An Eye for an Eye, An Ear for an Ear: A Midwestern Case Report of Vogt-Koyanagi-Harada Disease.

INTRODUCTION: Vogt-Koyanagi-Harada (VKH) disease is an autoimmune condition affecting both ocular and extraocular systems. This case highlights the need for research into the epidemiology and pathophysiology of VKH. CASE PRESENTATION: A 23-year-old cisgender Hispanic female presented to our tertiary care center with severe headache, eye pain, vision changes, photophobia, hearing loss with tinnitus, phonophobia, nausea, vomiting, and vertigo. She was diagnosed with VKH disease. DISCUSSION: This report shares a case of VKH disease in the Midwestern United States. A 2023 Northwestern University

AI-Assisted Detection Support for Middle Ear Diseases Using Multimodal Large Language Models.

Middle ear diseases, such as otitis media and middle ear effusion, are difficult to accurately detect in primary care. We developed an AI-powered system using Azure OpenAI's GPT-4 Vision, the first multimodal large language model (LLM) applied to analyze otoscopic images and detect ten middle ear conditions. Built with a Model-View-Controller (MVC) architecture, the system processes images in under 5 seconds and provides bilingual (English and Chinese) detection reports with confidence scores and treatment recommendations. This scalable solution integrates advanced image analysis and natural l

[Sniff-induced negative pressure--a cause for the development of middle ear diseases?].

The significance of "sniffing" causing tubal dysfunction and thus leading to the development of cholesteatoma is discussed. We examined 244 healthy ears (18% sniff-positive), 21 ears with a traumatic perforation of the tympanic membrane (10% sniff-positive), 46 ears with chronic mesotympanic otitis (17% sniff-positive) and 40 ears with cholesteatoma (60% sniff-positive). Two different methods were used: on intact tympanic membranes the pressure chamber impedance method described by Munker-Pedersen (1972), and for perforated tympanic membranes a pressure transducer. Our results show that: (1) "

Image-Based Artificial Intelligence Technology for Diagnosing Middle Ear Diseases: A Systematic Review.

Otolaryngological diagnoses, such as otitis media, are traditionally performed using endoscopy, wherein diagnostic accuracy can be subjective and vary among clinicians. The integration of objective tools, like artificial intelligence (AI), could potentially improve the diagnostic process by minimizing the influence of subjective biases and variability. We systematically reviewed the AI techniques using medical imaging in otolaryngology. Relevant studies related to AI-assisted otitis media diagnosis were extracted from five databases: Google Scholar, PubMed, Medline, Embase, and IEEE Xplore, wi

Efficient and accurate identification of ear diseases using an ensemble deep learning model.

Early detection and appropriate medical treatment are of great use for ear disease. However, a new diagnostic strategy is necessary for the absence of experts and relatively low diagnostic accuracy, in which deep learning plays an important role. This paper puts forward a mechanic learning model which uses abundant otoscope image data gained in clinical cases to achieve an automatic diagnosis of ear diseases in real time. A total of 20,542 endoscopic images were employed to train nine common deep convolution neural networks. According to the characteristics of the eardrum and external auditory

[Local antibacterial therapy for the inflammatory diseases of the external and middle ear].

The objective of the present study was to estimate the efficacy and safety of candibioic designed for the treatment of the patients presenting with diffuse otitis externa and chronic otitis media. The open randomized trial included 40 patients with diffuse otitis externa and 40 ones with chronic otitis media who had undergone a surgical sanitation procedure. Their standard clinical and otorhinolaryngological examination was supplemented by the characteristic of clinical symptoms and personal complaints using the 10-score scale. The composite preparation candibiotic was found to be equally effe

Middle ear diseases in relation to atopy and nasal metachromatic cells in infancy.

The relation between middle ear morbidity and atopy was prospectively studied in 44 infants with and 20 without a confirmed family history of atopy. Signs of atopy were recorded at 3, 6, 9 and 18 months of age and signs of middle ear disease were recorded and controlled as needed. Twenty-seven children developed definite signs of atopy, while 12 showed probable signs and 25 remained free from allergic symptoms. The number of episodes of acute otitis media was higher and the duration of episodes of otitis media with effusion were longer in 13 infants with respiratory tract allergy, as compared

Toynbee Memorial Lecture 1997. Middle ear mechanics in normal, diseased and reconstructed ears.

A review of the structure-function relationships in normal, diseased and reconstructed middle ears is presented. Variables used to describe the system are sound pressure, volume velocity and acoustic impedance. We discuss the following: (1) Sound can be transmitted from the ear canal to the cochlea via two mechanisms: the tympanoossicular system (ossicular coupling) and direct acoustic stimulation of the oval and round windows (acoustic coupling). In the normal ear, middle-ear pressure gain, which is the result of ossicular coupling, is frequency-dependent and smaller than generally believed.

Eyes in Ears: A Miniature Steerable Digital Endoscope for Trans-Nasal Diagnosis of Middle Ear Disease.

The aim of this work is to design, fabricate and experimentally validate a miniature steerable digital endoscope that can provide comprehensive, high-resolution imaging of the middle ear using a trans-nasal approach. The motivation for this work comes from the high incidence of middle ear diseases, and the current reliance on invasive surgery to diagnose and survey these diseases which typically consists of the eardrum being lifted surgically to directly visualize the middle ear using a trans-canal approach. To enable less-invasive diagnosis and surveillance of middle ear disease, we propose a

Continuous long-term measurements of the middle ear pressure in subjects without a history of ear disease.

A new method was used for continuous measurement of the middle ear (ME) pressure during a 24-h period. In 10 subjects without a history of ear disease a small perforation was made through the tympanic membrane. A tight rubber stopper containing a small polyethylene tube was fitted into the external ear canal. Conventional tubal function tests were performed. The equipment was then carried by the subjects for 24 h of normal activity to monitor any slow or rapid dynamic pressure change in the ME. Body position was found to be the most important factor affecting ME pressure variation, during the

Optimized fine-tuned ensemble classifier using Bayesian optimization for the detection of ear diseases.

External and middle ear diseases are common disorders, especially in children, and can be examined using a digital otoscope. Hearing loss can result from delayed diagnosis and treatment which is subjective and error-prone depending on the expertise of the otolaryngologist. For these reasons, deep learning-based automated diagnostic systems are highly needed. In this study, a novel weighted average voting ensemble classifier between MobileNet and DenseNet169 has been developed to diagnose and detect different ear conditions. Bayesian optimization was used to select hyperparameters that gave the
