A computational model for retinal haemodynamics under gravitational and postural variations.

Spaceflight-associated neuro-ocular syndrome presents a significant risk during long-duration spaceflight, yet the retinal mechanisms underlying these visual changes remain poorly characterized. We developed a physiologically detailed lumped-parameter model coupling a five-compartment retinal vascular network with a dynamic intraocular pressure module responsive to hydrostatic shifts. Head-down tilt experiments were reproduced to emulate cephalad fluid shifts in microgravity. Model outputs are closely aligned with experimental benchmarks. Intraocular pressure increased from 16.3 mmHg (upright)

Gravity-driven modified whole-lung lavage improves procedural efficiency in pulmonary alveolar proteinosis.

BACKGROUND: Whole-lung lavage (WLL) remains the gold standard for symptomatic pulmonary alveolar proteinosis (PAP). However, conventional protocols are resource-intensive, relying on manual chest percussion, frequent position changes, and active negative-pressure suctioning to accelerate effluent drainage. We evaluated a modified gravity-driven workflow designed to improve procedural efficiency by optimizing hydrodynamics and minimizing manual airway manipulation. METHODS: In this single-center retrospective cohort study, we analyzed 112 WLL procedures performed in 88 patients between October

[An unusual association: tracheobronchomegaly with a normal pulmonary function test].

Tracheobronchomegaly is a rare condition characterised by marked dilation of the trachea and the main bronchi. The clinical presentation of this disease is nonspecific and the diagnosis is based on the radiological features, especially computed tomography of chest. Pulmonary function tests are often abnormal showing airflow limitation with increased residual volume. The authors report a rare case of a 31-year-old man presenting tracheobronchomegaly is normal pulmonary function test.

Varicocele is the root cause of BPH: Destruction of the valves in the spermatic veins produces elevated pressure which diverts undiluted testosterone directly from the testes to the prostate.

In varicocele, there is venous flow of free testosterone (FT) directly from the testes into the prostate. Intraprostatic FT accelerates prostate cell production and prolongs cell lifespan, leading to the development of BPH. We show that in a large group of patients presenting with BPH, bilateral varicocele is found in all patients. A total of 901 patients being treated for BPH were evaluated for varicocele. Three diagnostic methods were used as follows: physical examination, colour flow Doppler ultrasound and contact liquid crystal thermography. Bilateral varicocele was found in all 901 patien

Traumatic intraventricular hemorrhage treated with intraventricular recombinant-tissue plasminogen activator: technical case report.

OBJECTIVE AND IMPORTANCE: Traumatic intraventricular hemorrhage (IVH) can result in association with acute obstructive hydrocephalus, repetitive malfunction of external ventricular drains (EVDs), and uncontrollable increased intracranial pressure. We report a case showing the safe and effective use of intraventricular recombinant-tissue plasminogen activator in a child with severe brain injury and acute hydrocephalus from IVH. CLINICAL PRESENTATION: A 15-year-old male patient presented to us after a motor vehicle accident with bilateral extensor posturing, intracerebral and IVH, and acute obs

Demonstration of postural effects and layering by cranial computed tomography.

The superb contrast resolution of computed tomography permits visualization of layering of fluid components of varying densities in tumors and hematomas which cannot be observed with conventional radiographic techniques. Layering confirms the fluid nature of a cystic tumor and suggests the possibility of draining the fluid contents in an effort to reduce mass effect. Verification of layering is achieved by noting shifting of the dependent material with axial rotation of the patient. While unusual, this finding is significant because solid tumors occasionally show low densities simulating fluid

Emptying of the intrathoracic stomach using three different pylorus drainage procedures--results of a comparative study.

Patterns of gastric emptying in the vagotomized intrathoracic stomach (used for esophageal replacement) were studied using radioisotope techniques. Following esophagectomy and gastric mobilization, the patients were randomized into three groups: group 1, pyloroplasty; group 2, pyloromyotomy; and group 3, pylorus stretching. A total of 30 patients surviving the operation and who were still alive at least 3 months afterwards were included in this study. Gastric emptying (GE) was evaluated 6-8 weeks after the operation. The mean GE time for liquids was 3.3 +/- 2.7, 4.1 +/- 3.1, and 5.5 +/- 4.3 mi

Respiratory Physiotherapy Techniques in Neonatal Intensive Care Units: A Scoping Review.

BACKGROUND AND PURPOSE: Certain physiological characteristics increase the susceptibility of newborns, particularly preterm infants, to pulmonary collapse. In addition, newborns are prone to the accumulation of airway secretions, which, even in small volumes, can compromise respiratory function. Consequently, breathing therapy plays a crucial role in promoting airway clearance and supporting effective ventilation in this population. This scoping review aimed to map the breathing therapy methods used for newborns admitted to intensive care units and also to identify the clinical outcomes assess

A left thoracic approach in a prone position for thoracoscopic thoracic duct ligation in a patient with post-esophagectomy chylothorax: A case report.

INTRODUCTION: We debate whether or not to approach from right thorax for the left chylothorax after esophagectomy. PRESENTATION OF CASE: A 50s-year-old female underwent right-sided thoracoscopic esophagectomy with three-field lymphadenectomy for esophageal carcinoma (type 0-IIa, 3.4×2.2cm, T1bN0M0, Stage IA), followed by reconstruction with esophagogastric anastomosis through the posterior mediastinum. The thoracic duct was excised and ligated. The left thoracic drainage increased to 2115mL/day on the fifth postoperative day. Thoracic duct injury was diagnosed, and surgery was performed on si

Pulmonary complications of general surgery.

1. Patients should be divided preoperatively into low- or high-risk categories, depending on their probability of developing postoperative pulmonary complications. The evaluation should include spirometry as well as an assessment of the previously defined risk factors. 2. Patients in a low-risk category need only instruction in deep breathing pre- and postoperatively. Routine use of supplemented oxygen postoperatively is reasonable until it can be demonstrated whether such is necessary. 3. High-risk patients should be as free as possible of respiratory secretions at the time of surgery. A regi

Electrical Impedance Tomography-Guided Precision Nursing in an Older Patient With Septic Shock and ARDS: A Case Report.

Septic shock complicated by acute respiratory distress syndrome (ARDS) and multiple organ dysfunction syndrome (MODS) presents a fundamental therapeutic conflict: shock demands aggressive fluid resuscitation, whereas lung protection requires fluid restriction and a negative fluid balance. We report the precision nursing management of a 79-year-old patient with septic shock, ARDS and MODS who received invasive mechanical ventilation and continuous renal replacement therapy (CRRT). A closed-loop 'monitoring-decision-intervention' strategy was built around electrical impedance tomography (EIT): E
