Inner ear inflammation and round window otosclerosis.

Recently, it has been suggested that otosclerosis represents the host's ongoing immunologic response to measles or other viral antigens. Documentation of past inflammation within the inner ear would serve as further evidence that this mechanism may be at play in the pathogenesis of the disease. Among the characteristic signs of prior inflammation in the inner ear is the presence of lamellar bone at the site of inflammation. This has been described in the temporal bone of a patient with immune-mediated deafness and with the temporal bones of experimental models of immune-mediated inner ear dise

Clinical diagnosis of immune inner-ear disease.

The purpose of this study was to develop a high-risk clinical profile of patients with autoimmune (immune-mediated) inner-ear disease. The records of 52 patients diagnosed over the past 5 years were reviewed. Age, sex, bilateral versus unilateral involvement, otologic symptoms, concomitant systemic immune disease, and presenting clinical diagnoses were recorded. The presenting diagnoses were Cogan's syndrome, Meniere's syndrome, Dandy's syndrome without hearing loss, or progressive sensorineural hearing loss without dizziness. Because Cogan's and Dandy's syndromes were relatively uncommon, the

Animal models of inner ear vascular disturbances.

Animal models of vascular disorders are identified or developed for the evaluation of functional deficits and morphologic alterations. This information will serve a useful purpose for a better understanding of sudden deafness, Meniere's disease, and presbycusis. The study of microcirculation of the inner ear vessels reveals that their responses to various stimuli, such as anoxia, sympathetic nerve stimulation, hypothermia, and drugs, are different from those of the middle ear vessels. In sudden occlusion of the major vascular supply to the inner ear, the cochlea is found to be more vulnerable

Inner ear fluids dynamics and endolymphatic hydrops.

The inner ear possesses remarkably stable homeostatic mechanisms for the maintenance of the functional integrity of the inner ear end-organs. The inner ear fluid maintains its homeostasis by a variety of subtle regulatory mechanisms, both locally and systemically. Any disturbance in one of these mechanisms can induce the disruption of homeostasis expressed by ionic, osmotic, or metabolic imbalance between the compartments. This can be manifested as membrane displacement or abnormal functions of the inner ear, depending on the degree of the disturbance. Further studies are necessary to clarify

Inner ear pathology associated with Reye's syndrome.

Severe pathological changes were observed in the inner ear tissues of a 2-month-old patient who died of Reye's syndrome after 5 days of hospitalization. In the organ of Corti, the inner hair cells appeared to be more severely damaged than the outer hair cells. Various degrees of degeneration were observed in all non-sensory epithelial cells lining the cochlear duct. In most turns of the cochlear duct, Reissner's membrane was ruptured and/or collapsed onto the organ of Corti. Likewise, both sensory and non-sensory cells of the vestibular end organs were markedly degenerated. These observations

Unilateral cystic inner ear anomalies in siblings.

Unilateral cystic inner ear anomalies were diagnosed in two siblings, a 9 year old boy and a 6 year old girl. X-ray examination of the temporal bone was performed, together with audiological examinations and vestibular function tests. The common tomographic X-ray findings consisted of an enlarged solitary sac type deformity of the vestibule with narrowing of the internal auditory canal, severe hypoplasia of the anterior semicircular canal and no visualized cochlea. Pure-tone audiometry revealed severe mixed type of hearing loss in the right ear in both children. The test for vestibular functio

Inner ear damage induced by bacterial endotoxin.

Repeated i.d. injection doses of endotoxin lipopolysaccharide of E. coli at certain time intervals were found to give rise to the following changes in the inner ear in mice: In the organ of Corti of the cochlea, there were noted degeneration and disappearance of the inner and outer hair cells of the lower turn, partly with a partial disappearance of nerve fibers in the osseous spiral lamina. Degenerative changes seen in the upper turn were of milder degree. The stria vascularis exhibited enlarged intercellular spaces, the appearance of lysosomes in the constituent cells, and degeneration there

Viral causes of sudden inner ear deafness.

Clinical, pathologic, and laboratory studies suggesting a viral etiology for sudden deafness have been presented. Although a cause and effect relationship has not been proven, the frequent close association between viral infection and sudden deafness constitutes a strong argument in favor of a viral etiology of sudden deafness in about one of three patients with sudden deafness.

[Surgery of inner ear diseases (author's transl)].

In the treatment of Menière's disease, surgical procedures are available when conservative measures have failed. The most distressing symptom of vertigo can frequently be arrested, tinnitus reduced and hearing improved. Sudden deafness due to round window rupture is treatable. The most important disorder with labyrinthe symptoms is the acoustic neuroma, the smaller ones of which can be operated on by otologists in collaboration with neurosurgeons using the transtemporal or translabyrinthine approach with preservation of the facial nerve.

Induction of an inner-ear-specific autoreactive T-cell line for the diagnostic evaluation of an autoimmune disease of the inner ear.

Different patterns of sensorineural hearing loss with a potential improvement in auditory function following immunosuppressive therapy might be caused by an isolated autoimmune disease of the inner ear. Because of the lack of well-defined detection methods to identify autoimmune processes within the inner ear and the fact that the human inner ear is one of the few organs of the body not amenable to diagnostic biopsy, there has been great interest in developing animal models. Previous studies found evidence that this entity might be cellular mediated. By heterologeous immunization of inbred Lew

[A study on serum autoantibodies against inner ear tissue in the blood of patients with inner ear disorders of unknown etiology].

Membranous labyrinth antigen of guinea pigs was prepared. The sera of 33 patients with inner ear disorders of unknown etiology were examined by immunoblotting assay. The autoantibodies against inner ear antigen were found in patient's sera (30.3%). This indicated that the autoimmune activity may play a role in the development of some cases of these diseases. The detection of autoantibodies to inner ear antigen will be of great value to further study on the action of autoimmunological factor contributing to inner ear disorders of unknown etiology.

Inner Ear Organoids: Recent Progress and Challenges.

Hearing loss is a prevalent sensory disorder primarily caused by the irreversible loss of cochlear hair cells and the limited regenerative capacity of the mammalian inner ear. The structural complexity and inaccessibility of inner ear tissues have long hindered mechanistic studies and therapeutic development. In recent years, inner ear organoids have emerged as a robust in vitro platform that recapitulates key aspects of inner ear development, cellular composition, and functional organization. Derived from pluripotent stem cells or tissue-specific progenitors, inner ear organoids exhibit intri

Recurrent Meningitis in Congenital Inner Ear Malformation.

Spontaneous cerebrospinal fluid otorrhea is very rare. Because of nonspecific symptoms, it is usually diagnosed when complications such as meningitis occur. Cerebrospinal fluid leak may be caused by cochlea malformation, which permits nonphysiological communication between subarachnoid space and tympanomastoid cavity. Nearly 20% of congenital sensorineural hearing loss is connected with inner ear bone malformation. We present a case of 40-year-old man suffering since early childhood from recurrent meningitis and right ear deafness, caused by congenital internal ear malformation. For many years

Cogan's syndrome: an autoimmune inner ear disease.

OBJECTIVES: The objective of our study was to review our current knowledge of the aetiopathogenesis of Cogan's syndrome, including viral infection and autoimmunity, and to discuss disease pathogenesis with relevance to pharmacotherapy. SYSTEMATIC REVIEW METHODOLOGY: Relevant publications on the aetiopathogenesis and pharmacotherapy of Cogan's syndrome from 1945 to 2012 were analysed. RESULTS AND CONCLUSIONS: Cogan's syndrome is a rare autoimmune vasculitis, and its pathogenesis is unknown. Infection, but primarily autoimmunity, may play contributing roles in the pathogenesis of this disease.

Recent advances in viral inner ear disorders.

PURPOSE OF REVIEW: To highlight the recent advances in the understanding of the diagnosis and management of viral inner ear disorders. Congenital sensorineural hearing loss (cSNHL), sudden sensorineural hearing loss (SSNHL), Ménière's disease, and vestibular neuritis/viral labyrinthitis are discussed. RECENT FINDINGS: Cytomegalovirus infection during pregnancy is an under-recognized cause of hearing loss and central nervous system disease amongst the general population. Prevention of maternal infection and treatment of affected newborns with ganciclovir are promising interventions. Recent evi
