Combined reconstruction of congenital auricular atresia and severe microtia.

OBJECTIVES: Due to their embryological development, auricular atresia and severe microtia are, in most cases, combined malformations. The aims of this study were firstly to develop a surgical technique for combined esthetic and functional reconstruction with a minimum of operations and secondly to evaluate its results. STUDY DESIGN: Prospective clinical evaluation. PATIENTS AND METHODS: Fifty-two patients with third-degree microtia and congenital aural atresia with a sound-conducting block of about 50 dB were treated. In the first operation, autogenous cartilage was harvested, and the auricu

Evaluation of reconstructed auricles by convolutional neural networks.

The difficulty in determining which structures are crucial to ensure a natural-looking ear has been plaguing surgeons for many years. This preliminary study explores the feasibility of training convolutional neural network (CNN) models to evaluate a reconstructed auricle as accurate as a human would. By visualizing the attention of trained models, the criteria for the design of a natural-looking auricle can be established. A total of 400 pictures were evaluated by 20 volunteers, and 20 labeled datasets were generated, which were then used to train ResNet models that had been pre-trained on Ima

Hereditary hidradenitis suppurativa restricted to the auricular region.

Hidradenitis suppurativa (HS) is part of the follicular occlusion tetrad, which typically presents as recurrent deep-seated nodules, abscesses, and communicating sinus tracts with a predilection for the axilla and genital region. There is only one prior report of HS restricted to the auricular region. Herein the first familial case of HS restricted to the auricular region is reported.

Surgical management of bilateral ear pinna lesions associated with traumatic aural hematoma in a 3-day-old goat kid.

BACKGROUND: Aural hematomas are not uncommon in ruminants' clinical practice; however, there is a lack of information regarding their management in newborn ruminants, especially for complicated cases with rupture of the hematoma and secondary ear pinna necrosis. CASE DESCRIPTION: A 3-day-old orphan goat kid was admitted due to swelling on the left ear pinna and trauma on the right pina caused by biting by other goats. The swelling on the left ear which was located at the convex surface was soft, painless, and fluid-filled, suggestive of aural hematoma located at the convex surface. The right

Costal Cartilage Assessment in Surgical Timing of Microtia Reconstruction.

BACKGROUND: Rib cartilage growth is closely related to age and determines the feasibility and outcomes of auricular reconstruction. Ear reconstruction is performed as early as age 6 in efforts to treat children before school matriculation while allowing for sufficient rib growth. But there is controversy over the optimal timing of microtia reconstruction. This study employs CT imaging and surface scanning to guide the timing of auricular reconstruction in children. METHODS: A retrospective analysis was performed on 6-year-old microtia patients between January 2016 and June 2016. A total of 37

A rare case of petrified ear.

Calcification or ossification of the auricle, also referred to as petrified ear, is a rare diagnosis in dermatology. In medical literature, it has most often been attributed to trauma, hypothermia and frostbite, or hypercalcemia secondary to a metabolic or endocrine disorder, such as Addison's disease. Here, we report the clinical and radiologic findings of a 79-year-old African American male whose unilateral petrified auricle was an incidental finding. He had a preceding history of hyperparathyroidism and subsequent hypercalcemia treated with a subtotal parathyroidectomy three years prior to

Correction of congenital microtia using the tissue expander.

We attempted auricular reconstruction using Radovan-type inflatable silicone expanders in six children and one adult, with the complete hypoplastic, the conchal remnant, and constricted type of microtia. Ear frameworks, including the helix, anthelix, concha, and tragus, were prepared using autologous rib cartilage. Based on the surface area of the normal adult auricle, the silicone expander was tentatively shaped and sized into a rotated semiellipse and expanded with 70 cc saline. Auricular reconstruction on the framework was completed at the time of insertion in four of the seven patients, re

Congenital periauricular fistulas: possible variants of the preauricular sinus.

OBJECTIVE: Although most preauricular sinuses are located near the anterior limb of the ascending helix, some are located in unusual areas around the auricle, as shown previously in limited reports. This study analyzed the clinical manifestations of congenital periauricular fistulas with unusual fistula locations and the possible relationship with the classical preauricular sinus. METHODS: We reviewed the medical records of patients who underwent congenital periauricular sinus excision by three surgeons and followed them for more than 6 months. Clinical manifestations were compared between cl

Total auricular reconstruction after traumatic total amputation of the auricle.

We present a case of successful total auricular reconstruction after total amputation of the auricle. Owing to the severe damage to the auricular cartilage, replantation was not attempted; instead, for the next step of the reconstruction, severed superficial temporal artery and vein were anastomosed. At the second stage, total auricular reconstruction was planned. A huge temporoparietal fascial flap was used (based on STA/STV), which was used to cover the costal cartilage ear framework. This case suggests that, even for the total loss of the auricle, autogenous reconstruction can still be succ

The importance of costal cartilage framework stabilization in microtia reconstruction: anthropometric comparison based on 216 cases.

This study explored anthropometric changes in the reconstructed auricle and the contralateral normal ear in a series of 216 microtia patients using different stabilization methods. Our main personal modifications concerning the preparation of the framework were the following: 1. The individualized framework grafting was based on patients with different ages and different degrees of the strength and thickness of the rib cartilage. 2. The framework was stabilized as a "C" shape by using a piece of cartilage or suture to reinforce the two end points of the "C". In group A (the thickness of cartil

[Angiolymphoid hyperplasia with eosinophilia].

INTRODUCTION: Angiolymphoid hyperplasia with eosinophilia (HALE) is a rare, benign proliferative vascular disorder of unknown etiology which typically presents as tumor-like nodules of the skin or subcutaneous tissue. It concerns mostly young female adults. We report a case of HALE located on the ear auricle diagnosed by biopsy. The outcome was total regression. OBSERVATION: An 81-year-old male patient presented with a lesion of the whole ear auricle. A diagnosis of HALE was confirmed histologically on biopsy. The evolution was marked by spontaneous and complete regression in 2 months. There

Age- and sex-related changes in the normal human ear.

The objective of this study was to supply information about: (1) normal sex-related dimensions of ears (linear distances and ratios, area); (2) left-right symmetry; and (3) growth changes between childhood and old age. The three-dimensional coordinates of several soft-tissue landmarks on the ears and face were obtained by a non-invasive, computerized electromagnetic digitizer in 497 male and 346 female healthy subjects aged 4-73 years. From the landmarks, paired ear width and length, the relevant ratios, ear areas and angles relative to the facial midline, as well as indices of left-right symm

Human-engineered auricular reconstruction (hEAR) by 3D-printed molding with human-derived auricular and costal chondrocytes and adipose-derived mesenchymal stem cells.

Microtia is a small, malformed external ear, which occurs at an incidence of 1-10 per 10 000 births. Autologous reconstruction using costal cartilage is the most widely accepted surgical microtia repair technique. Yet, the method involves donor-site pain and discomfort and relies on the artistic skill of the surgeon to create an aesthetic ear. This study employed novel tissue engineering techniques to overcome these limitations by developing a clinical-grade, 3D-printed biodegradable auricle scaffold that formed stable, custom-made neocartilage implants. The unique scaffold design combined str

Modification of the second stage of auricular elevation in Nagata's technique for treating microtia.

The ultimate goal in treating congenital microtia is the reconstruction of an auricle with a natural appearance as close as possible to the healthy one in terms of shape, contour, size, texture, position, and projection. Here we present another option to the second stage of raising the ear in Nagata's technique for treating microtia. The first step includes implantation of a cartilaginous framework and transposition of the ear lobe. Raising the cartilage leaves a skin defect behind the auricle. Instead of using a superficial temporoparietal fascia flap as described by Nagata, we raise a simple
