Surgical or endovascular repair is the cornerstone of interventional management for acute ascending aortic dissection and complicated descending aortic dissection. However, perioperative medical therapy plays a decisive role in stabilizing patients, preventing propagation of dissection and mitigating end-organ injury. Perioperative clinicians must navigate competing priorities: reducing aortic shear stress while maintaining adequate perfusion, managing severe pain and sympathetic activation, addressing malperfusion syndromes, and preparing patients for urgent or emergent intervention. This art

The clinical presentation of diseases involving the thoracic aorta ranges from a large number of asymptomatic patients with clinically undetectable thoracic aortic aneurysm to patients with symptoms of severe chest pain as a result of acute aortic dissection. Thoracic aortic disease often remains undiagnosed until a life-threatening complication occurs or the disease is discovered serendipitously on imaging studies performed for other purposes. Multidetector row computed tomography (MDCT) imaging of the aorta is used to diagnose various acute and chronic conditions, including aortic aneurysms,

Thoracic aortic dissections are a life-threatening pathology. They occur when there is an intimal tear causing separation of the layers of the aorta. Thoracic aortic dissections can be acute or chronic and depending on the pattern of the dissection can be difficult to treat. No acute dissections are the same, and herein we describe a case of a 62-male presenting with an acute thoracic aortic dissection requiring acute aorto-biiliac bypass and juxta-renal removal of aortic fenestrations.

We reviewed our experience for 6 patients of chronic dissecting aortic aneurysm (DeBakey type IIIb) with the false lumen extending into the abdominal aorta and the renal arteries being perfused from the false lumen. In three cases, whose abdominal aorta presenting with large aneurysmal lesions, we performed graft replacement of the thoracic descending aorta and abdominal aorta simultaneously. In other three cases, whose abdominal aorta presenting without aneurysmal lesions, and moreover age advanced or perioperative conditions poor, we performed graft replacement of thoracic descending aorta,

The frozen elephant trunk (FET) has been a mainstay in the treatment of acute as well as chronic aortic dissections. Although various complications have been reported in the literature, rupture of the descending thoracic aorta by the endovascular graft has, to our knowledge, not been published. We report an FET procedure following previous valve-sparing root replacement for Type A aortic dissection leading to rupture of the descending thoracic aorta.

Endovascular intervention has emerged as a less traumatic alternative treatment for several diseases of the thoracic aorta.(1,2) However, depending on the different aortic pathologies, procedure related complications have become increasingly evident: severe complications include type I endoleaks, migration, and endograft (EG) collapse, as well as those observed during conventional surgery (eg, stroke and paraplegia).(3,4) One of the emerging and most alarming complication of thoracic endografting is iatrogenic retrograde type A acute dissection (RTAAD).(5) Retrograde type A acute dissection is

Endovascular stent graft repair (EVAR) has revolutionized the management of aneurysms and dissections of the thoracic and abdominal aorta and is considered the first-line treatment in such pathologies. Initially designed for patients unfit for open repair, EVAR and thoracic endovascular aortic stent graft repair are associated with improved morbidity and mortality and a faster recovery process. The endovascular revolution of the aorta continues moving proximally, with fenestrated and branch stent grafts currently in clinical trials for the management of thoracoabdominal aortic aneurysms, and s

A 34-year-old woman with Marfan's syndrome had severe heart failure due to annulo-aortic ectasia and aortic insufficiency, which was accompanied also by a dissecting aneurysm (DeBakey type IIIb) that was demonstrated by aortography. 4 days before the operation, sudden progression of the aneurysm to a DeBakey type II, and finally DeBakey type I dissecting aneurysm was seen. The first operation was an extended aortic resection with replacement from the aortic valve to the descending thoracic aorta (level of the 7th thoracic vertebra) using selective cerebral perfusion. The second operation was a

Thirty-three patients with a variety of disorders of the thoracic aorta (aneurysm, dissection, Marfan syndrome, coarctation/pseudocoarctation, L-transposition, and Takayasu disease) were evaluated with magnetic resonance (MR) imaging. MR imaging delineated the presence and extent of thoracic aortic aneurysms and showed the relationship of the aneurysm to arch vessels; it also demonstrated intimal flaps and individual lumina in types A and B aortic dissection. Dilation of the ascending aorta in Marfan syndrome and focal narrowing of the aorta in coarctation were well visualized. The anteroposte

Thoracic aortic dissection (TAD) is the most devastating complication of vascular disease. The accuracy of the clinical diagnosis and treatment of TAD at the early stage is still limited. Herein, we report a nano-delivery strategy for early diagnosis and the first case of interleukin-33 (IL-33) based therapy for the effective intervention of TAD. A targeted fluorescent nano vector (FNV) is designed to co-assemble with IL-33, which protects IL-33 and prolongs its half-life. With specific targeting ability to the thoracic aorta, FNV can diagnose TAD at its early stage through fluorescent imaging

We describe a patient with the Marfan syndrome who presented with an acute aortic dissection. She underwent composite graft replacement of the aortic root. She returned two years later with dyspnoea and stridor due to tracheal compression by a large chronic dissection of the thoracic aorta. Marfan patients are at risk of chronic dissection involving the remaining distal aorta and require regular noninvasive assessment following surgery.

Aortic dissection (AD) is a cardiovascular emergency with an extremely high mortality rate. This review delineates its molecular pathogenesis from four pivotal dimensions: structural and functional abnormalities of the vascular wall, inflammatory and immune dysregulation, hemodynamic disturbances, and epigenetic dysregulation, while critically assessing the translational prospects and challenges of novel therapeutic strategies targeting these mechanisms. Despite continuous advancements in surgical and interventional techniques, the long-term prognosis of AD remains suboptimal. Future research

PURPOSE: To evaluate the feasibility of endoluminal stent-grafts in the treatment of acute type B aortic dissections.
METHODS: In five patients with acute aortic type B dissections, sealing of the primary intimal tear with an endoluminal stent-graft was attempted. Indication for treatment was aneurysm formation in two patients and persistent pain in three patients. One of the latter also had an unstable dissection flap compromising the ostium of the superior mesenteric artery. The distance from the intimal tear to the left subclavian artery was <0.5 cm in four patients, who had typical type B

Spin-echo MR is an established method to evaluate thoracic aortic dissections, but is not well suited to study the abdominal aorta. In this study we evaluated whether MR angiography could provide a complete examination of the abdominal aorta. In 28 patients (40 MR studies) with suspected (n = 6) or known (n = 34) aortic dissection, MR studies were performed. Thoracic aorta was evaluated with spin-echo and gradient-recalled-echo MR imaging. Axial two-dimensional time-of-flight MR angiography with thin overlapping slices was used to study the abdominal aorta. Intermediate and high signal intensi

We report an extremely rare case of multi-vessel dissection including left carotid artery, ascending aorta and thoracoabdominal aorta independently. A 65-year-old man suffered from cerebral infarction due to left carotid artery dissection. Five days later, he complained of severe back pain and was diagnosed as acute DeBakey type IIIb aortic dissection. He had been treated medically. One month later, computed tomography (CT) scan demonstrated DeBakey type II as well as type IIIb aortic dissections. The ascending aorta was replaced on August 8, 2001. Then replacement of the descending thoracic a

Aortic aneurysms and aortic dissections are major diseases affecting the thoracic aorta, with high morbidity and mortality. Thoracic aortic aneurysm (TAA) is characterized by bulge in the weakened aortic wall, which is usually asymptomatic and discovered incidentally during physical examination. Without timely intervention, approximately 50% of patients with TAA progress into aortic dissection. Thoracic aortic dissection (TAD) involves false lumen propagation from aortic intimal tear, with concomitant severe lancinating pain in the chest or back. TAA & TAD are highly similar in classification,

BACKGROUND: Stent grafting is a very important treatment for type B dissection. Some patients are unsuitable for endograft repair because of inadequate proximal and/or distal fixation zones. We reviewed our experience of proximal descending thoracic replacement combined with short-stented elephant trunk implantation for type B dissection for patients without adequate fixation zones for endografts.
METHODS: Twenty-one patients with type B dissection (10 acute, 11 chronic) underwent this procedure between August 2003 and December 2007. After replacement of the proximal descending thoracic aorta

Thoracic aortic dissection is a rare but recognized complication of crack cocaine inhalation. It is thought to be triggered in some cases by transient severe elevations in blood pressure, causing a shear effect on the thoracic aorta. Unrecognized, it can result in high morbidity and mortality. A case of an unusual presentation of thoracic aortic dissection following crack cocaine ingestion is reported.
