Infant crying during aircraft descent.

The phenomenon of infant crying during aircraft descent is described, based on in-flight observations of 37 infant-mother pairs and mother interviews. The hallmark was crying during descent that was not alleviated by mothers' strategies that had been effective prior to descent. A significant relationship was found between bottle feeding and crying during descent; 18 (78%) of nonfeeding infants cried, compared with 4 (29%) of the bottle feeders. All infants with colds cried during descent, and descent crying always occurred more than 9 minutes after adults perceived the need to clear their ears

[Surgical treatment of otogenic pain and auditory symptoms in a patient with hemifacial spasms].

Hemifacial spasms (HFS) are involuntary contractions of the facial muscles and affect approximately ten per 100,000 persons. Our case describes a 43-year-old woman with HFS and simultaneous, ipsilateral auditory symptoms and otalgia. Under local anaesthetic, the stapes tendon was cut with a laser. Immediately after, the patient reported loss of auditory symptoms and ear pain. These symptoms were still absent at two- and 12-week follow-ups. Our case highlights a possible treatment for patients with similar symptoms that, to our knowledge, has not been described before.

Association of anatomical features of the petrotympanic fissure and presence of foramen of Huschke with otalgia and tinnitus.

The petrotympanic fissure (PTF) and foramen of Huschke (FH) are anatomical structures in the temporal bone that can connect the temporomandibular joint (TMJ) and the ear. The purpose of this retrospective study was to investigate the association between PTF morphology and otalgia and tinnitus, as well as the prevalence of the FH and otological symptoms, using cone beam computed tomography (CBCT). CBCT images from 114 patients presenting with symptoms of a temporomandibular disorder were examined retrospectively. The PTF was classified into three subtypes (open, semi-open, closed) and the prese

Rare Diseases of the Middle Ear and Lateral Skull Base.

Otalgia, otorrhea and hearing loss are the most common ear-related symptoms that lead to the consultation of an otolaryngologist. Furthermore, balance disorders and affections of the cranial nerve function may play a role in the consultation. In large academic centres, but also in primary care, the identification of rare diseases of the middle ear and the lateral skull base is essential, as these diseases often require interdisciplinary approaches to establish the correct diagnosis and to initiate safe and adequate treatments. This review provides an overview of rare bone, neoplastic, haematol

The receptive field for visceral pain referred orofacially by the vagus nerves.

BACKGROUND: The nociceptive receptive field of the vagus nerves in animals includes virtually the entire thoracic, abdominal and laryngopharyngeal regions. However, the role of the vagus nerves in the transmission of visceral pain in humans, with the exception of pain from coronary artery diseases, is believed to be insignificant. AIM: The purpose of this report is to map out the clinical visceral pain receptive field of the vagus nerves relative to its nociceptive counterpart in animals. MATERIALS AND METHODS: The PubMed database and PMC were searched for case reports of patients with orofa

A Case Report of High Cervical Spinal Cord Stimulation for Chronic Otalgia and Tinnitus.

Neuromodulation continues to grow in its scope and application in the relief of chronic and debilitating disorders. Both otalgia and tinnitus can be multifactorial in etiology, with diagnostic and treatment challenges. This is a case of spinal cord stimulator placement providing a patient with near total relief of otalgia, total relief of tinnitus, and mild improvement in sensorineural hearing loss. We believe that this is the first report showing benefit of high spinal cord stimulation in tinnitus, and we consider whether there are neuronal connections between upper cervical nerve roots and t

12 minute consultation: an evidence-based management of referred otalgia.

BACKGROUND: Otalgia is a common presenting symptom to the ENT clinic. The aetiology of otalgia can be divided into otological and referred depending on its underlying pathology. This article aims to provide a systematic and evidence-based method of managing a patient with referred otalgia. METHODS: The data in this article are based on a literature review performed on MEDLINE and EMBASE in January 2010. The keywords used included otalgia in combination with diagnosis, management, investigations and treatment. Articles in the English language were selected and reviewed. Suitable references fro

[Otalgia as presentation of primary nasopharynx tuberculosis].

We report a case of a 63-year-old woman who presented with left otalgia during more than six months. On examination, otoscopy was normal, whereas flexible nasopharyngoscopy revealed an asymmetry in the nasopharynx. CT scans confirmed the presence of a left nasopharyngeal mass. A transnasal endoscopic removal of the nasopharyngeal lesion was performed. Necrotizing granulomatosis was found. Tuberculosis of the nasopharynx was considered the final diagnosis, but oral treatment was not started. After 36 months of follow-up, nasal endoscopy and CT images show no sign of recurrence. Discussion is fo

Chronic paroxysmal hemicrania presenting as otalgia with a sensation of external acoustic meatus obstruction: two cases and a pathophysiologic hypothesis.

OBJECTIVE: To describe two cases of chronic paroxysmal hemicrania manifested by otalgia with a sensation of external acoustic meatus obstruction and to suggest that the trigeminal-autonomic reflex is a mechanism for the sensation of ear blockage. BACKGROUND: Maximum pain in chronic paroxysmal hemicrania is most often in the ocular, temporal, maxillary, and frontal regions. It is less often located in the nuchal, occipital, and retro-orbital areas. Review of the literature on chronic paroxysmal hemicrania found no reports of pain primarily localized to the ear and associated with a sensation o

Neuroborreliosis complicated by severe bilateral sensorineural hearing loss.

INTRODUCTION: Neuroborreliosis (NB) is a neurologic complication of Lyme disease that can cause sudden hearing loss, which can in some cases be bilateral. CASE REPORT: A 61-year-old woman consulted for bilateral sudden hearing loss with headache and earache, following an episode of fever. Interview found a skin lesion suggestive of migratory erythema, which had developed 3months previously after a walk in a German forest and had not initially been treated. Examination revealed severe sensorineural hearing loss and lymphocytic meningitis. Blood tests found long-standing IgM and IgG positive Bo

Isolation and characterization of novel antibacterial compound from an untapped plant, Stereospermum fimbriatum.

Stereospermum fimbriatum or locally known as "Chicha" is traditionally used for itchy skin, earache, stomachache and postpartum treatments. This study was designed to evaluate the antimicrobial potential of S. fimbriatum's stem bark against 11 pathogens and isolate its bioactive compound. Successive soxhlet extraction was conducted using n-hexane, dichloromethane (DCM) and methanol. Disc diffusion, minimum inhibitory and bactericidal concentration (MIC & MBC) assays were done to examine the antimicrobial activity. Bioassay-guided isolation was conducted on S. fimbriatum's extract. The DCM extr

[Case of atypical Ramsay-Hunt syndrome who presented with severe vertigo and vomiting].

We herein present a case of 6-year-old female demonstrating atypical Ramsay-Hunt syndrome. She presented with an earache, severe vertigo, and vomiting at onset, and thereafter, herpes zoster oticus appeared. No facial nerve palsy was seen. She was vaccinated for varicella and had no past history of apparent varicella infection. The patient showed positive IgG and negative IgM serum antibodies for varicella-zoster virus (VZV). An analysis of VZV-DNA from the patient's ear lesion using alleric discrimination real-time PCR identified a wild-type strain of VZV. We diagnosed her to have atypical Ra

Diagnosis and treatment of temporomandibular disorders.

Temporomandibular disorders (TMD) are a heterogeneous group of musculoskeletal and neuromuscular conditions involving the temporomandibular joint complex, and surrounding musculature and osseous components. TMD affects up to 15% of adults, with a peak incidence at 20 to 40 years of age. TMD is classified as intra-articular or extra-articular. Common symptoms include jaw pain or dysfunction, earache, headache, and facial pain. The etiology of TMD is multifactorial and includes biologic, environmental, social, emotional, and cognitive triggers. Diagnosis is most often based on history and physic

Saturation diving with heliox to 350 meters. Observation on hearing threshold, brainstem evoked response and acoustic impedance.

Four divers were compressed to 350 m to observe changes in hearing threshold, brainstem evoked response and acoustic impedance. The divers experienced no tinnitus, impairment of hearing, earache during compression. Examination showed that the threshold of lower frequency range of hearing was elevated because of the masking effect of the noise in the hyperbaric chamber. Changes in waveform and latency of brainstem evoked response were due to changes in sound wave transmission affected by the chamber pressure and a poor ratio of signal to noise in the hyperbaric environment with heliox. All thes

Cervical C2 osteoma, unusual presentation and transoral approach for complete excision.

BACKGROUND CONTEXT: Osteoid osteomas are benign tumors of the bone with 10% occurring in the spine. The authors discuss a case of a 70-year-old lady with an unusual presentation of a cervical C2 osteoma. The presentation was with mild dysphagia and pressure symptoms behind the left ear similar to earache experienced on an aircraft at high altitude with no hearing loss. PURPOSE: The authors postulate that with the appropriate preoperative imaging, operation, and postoperative care, patients with C2 body osteomas can make a good recovery. STUDY DESIGN: This is the case of a patient with a C2 o
