The effect of antacid and mineral supplements on bictegravir pharmacokinetics: results from a Phase 1, open-label, drug-drug interaction study.

The mechanism of action of integrase strand transfer inhibitors involves binding to magnesium ions in the active site of the HIV integrase enzyme, making them susceptible to chelation-type drug-drug interactions with metal cation-containing medications. This study evaluated the potential of metal cation-containing antacids and mineral supplements to impact bictegravir (BIC) exposure and assessed alternative approaches for combined use. This was an open-label, single-dose, Phase 1 study in adult participants without HIV. The pharmacokinetics and safety of BIC (administered as part of a single-t

The safety of double- and triple-drug community mass drug administration for lymphatic filariasis: A multicenter, open-label, cluster-randomized study.

BACKGROUND: The Global Programme to Eliminate Lymphatic Filariasis (GPELF) provides antifilarial medications to hundreds of millions of people annually to treat filarial infections and prevent elephantiasis. Recent trials have shown that a single-dose, triple-drug treatment (ivermectin with diethylcarbamazine and albendazole [IDA]) is superior to a two-drug combination (diethylcarbamazine plus albendazole [DA]) that is widely used in LF elimination programs. This study was performed to assess the safety of IDA and DA in a variety of endemic settings. METHODS AND FINDINGS: Large community stud

DALES, Drug Allergy Labels in Elective Surgical patients: a prospective multicentre cross-sectional study of incidence, risks, and attitudes in penicillin de-labelling strategies.

BACKGROUND: Penicillin allergy is associated with a range of poor health outcomes. Allergy testing can be made simpler by using a direct drug provocation test in patients at low risk of genuine allergy. This approach could allow population-level 'de-labelling'. We sought to determine the incidence and nature of penicillin allergy labels in UK surgical patients and define patient and anaesthetist attitudes towards penicillin allergy testing. METHODS: A prospective cross-sectional questionnaire study was performed in 213 UK hospitals. 'Penicillin allergic' patients were interviewed and risk-str

Drug Reaction With Eosinophilia and Systemic Symptoms and Severe Drug-Induced Liver Injury After Off-Label Zonisamide Use for Weight Loss.

Drug-related eosinophilia with systemic symptoms (DRESS) is a drug-induced hypersensitivity reaction causing rash and systemic symptoms. Associated liver injury ranges from abnormal laboratory results to liver failure. We report a case of zonisamide-induced DRESS with severe liver injury, characterized by vanishing bile duct syndrome. Despite stopping zonisamide and initiating immunosuppressive therapy, the patient's hepatic function remained abnormal. After a prolonged course, the patient died from pneumonia and Hodgkin lymphoma. This case highlights the gravity of DRESS syndrome and drug-ind

When coverage is not access: A heart transplant recipient's perspective on everolimus, U.S. Food and Drug Administration labeling, and the problem of "ghost approval".

Access to immunosuppressive therapy is essential for long-term survival in heart transplant recipients, yet insurance coverage does not always translate into meaningful access. This perspective introduces the concept of "ghost approval," in which a medication is technically approved but remains financially or logistically inaccessible to the patient. Drawing on a first-person experience with everolimus, an mechanistic target of rapamycin inhibitor without a U.S. Food and Drug Administration indication for heart transplantation, this article highlights the gap between regulatory labeling, clini

Label-Free Visualization of the Antifungal Polyene Drug, Nystatin, in Biological Membranes Using Raman Microscopy.

Polyene macrolides are a class of antifungal drugs that remain essential for treating life-threatening fungal infections. The complex relationship between the chemical structure of the polyene macrolide, its interaction with biological membranes and subsequent intracellular localization, is poorly understood. Nystatin is an archetypal polyene macrolide, which has been in clinical use for the treatment of oral and dermal Candida albicans infections over several decades. To investigate the interaction of nystatin with biological membranes, we applied label-free stimulated Raman scattering (SRS)

Off-label drug use in China after the Physician Law (2021): legal challenges and solutions.

INTRODUCTION: In 2021 China enacted the Physician Law, which specifically outlines the legal requirements for off-label drug use in Article 29. Legislators, medical institutions, and judicial authorities all hoped that this new regulation would effectively reduce illegal off-label drug use. However, many violations still occurred in medical practice even after the enactment of the Physician Law, leading to judicial cases. This article examines the current state of off-label drug use in China, and the legal liability risks faced by medical institutions. METHODS: This study investigates data fr

The Evaluation of Interactions Between Multiple Intrinsic and Extrinsic Factors Reported in Labeling for FDA-Approved Drugs.

A critical aspect of drug development is evaluating pharmacokinetics (PK) and pharmacodynamics to assess how intrinsic (e.g., age, sex, comorbidities, and genomics) and extrinsic (e.g., drug-drug interactions [DDIs] and food interaction) factors influence drug exposure and response. These aspects guide dose selection and inform drug labeling by accounting for interindividual variability to ensure safe and effective use across complex patient populations. However, identifying the relationship of co-occurring factors, resembling complex patient populations, remains challenging. Herein, we analyz

Cardiac safety of off-label COVID-19 drug therapy: a review and proposed monitoring protocol.

More than 2,000,000 individuals worldwide have had coronavirus 2019 disease infection (COVID-19), yet there is no effective medical therapy. Multiple off-label and investigational drugs, such as chloroquine and hydroxychloroquine, have gained broad interest due to positive pre-clinical data and are currently used for treatment of COVID-19. However, some of these medications have potential cardiac adverse effects. This is important because up to one-third of patients with COVID-19 have cardiac injury, which can further increase the risk of cardiomyopathy and arrhythmias. Adverse effects of chlo
