remained uncharacterized. We used remote sensing techniques to investigate the association between deforestation in time and space, with EVD outbreaks in Central and West Africa. Favorability modeling, centered on 27 EVD outbreak sites and 280 comparable control sites, revealed that outbreaks located along the limits of the rainforest biome were significantly associated with forest losses wi

Climate patterns and mosquito-borne disease outbreaks in South and Southeast Asia.

BACKGROUND: Vector-borne infectious diseases, particularly mosquito-borne, pose a substantial threat to populations throughout South and Southeast Asia. Outbreaks have affected this region several times during the early years of the 21st century, notably through outbreaks of Chikungunya and Dengue. These diseases are believed to be highly prevalent at endemic levels in the region as well. With a changing global climate, the impacts of changes in ambient temperatures and precipitation levels on mosquito populations are important for understanding the effects on risk of mosquito-borne disease ou

Legionnaires' Disease Outbreaks and Cooling Towers, New York City, New York, USA.

The incidence of Legionnaires' disease in the United States has been increasing since 2000. Outbreaks and clusters are associated with decorative, recreational, domestic, and industrial water systems, with the largest outbreaks being caused by cooling towers. Since 2006, 6 community-associated Legionnaires' disease outbreaks have occurred in New York City, resulting in 213 cases and 18 deaths. Three outbreaks occurred in 2015, including the largest on record (138 cases). Three outbreaks were linked to cooling towers by molecular comparison of human and environmental Legionella isolates, and th
