Auto brewery syndrome from the perspective of the neurologist.

BACKGROUND AND PURPOSE: Auto-brewery syndrome (ABS) is a rare condition that causes the digestive system to produce intoxicating amounts of alcohol due to abnormal growth of the gut microbiota. Medicolegal inferences of ABS have two distinct edges. First, malingering in drunk-driving and abusing the syndrome as a factitious disorder may occur. Second, patients suffering from the syndrome may face medicolegal and social problems. Moreover, chronic exposure to alcohol due to undiagnosed ABS might result in cognitive and behavioral disturbances. Here, we present a patient with recurrent encephalo

Association between legalization of recreational cannabis and fatal motor vehicle collisions in the United States: an ecologic study.

BACKGROUND: With the recent legalization of recreational cannabis in Canada, cannabis-impaired driving is an important public safety concern. Our aim was to examine the association between recreational cannabis legalization and fatal motor vehicle collisions using data from the United States, which present a timely natural experiment of cannabis legalization. METHODS: We conducted an ecologic study using the number of fatal motor vehicle collisions and the associated number of deaths for US jurisdictions with legalized recreational cannabis (2007-2018) retrieved from the US Fatality Analysis

An Autopsy Case of Injuries Caused by Automobile Dragging for a Distance of 3.4 km Without Thoracoabdominal Organ Injuries.

Dragging injuries caused by automobiles involved in an accident are occasionally reported; however, those without injury to the thoracoabdominal organs are rare. We present an autopsy case of a man in his 20s found dead on the road after being dragged by a passenger automobile for a distance of 3.4 km. The accident caused fatal dragging injuries but no thoracoabdominal injuries. The victim appeared to be drunk and was lying on the road in front of the parked automobile. Dragging injuries are a unique type of traffic accident that sometimes result from limited and poor visibility due to darknes

Three-year review of tetrahydrocannabinol and alcohol blood concentrations and effects in impaired driving investigations in Ontario, Canada.

Driving under the influence of cannabis is a significant public safety issue. Tetrahydrocannabinol (THC), the main psychoactive compound in cannabis, can impair driving ability and when used in combination with alcohol, the level of impairment can increase. Case studies documenting blood concentrations of THC and associated observations of impairment in individuals suspected of impaired driving continue to provide important contributions to the scientific literature. Retrospective analysis was performed using data from impaired driving investigations in which THC alone, and in combination with

The Role of Parenting Behaviors and Their Influence on Adolescent Drunk and Drugged Driving: 2016-2019, USA.

Drugged driving, the act of driving a vehicle under the influence of illicit drugs, by adolescents is a serious public health concern. Many factors contribute to this risk behavior, but much less is known regarding the role of parenting behaviors in this phenomenon. The purpose of this study was to examine specific parenting behaviors and their influence among a nationally representative sample of adolescents. Pooled data from the 2016-2019 National Survey on Drug Use and Health (NSDUH) among 17,520 adolescents ages 16-17 years old were analyzed. Differences were found in specific parenting be

Event-level influences of alcohol, cannabis, and simultaneous use on perceived driving risk.

Alcohol-impaired driving is highly prevalent and a leading cause of death. Cannabis is commonly used among people who drink alcohol, and using alcohol and cannabis simultaneously is associated with a greater frequency of alcohol-impaired driving. Laboratory studies demonstrate the harmful effects of simultaneous use on driving ability compared to alcohol use alone, yet driving under the influence of cannabis is perceived as a low risk. We tested the influences of alcohol, cannabis, and their simultaneous use on perceived driving impairment and willingness to drive in daily life. Participants w

Toxicological and Demographic Profiles of Phencyclidine-Impaired Driving Cases in Houston.

Phencyclidine (PCP) was first synthesized in 1926 and originally developed in 1950s as a general anesthetic agent. Abuse of PCP declined at the national level since its first illicit use in 1960s, but it continues in certain areas including Houston. This research evaluates PCP-positive cases of driving while intoxicated (DWI) in 2013-2018. The blood samples were collected from drivers, submitted by the Houston Police Department and analyzed for alcohol and drugs. Toxicological findings and demographic information were evaluated for the impaired driving cases tested positive for PCP in blood. A

Bystanding to prevent drink-driving: Young adults' descriptions of factors that promote and restrict their behavior.

INTRODUCTION: Drink driving remains a leading contributor to motor-vehicle fatalities. For young adults, a sizeable proportion of their drinking is with friends, who may have opportunity to plan ahead or potentially intervene and speak up against drink-driving. We sought to explore young adults' preventive behaviors toward friends and the potential relevance for the classic Latané and Darley social psychology theory of bystander intervention. METHOD: Across two studies we interviewed 63, 18-25-year-old Australians, using semi-structured interview schedules (approximately 30-45 min). Study 1 f

Reliability of roadside oral fluid testing devices for ∆9-tetrahydrocannabinol (∆9-THC) detection.

Driving under the influence of cannabis (DUIC) is increasing worldwide, and cannabis is the most prevalent drug after alcohol in impaired driving cases, emphasizing the need for a reliable traffic enforcement strategy. ∆9-tetrahydrocannabinol (THC) detection in oral fluid has great potential for identifying recent cannabis use; however, additional data are needed on the sensitivities, specificities, and efficiencies of different oral fluid devices for detecting cannabinoids at the roadside by police during routine traffic safety enforcement efforts. At the roadside, 8945 oral fluid THC screeni

Bromazolam in impaired driving investigations.

The designer benzodiazepine bromazolam is increasingly encountered in forensic casework, including impaired driving investigations. A series of suspected impaired driving cases that tested positive for bromazolam are described herein along with information about driving performance, driver appearance, and observed behavior. Bromazolam was indicated in casework either through screening by liquid chromatography-time of flight mass spectrometry (LC-TOF-MS) and/or a positive benzodiazepine immunoassay screen. Blood samples were forwarded for quantitative confirmatory analysis using a liquid chroma

Drinking and driving relapse: Data from BAC and MMPI-2.

Road traffic injuries are the ninth cause of death across all age groups, globally (WHO, 2015). Many road traffic crashes are caused by Driving Under the Influence (DUI) of alcohol by persons who have previously had their license suspended for the same reason. The aim of this study was to identify specific risk factors and personality characteristics in repeat offenders. The sample was comprised of 260 subjects who were not repeat DUI offenders (DUI-NR), but had a single license suspension between 2010 and 2011; and 97 repeat offenders who received at least two DUI convictions within a period

Drinker driver flyer diver.

Blood alcohol concentrations above defined levels are detrimental to cognitive performance. Empirical and published evidence suggest that nitrogen narcosis is analogous to alcohol intoxication with both impairing prefrontal cortex function. Nitrogen narcosis is also known to have been a factor in fatal accidents. To examine the effects of nitrogen narcosis, a recent publication used the Iowa Gambling Task tool, to simulate dynamic real-life risky decision-making behaviour. If the reported outcomes are corroborated in larger rigorously designed studies it is likely to provide further evidence t

Does the Quantification of Δ9-Tetrahydrocannabinolic Acid A in Serum/Plasma Provide Any Additional Information About Consumption Pattern from Drivers Under the Influence of Cannabis?

Introduction: Δ9-tetrahydrocannabinolic acid A (THCA-A) is one of the main ingredients of cannabis plants and is converted to the psychoactive substance Δ9-tetrahydrocannabinol (THC) by decarboxylation during heating above ∼90°C. During the consumption of cannabis, a varying proportion of THCA-A is absorbed into the body. Therefore, the quantification of THCA-A in serum/plasma might provide additional information on consumption behavior in driving under the influence of cannabis cases. Materials and Methods: In this study, an already established gas-chromatography mass-spectrometry (GC-MS) met

North America laboratory survey data for drug testing in drug-impaired driving and traffic fatality investigations.

In 2004, the National Safety Council's Alcohol, Drugs, and Impairment Division set out to provide guidance for the standardization of laboratory testing practices in driving under the influence of drugs and fatal motor vehicle crash investigations after identifying a lack of consistency in testing practices in this type of casework. A survey about laboratory testing practices, scopes of testing, and cutoffs was created using SurveyMonkey®, an online survey instrument, and sent to laboratories throughout the USA and Canada. Based on the analysis of survey results and discussion, the first set o
