A new corrective method for the Tanzer's group IIB constricted ear: Helical expansion using V-Y advancement flap combined with concha cartilage graft.

BACKGROUND: The complexity of a constricted ear shape renders the aim of establishing a uniform surgical method unattainable, thus posing an ongoing challenge in its correction. The Tanzer's group IIB constricted ear is characterized by a prominent downward folding, an underdeveloped antihelix, and the absence of sacpha. The present study used a V-Y advancement flap combined with concha cartilage for the repair of Tanzer's group IIB constricted ear. METHOD: A total of 16 patients diagnosed with type IIB ear constriction from September 2016 to September 2022 were enrolled in this retrospective

Cartilage-Sparing Otoplasty: The Effects of Adipo-Perichondrial Flap-Assisted Posterior Auricular Muscle Complex Flap Technique on the Repair of Prominent Ear Deformities.

BACKGROUND: In the repair of prominent ear deformities, the main objectives are to recreate the antihelical fold, reduce projection in patients with a large concha, and normalize the distance between the ear and the cranium. This study evaluated the efficacy of the adipo-perichondrial flap-assisted posterior auricular muscle complex (PAMC) flap technique, which involves a new approach to the repair of prominent ear deformities. MATERIALS AND METHODS: A medially based PAMC flap and a laterally based adipo-perichondrial flap were elevated in 22 patients with prominent ear deformities. In order

Efficacy and safety of ultrasound-guided bilateral superficial serratus anterior plane block for postoperative analgesia in pediatric ear reconstruction with costal cartilage harvest: a randomized controlled trial.

BACKGROUND: Patients with microtia undergoing ear reconstruction with costal cartilage harvest often experience significant postoperative chest pain. The efficacy and safety of ultrasound-guided superficial serratus anterior plane block (SSAPB) in pediatric patients remain unclear. METHODS: In this randomized controlled trial, sixty children were randomized to SSAPB or incision infiltration anesthesia (IA) (30 ml of 0.25% ropivacaine each). PRIMARY OUTCOME: group × time interaction on pain NRS trajectories (rest/coughing) over 48 h. Secondary outcomes were patient-controlled intravenous anal

Congenital microtia patients: the genetically engineered exosomes released from porous gelatin methacryloyl hydrogel for downstream small RNA profiling, functional modulation of microtia chondrocytes and tissue-engineered ear cartilage regeneration.

BACKGROUND: Mesenchymal stem cells (MSCs) exosomes were previously shown to be effective in articular cartilage repairing. However, whether MSCs exosomes promote mature cartilage formation of microtia chondrocytes and the underlying mechanism of action remains unknown. Additionally, some hurdles, such as the low yield and unsatisfactory therapeutic effects of natural exosomes have emerged when considering the translation of exosomes-therapeutics to clinical practices or industrial production. Herein, we investigated the roles of human adipose-derived stem cells (ADSCs) exosomes in modulating m

Safety and efficacy of connecting the residual ear to the cartilage scaffold in the first-stage versus second-stage surgery: a protocol of a randomised controlled trial in non-expanded auricular reconstruction for concha-type microtia.

INTRODUCTION: Concha-type microtia, classified as a milder form of microtia under Nagata's classification, is characterised by the presence of more normal ear subunits compared with other types. The non-expanded auricular reconstruction using autologous rib cartilage is a standard procedure treating conchal microtia, and this operation typically comprises two stages. To maximise the utilisation of native auricular cartilage, the lower half of a patient's ear is preserved as residual ear, which should be connected to the rib cartilage scaffold in this operation. However, it remains unclear whet

Treatment of traumatic separation of the auricular and annular cartilages without ablation: results in five dogs.

Five dogs were diagnosed as having unilateral separation of the otic cartilages (auricular and annular). Four dogs had sustained prior trauma (hit by a car). Clinical signs were typical of ear canal stenosis and included swelling at the base of the ear (n = 2), periotic fistulation (n = 2), head tilt toward the affected side (n = 2), and pain when opening the mouth (n = 1). Palpation revealed separation of the horizontal and vertical ear canals. Otoscopic findings included a shallow ear canal and a pseudotympanic membrane across the proximal end of the auricular cartilage. Findings at surgery

Effect of transducer placements on thresholds in ears with an abnormal ear canal and severe conductive hearing loss.

OBJECTIVES: Providing hearing compensation to patients with aural atresia is considerably challenging. Hearing aid transducers vibrating the aural cartilage (cartilage conduction; CC) have been devised, and hearing aids utilizing them (CC hearing aids) have quickly become a beneficial option for aural atresia in clinical applications. However, it remains unclear which placement (on the aural cartilage or mastoid) is beneficial to signal transmission. METHODS: This study included 35 patients (53 ears with an abnormal ear canal and severe conductive hearing loss) who were using CC hearing aids.

Surgery of Severe Cauliflower Ear Deformity.

A cauliflower ear is an auricular deformity characterized by thickened soft tissue and cartilage. A subperichondrial hematoma or fluid collection causes this malformation. As a result of being cut off from the perichondrium blood supply, the ear cartilage becomes ischemic, developing scar tissue, fibrous tissue, new cartilage overgrowth, or necrosis beneath the skin, resulting in a permanent alteration in the shape of the external ear resembling that of a cauliflower. To properly fix defects, the surgeon must grasp ear anatomy and be familiar with a variety of reconstructive alternatives. It r

Management of auricular hematoma and the cauliflower ear.

Acute auricular hematoma is common after blunt trauma to the side of the head. A network of vessels provides a rich blood supply to the ear, and the ear cartilage receives its nutrients from the overlying perichondrium. Prompt management of hematoma includes drainage and prevention of reaccumulation. If left untreated, an auricular hematoma can result in complications such as perichondritis, infection, and necrosis. Cauliflower ear may result from long-standing loss of blood supply to the ear cartilage and formation of neocartilage from disrupted perichondrium. Management of cauliflower ear in

Correction of lobule-type microtia: I. The first stage of costal cartilage grafting.

BACKGROUND: Recently, auriculoplasty with costal cartilage grafting has been successfully used for correcting microtia and creating a clearly refined contour and a natural appearance of the ear. However, several important problems remain unsolved in these techniques. The authors describe an improved technique for harvesting costal cartilage with minimal morbidity and a new procedure for fabricating a cartilage frame that ensures a refined shape and rigid structure of the constructed ear. METHODS: Costal cartilage is harvested directly with a chisel. This technique enables some of the cartilag

Cartilage tympanoplasty.

This chapter describes two techniques for cartilage reconstruction of the tympanic membrane: the perichondrium/cartilage island flap, which uses tragal cartilage, and the palisade technique, which uses cartilage from the tragus or cymba. The perichondrium/cartilage island flap is preferred for management of the atelectatic ear and the high-risk perforation. The palisade technique is preferred in cases of cholesteatoma and when ossicular reconstruction is needed in the malleus-present situation. Descriptions of the modifications that should be taken in response to specific surgical indications

Ossification of the external ear: a case report and review of the literature.

A rare case of idiopathic bilateral ossification of the auricular cartilage and cartilage of the external ear canal is reported. The hardening and thickening of the external ear canals with subsequent narrowing and impaired exposure resulted in symptomatic hearing loss because of the recurrent impediment of inspissated cerumen, the removal of which finally became impossible. Surgical resection of the cartilage external ear canal and tragus was indicated to provide a more permanent treatment of conductive hearing loss. Histological examination revealed complete replacement of the fibroelastic c

Surgical Correction of the Lying Ear Deformities.

BACKGROUND: In contrast to prominent ear, lying ear can be defined when the antihelix of the ear is excessively folded, i.e., the conchoscaphal angle is much less than 90°. In such case, ears may look smaller than the face. These patients want their ears to be exposed more. In Asia, there is also a tendency to prefer large and long ears. The objective of this study was to present a surgical method for correcting lying ear deformities. METHODS: From August 2017 to June 2018, 37 patients (72 ears) underwent surgery using our surgical method for lying ear deformities. After the cartilage was exp

The burned ear; possibilities and challenges in framework reconstruction and coverage.

Ear reconstruction remains a challenging procedure, especially in burn victims. The ear is particularly vulnerable to thermal injury because of its location and the thin integument. The thermal injury could subsequently include skin and the deeper located auricular cartilage framework. This type of injury could have long lasting mutilating effect not only because the ear's morphology is mainly related to this framework but also because it will not recover or regenerate once injured. Grafts of costal cartilage or synthetic materials might replace missing cartilage. However, the poor quality of
